Tricuspid valve replacement is becoming more and more popular at various medical centres due to the increase in numbers of patients with tricuspid regurgitation. We report on a case of a 59-year-old man who had undergone tricuspid valve replacement with preservation of the native leaflets two years earlier, and developed early prosthetic dysfunction, which may have been caused by fusion of the native valve leaflets with the prosthetic valve leaflets. The experience of this case informs us that preserving the subvalvular apparatus may impede the motion of the prosthesis, and that adapting the individual morphology of the native tricuspid valve during tricuspid valve replacement could benefit the patient and avoid re-operation.
The rate of cardiac implantable electronic device (CIED) implantation in low- and middle-income countries is increasing. Patients recieving these devices are frequently older and with multiple co-morbidities, which may later lead to complications requiring CIED removal. CIED removals are associated with life-threatening complications. However, high sucesss rates are reported in high-income countries. The purpose of this study was to report on the experience of CIED removal in a resource-constrained setting.
In this retrospective study, we included consecutive adult patients admitted to Groote Schuur Hospital and the University of Cape Town Private Academic Hospital for CIED removal from 1 January 2008 to 31 December 2019.
During the study period, 53 patients underwent CIED removal (26 extractions and 27 explants). The patients had a mean (standard deviation) age of 59.1 (16.0) years. A history of systemic hypertension was present in 50.9% of patients, diabetes mellitus in 30.2% and dilated cardiomyopathication rate; results which are comparable to high-volume centres.
Left atrial appendage (LAA) morphology is a powerful predictor of thrombogenesis of the left atrium (LA) in patients with non-valvular atrial fibrillation (NVAF). However, it remains unknown whether LAA morphology is useful for stroke risk stratification in patients with NVAF.
A total of 555 atrial fibrillation patients were divided into thrombus and non-thrombus groups according to transoesophageal echocardiography. We analysed the correlation between LAA morphology and the CHADS

score and compared the ability to predict LA/LAA thrombosis of the CHADS

The odds ratio of non-chicken wing LAA morphology was 11.48. Non-chicken wing LAA morphology was significantly correlated with LA/LAA thrombosis. We incorporated LAA morphology into the CHADS
score and named it the L

score. The AUC of the L
CHADS
score (0.767) in predicting LA/LAA thrombosis was significantly higher than that of the CHADS
(0.558) or CHA
DS
-VASc scores (0.557). The positive and negative predictive values of the L
CHADS
score (13.1 and 98.7%) were higher than those of the CHADS
(8.7 and 94.2%) and CHA
DS
-VASc scores (6.9 and 6.9%).

Through a right anterior mini-thoracotomy, unmounted stents were successfully placed in the coarctant segment. One-year follow up did not show echocardiographic improvement but the patients were controlled on medical therapy. P-wave dispersion (PWD), which represents inhomogeneous atrial conduction and discontinuation of impulses, has been observed, when prolonged, to predict atrial fibrillation, particularly in the setting of hypertension. This study of PWD in 150 hypertensive patients and controls sought to determine the prevalence of PWD in Nigerian hypertensives and its relationship to left ventricular mass index and left ventricular function. Mean PWD in normal subjects was 32.14 ± 4.72 ms and was significantly shorter than that in hypertensive patients at 38.29 ± 8.02 ms. Herein, we report the first umpolung alkylation reaction of α-iminoesters with alkyl halides mediated by iminophosphorane as an organic superbase. The desired products were obtained in up to 82% yield with almost perfect regioselectivities. The key to the regioselectivity of this reaction was the use of 4-trifluoromethyl benzyl imines as a substrate. The products were successfully derivatised into the more functionalised molecules in good yields.
