This patient is an active and vigorous 79-year-old man who plays competitive tennis and pickleball. He steadfastly adheres to an antediluvian regimen for the management of his AF, but this may be viewed in the context of the famous quotation by Bert Lance, Director of the Office of Management and Budget in the US under President Carter who said "sometimes, if it ain't broke, don't fix it." In addition to the evolution of drug therapies from digoxin to beta-adrenergic blockers for rate control as well as from aspirin to warfarin to apixaban for the prevention of stroke, there have been other recent remarkable advances. For example, recent promising findings from randomized trials include that early rhythm control was more effective than rate control as well as that cryoballoon ablation was superior to drug therapies. These findings require confirmation in additional randomized trials designed a priori to test these promising but unproven hypotheses.Capecitabine is a well-established agent for adjuvant chemotherapy in breast and colorectal cancers. However, one of the limiting adverse events of this therapy is severe diarrhea, which is reported with increasing frequency as of late. Capecitabine-induced ileitis should be suspected in cases with severe, treatment-refractory diarrhea. We present a case of capecitabine-induced terminal ileitis in a patient who received the medication as adjuvant therapy for previously resected colon adenocarcinoma. Capecitabine-induced diarrhea secondary to ileitis is a severe adverse drug event, which occurs during adjuvant chemotherapy and does not respond to conservative treatment with antidiarrheals, often necessitating permanent drug withdrawal. A high index of suspicion is critical as the complications, such as dehydration and the associated electrolyte derangements, may be life-threatening if diagnosis and cause-specific treatment are delayed.Bariatric surgery is a potential modality for the management of obesity. Bariatric patients may experience skin disorders and hair loss postoperatively. A 24-year-old woman with polycystic ovarian syndrome-associated obesity successfully underwent bariatric surgery. Within seven weeks after surgery, she developed diffuse and progressive hair loss, characteristic of telogen effluvium. Alopecia following bariatric surgery may be acute in onset, occurring within the first three months and often associated with telogen effluvium. In addition, bariatric surgery postoperative hair loss may be the result of nutritional deficiencies; in this setting, it is often chronic in onset, occurring six months after surgery. Also, hair loss in bariatric patients may be multifactorial in etiology. We introduced an acronym to facilitate the description of patients who experience bariatric surgery-induced telogen effluvium Bar SITE.Diabetes mellitus and its complication such as heart disease, stroke, kidney failure, etc. is a serious concern all over the world. Hence, monitoring some important blood parameters non-invasively is of utmost importance, that too with high accuracy. This paper presents an in-house developed system, which will be helpful for diabetes patients with Chronic Kidney Disease (CKD) to monitor blood urea and glucose. This manuscript discusses a comparative study for the prediction of blood urea and glucose using Backpropagation Artificial Neural Network (BP- ANN) and Partial Least Square Regression (PLSR) model. The NVIDIA Jetson Nano board controls the five fixed LED wavelengths in the Near Infrared (NIR) region from [Formula see text] to [Formula see text] with a constant emission power of 1.2 mW. The spectra for 57 laboratory prepared samples conforming with major blood constituents of the blood sample were recorded. From these samples, 53 spectra were used for training/calibration of the BP-ANN/PLSR model and the remaining 4 samples were used for validating the model. The PLSR model predicts blood urea and glucose with a Root Mean Square Error (RMSE) of 0.88 & 12.01 mg/dL, Coefficient of Determination R2 = 0.93 & R2 = 0.97, Accuracy of 94.2 % and 90.14 %, respectively. To improve the prediction accuracy, BP-ANN model is applied. Later the Principal Component Analysis (PCA) technique was applied to these 57 spectra values. These PCA values were used to train and validate the BP-ANN model. After applying the BP-ANN model, the prediction of blood urea & glucose improved remarkably, which achieved RMSE of 0.69 mg/dL, R2 = 0.96, Accuracy of 95.96 % for urea and RMSE of 2.06 mg/dL, R2 = 0.99, and Accuracy of 98.65 % for glucose. The system performance is then evaluated with Bland Altman analysis and Clarke Error Grid Analysis (CEGA).
A combination of gas chromatography-electroantennographic detection (GC-EAD) and gas chromatography-mass spectrometry (GC-MS) is typically used to screen active compounds that play a role in the regulation of insect behavior. This method uses two kinds of gas chromatography (GC) equipment and needs to compare compounds between the two chromatograms, and it is tedious and costly. To improve detection efficiency, as well as reduce costs and the rate of missed detection, we designed a system connecting gas chromatography (GC), electroantennography (EAG), and mass spectrometry (MS), with MS used instead of the flame ionization detector (FID) as the GC-EAD detector. To verify the feasibility of the improved method, we compare two methods-GC-EAG-MS and GC-EAD-through a series of experiments. Some researchers made similar improvements, but these were not compared with GC-EAD, and their method needed to be improved in the synchronization and split ratio. Our method has been optimized and improved in these aspects.
