Future research should assess the feasibility and impact of integrating postpartum counseling into pediatric visits.BACKGROUND Human milk is the preferred source of infant nourishment; yet, acceptance of human milk as the norm for infant feeding is debated. In the United States there is a disparity in breastfeeding rates, which are significantly lower in African Americans compared to Whites. Multiple variables appear to influence breastfeeding, including the male partner. OBJECTIVES To assess and compare breastfeeding exposure, attitude, and knowledge between African American and White college males with no biological children. A second objective was to explore the relationship between breastfeeding variables and educational attainment, academic major, and sibling presence. METHODS A subset of responses from a larger cross-sectional survey were analyzed. African American and White collegiate males aged 18-40 years without biological children were included in the study. The questionnaire included demographic, exposure, attitude, and knowledge items. RESULTS A total of 117 African American and 713 White males were included and represented all academic colleges on campus. African American participants documented less exposure to breastfeeding, a more positive attitude, and similar poor knowledge scores compared to their White counterparts. Sibling presence was linked with exposure, and exposure was linked to attitude. Both African American and White science majors scored higher on all factors than humanities or business majors. CONCLUSIONS FOR PRACTICE Breastfeeding exposure influences males' attitudes toward breastfeeding. Future research should consider types of exposure to identify the sources most influential. Furthermore, tailoring breastfeeding interventions to increase exposure and knowledge in core wellness courses may improve attitudes and social acceptance among collegiate males.BACKGROUND Hashimoto thyroiditis (HT) is an autoimmune lymphocytic thyroiditis and is the most common form of thyroid inflammatory diseases. The association of HT with papillary thyroid carcinoma (PTC) has been described. PTC is the most common form of malignancy associated with HT. When papillary carcinoma develops on top of Hashimoto thyroiditis, the disease tends to be less aggressive and lymph node and extra-thyroidal invasion are infrequent. RESULTS We retrospectively examined the pathological features of our patients who were diagnosed with concomitant HT and thyroid cancer. In Egyptian patients, PTC was the main type of malignancy associated with HT (96.2%) and was often multifocal (46.2%). In contrast to the published literature, lymph node invasion and extra-thyroidal extension were as frequent in association with HT as in other cancer cohorts. We also observed the frequent occurrence of Hürthle cell metaplasia (23.1%) and the appreciable incidence of aggressive histological types of PTC (32%). CONCLUSION Thyroid carcinoma with HT may have some aggressive features in areas with endemic goiter background.PURPOSE Patellar dislocation is a serious complication leading to patient morbidity following total knee arthroplasty (TKA). The cause can be multifactorial. Extensor mechanism imbalance may be present and result from technical errors such as malrotation of the implants. We sought to understand the reasons for post-arthroplasty patellar dislocation and the clinical outcomes of patients in whom it occurs. METHODS This is a retrospective cohort study assessing the outcomes of revision surgery for patellar dislocation in patients with component malrotation in both primary and revision TKAs. Patient demographics, dislocation etiology, presurgical deformity, intraoperation component position, complications, reoperation, and Knee Society Scores (KSS) were collected. RESULTS Twenty patients (21 knees) were identified. The average time from primary arthroplasty to onset of dislocation was 33.6 months (SD 44.4), and the average time from dislocation to revision was 3.38 months (SD 2.81). Seventeen knees (80.9%) had internal rotation of the tibial component and seven knees (33.3%) had combined internal rotation of both the femoral and tibial components. Fifteen knees (71.4%) were treated with a condylar constrained implant at the time of revision, and five knees were converted to a hinged prosthesis. The average follow-up time was 56 months. During this time, one patient (4.54%) had a recurrent dislocation episode, requiring further surgery. At final follow up, the mean KSS was 86.2. CONCLUSION Revision TKA following patellar dislocation for patients with malrotated components was associated with high success rates. After revision surgery, patients had a low recurrence of patellar dislocation, low complication rates, and excellent functional outcomes.PURPOSE OF REVIEW Recent years have seen the development of gene expression profiling and next-generation sequencing in diffuse large B cell lymphoma (DLBCL), leading to a more defined characterization of this disease into distinct subentities. The genomic era has ushered in the possibility of using precision guided therapy, in part based on targeting genes with somatic mutations. Such precision-targeted therapies will ultimately reduce the need for chemotherapy, induce fewer adverse events, and likely enhance the cure rate for these patients. Here, we discuss emerging therapeutic strategies that have been recently developed for the upfront and relapse setting of DLBCL. RECENT FINDINGS Clinical trials exploring precision medicine have showed promising results; however, attempts to enhance frontline immunochemotherapy by adding targeted agents to the R-CHOP backbone did not confirm the expected benefit.
