Clinical importance lies in distinguishing hibernoma from other benign and malignant breast neoplasms, as well as inflammatory conditions that come into the histologic or radiologic differential. Laparoscopic surgeries are becoming more popular in many surgical procedures particularly in pediatric age group. The physical responses and hemodynamic changes that occur during laparoscopic surgery differ between adults and pediatrics.
We report a sudden hemodynamic instability and abrupt ECG changes associated with pneumomediastinum (PM) during laparoscopic Nissen fundoplication surgery in a 19 months old boy who had gastro-esophageal reflux disease. Procedure was converted to open surgery and all parameters were spontaneously returned back to normal values. The patient was successfully extubated then transferred to intensive care unit (ICU) for close monitoring. He had uneventful post-operative course where PM has been completely resolved in the repeated chest x-ray.

We present a case series of three patients with advanced cervical cancer who either refused the standard of care systemic or chemoradiation treatment or did not benefit from it.
We treated patients with isolated hypoxic pelvic perfusion (HPP).
Two patients achieved complete clinicopathologic response and one patient required surgical excision of the necrotic residual mass containing no viable cancer cells. There were no long-term systemic or local side effects. All patients are cancer free for up to 15 years after conclusion of treatment.
HPP is an effective option for treatment of advanced cervical cancer that generates rapid and onlasting remissions at low side effects. Gynecologic oncologists shall be aware of HPP to facilitate wider adaption of our technique.

Polyorchidism is a rare congenital malformation with about 200 cases reported in literatures. Triorchidism and left side occurrence are the most common presentation. This case report was aimed to report the rare presentation of polyorchidism which was on the right side and along with incarcerated inguinal that make it by far the least encountered presentation. The intraoperative management for the polyorchidism was an issue of dilemma but adult type can be excised safely.
A 43 year old farmer presented with right side inguino-scrotal mass and recent failure to reduce to abdomen. He used to have pain lower abdominal pain with slight pressure since childhood but didn't get any medical evaluation. He has completed his family number and didn't complain any sexual problem. Physical examination showed that there was non-tender irreducible soft right side inguino-scrotal mass. The right side testis was not appreciated.
