It was notable that indicator composed of IL-6 and neutrophils/lymphocytes ratio (NLR) occupied the superior position of Area Under Curve (AUC) values in cancer cases, especially in patients with solid tumors (AUC = 0.85). The newly-found indicator could also be referred as an independent risk factor, which provided us with novel clues on the investigation of more reliable and affordable clinical indexes in tumor prediction.Introduction Polycythemia vera is a chronic hematologic malignancy frequently presented with constitutional symptoms and associated with an increased risk of thrombosis, hemorrhage, and progression to myelofibrosis or acute myeloid leukemia. Current treatment strategies reduce thrombohemorrhagic risk by controlling blood counts and inhibiting platelets, but often fail to address disease-related symptoms or biologically modify the disease.Areas covered We review the current paradigm for treating polycythemia vera, highlight areas of unmet need, review therapeutic agents in late stage clinical development, and provide an overarching view of how these emerging agent may fit into the future armamentarium of polycythemia vera treatments.Expert opinion The shift from focusing solely on secondary prevention of thrombohemorrhagic events to a comprehensive treatment strategy that additionally aims to improve quality of life and prevent disease progression has resulted in a rapidly evolving therapeutic landscape that promises to move the treatment of polycythemia vera out of antiquity into the modern age.
A newly diagnosed HIV patient with tuberculosis-like symptoms was confirmed with disseminated nocardiosis involving the central nervous system (CNS). This case report provides novel insights into the diagnosis and treatment of nocardiosis.
A 44-year-old male newly diagnosed HIV patient presented on March 20, 2017, with the major symptoms of fever and headache. Computed tomography (CT) of the chest showed a mass in the lower lobe of the right lung. Brain magnetic resonance imaging (MRI) revealed multiple abnormal signals and ring enhancement. Blood culture suggested the presence of mycobacteria and positive acid-fast staining, and the strain was identified as
. He was administered an anti-infective therapeutic regimen consisting of cefotaxime sodium and sulbactam sodium (2.25g, qid, iv), linezolid (0.6g, bid, iv), moxifloxacin (0.4g, qd, iv), sulfamethoxazole (1.44g, qid, per os), and amikacin (0.4g, qd, iv). After treatment, fever and skin nodules resolved, and reexamination by chest CT and brain MRI suggested improvement compared with the pretreatment situation.
At present, there are few reports of disseminated
infection involving the CNS in HIV patients in China. This case report will help to enhance the understanding of disseminated
infection. In the process of clinical diagnosis and treatment,
may be initially misdiagnosed as disseminated TB. In the present case, tuberculosis-like symptoms actually corresponded to disseminated nocardiosis involving the CNS.
At present, there are few reports of disseminated Nocardia infection involving the CNS in HIV patients in China. This case report will help to enhance the understanding of disseminated Nocardia infection. In the process of clinical diagnosis and treatment, Nocardia may be initially misdiagnosed as disseminated TB. In the present case, tuberculosis-like symptoms actually corresponded to disseminated nocardiosis involving the CNS.Metformin has been in clinical use for more than half a century, yet its molecular mechanism of action is not entirely understood. Metformin has been shown to have antiproliferative and synergistic effects on various types of cancers. The anticancer effects of metformin are potentially applicable to both diabetic and nondiabetic patients. Areas of ongoing investigation focus on metformin's ability to activate adenosine monophosphate kinase (AMPK), in addition to its effect on Myc mRNA, monocarboxylate transporter 1 (MCT1), hypoxia-inducible factor 1 (HIF1), mammalian target of rapamycin (mTOR), and human epidermal growth factor receptor 2 (HER2). Additional anticancer effects are exhibited by acting on liver kinase B1 (LKB1), CREB-regulated transcription coactivator 2 (CRTC2), nitric oxide, and reactive oxygen species. Further investigation will be focused on elucidating metformin's metal-binding properties and how they may be harnessed for their anticancer effect. buy Thapsigargin The acquired knowledge about metformin properties has expanded the number of targets for drug discovery such as microRNA, hexokinase, adenylate cyclase, transcription factors, various cyclins, and copper. In order to design anticancer drugs that mimic metformin's mechanism of action, binding assay studies must be conducted to fully understand and utilize the AMPK-dependent and independent mechanisms. Metformin's complex mechanisms that can potentially make this drug a multifaceted therapy targeting tumorigenesis in addition to information from ongoing clinical trials implicate that metformin can be a potential chemotherapeutic drug or adjuvant that could prove to be vital to future strategies against several types of cancer.
Cirrhosis is an irreversible disease that replaces healthy tissues with scar tissues. According to the Pakistan Medical Research Council's national general population survey (2008-2014), the ratio of cirrhosis was 4.8% among the population.
To assess the nutritional status of cirrhotic patients on enteral and parenteral nutrition and to assess the degree of malnutrition among cirrhotic patients.
A cross-sectional study was conducted at Shalamar and Shiekh Zayed Hospitals of Lahore from January-April 2017. The convenient sampling technique was used. Inclusion criteria were the patients with decompensated liver cirrhosis, whereas compensated patients were excluded from the study. Data were collected through the patient's file, nurse's, and dietitian's notes. The nutritional status of participants was assessed based on socio-demographic characteristics, anthropometric measurements, biochemical assessment, clinical signs, and dietary recall. Variables were categorized and compared to observe the level of significance in enteral and parenteral nutrition.
