Objectives The processing of seafood (fish and shellfish) for human consumption can lead to health consequences, including occupational asthma (OA). Several non-UK studies have reported both respiratory outcomes and airborne levels of major allergens in seafood processing. However, there is a paucity of such evidence in the UK land-based seafood processing sector, which employs some 20 000 workers. Methods University of Manchester's Surveillance of Work-related and Occupational Respiratory Disease (SWORD) reporting system has been interrogated over the period 1992-2017 to define the incidence rate of OA cases that can be ascribed to the UK land-based processing sector, and the seafood species implicated. Airborne allergen monitoring data undertaken at Health and Safety Executive's laboratory from 2003 to 2019 have also been collated. Results The estimated annual OA incidence rate in seafood processors was 70 [95% confidence intervals (CIs) 48.9, 91.1] per 100 000 workers compared with 2.9 (95% CIs 2.8, 3.1) ie to major seafood allergens can occur in this industry. Further investigation of current levels of respiratory ill-health and the sources of allergen exposure are warranted.Objectives The misuse of pesticides among farmworkers in Nepal is commonplace. To address this, we implemented a pilot educational intervention (three modules delivered over 3 days and lasting approximately 3 h each) in Kavre District of Nepal. Modules included (i) health and environmental effects of pesticides, (ii) use of personal protective equipment, and (iii) label literacy and behavioral factors that influence pesticide exposure. In addition, 10 posters with key messages from each of the modules were hung throughout communities. Practices relating to pesticides at baseline and endline were compared using multivariable logistic regression to adjust for differences in demographic and socioeconomic characteristics between the samples. Results Compared with the baseline sample, farmworkers in the endline sample were significantly more likely to report getting information regarding the amount of pesticides to use from experts or pesticide labels (versus personal judgment); wearing gloves while mixing pesticides; wearing boots while working in the field; using personal hygiene practices after handling pesticides such as bathing or washing hands and feet; changing clothes after handling pesticides; checking the wind direction before spraying; and delaying entry for a longer period of time after spraying. Conclusions These results suggest that a simple educational intervention can improve pesticide handling practices among farmworkers in Nepal. Future research should explore the impact of such interventions on pesticide exposure levels and health outcomes, and the potential to scale up these programs nationally.Background Spouses often attempt to influence patients' diabetes self-care. Spousal influence has been linked to beneficial health outcomes in some studies, but to negative outcomes in others. Purpose We aimed to clarify the conditions under which spousal influence impedes glycemic control in patients with type 2 diabetes. Spousal influence was hypothesized to associate with poorer glycemic control among patients with high diabetes distress and low relationship quality. Methods Patients with type 2 diabetes and their spouses (N = 63 couples) completed self-report measures before patients initiated a 7-day period of continuous glucose monitoring. Mean glucose level and coefficient of variation (CV) were regressed on spousal influence, diabetes distress, relationship quality, and their two- and three-way interactions. Results The three-way interaction significantly predicted glucose variability, but not mean level. Results revealed a cross-over interaction between spousal influence and diabetes distress at high (but not low) levels of relationship quality, such that spousal influence was associated with less variability among patients with low distress, but more among those with high distress. Among patients with high distress and low relationship quality, a 1 SD increase in spousal influence predicted a difference roughly equivalent to the difference between the sample mean CV and a CV in the unstable glycemia range. Conclusions This was the first study to examine moderators of the link between spousal influence and glycemic control in diabetes. A large effect was found for glucose variability, but not mean levels. These novel results highlight the importance of intimate relationships in diabetes management.A dataset of knee kinematics in healthy, uninjured adults is needed to serve as a reference for comparison when evaluating the effects of injury, surgery, rehabilitation, and age. Most currently available datasets that characterize healthy knee kinematics were developed using conventional motion analysis, known to suffer from skin motion artifact. More accurate kinematics, obtained from bone pins or biplane radiography, have been reported for datasets ranging in size from 5 to 15 knees. The aim of this study was to characterize tibiofemoral kinematics and its variability in a larger sample of healthy adults. Thirty-nine knees were imaged using biplane radiography at 100 images/s during multiple trials of treadmill walking. Multiple gait trials were captured to measure stance and swing phase knee kinematics. 6DOF kinematics were determined using a validated volumetric model-based tracking process. A bootstrapping technique was used to define average and 90% prediction bands for the kinematics. The average ROM during gait was 7.0 mm, 3.2 mm, and 2.9 mm in AP, ML and PD directions, and 67.3°, 11.5° and 3.7° in FE, IE, and AbAd. Continuous kinematics demonstrated large inter-knee variability, with 90% prediction bands spanning approximately ±4 mm, ±10 mm, and ±5 mm for ML, AP, and PD translations and ±15°, ±10°, and ±6° in FE, IE, and AbAd. This dataset suggests substantial variability exists in healthy knee kinematics. This study provides a normative database for evaluating knee kinematics in patients who receive conservative or surgical treatment.
