Objective To evaluate episode-based payments for upper respiratory tract infections (URI) and perinatal care in Arkansas's Medicaid population. Study setting Upper respiratory infection and perinatal episodes among Medicaid-covered individuals in Arkansas and comparison states from fiscal year (FY) 2011 to 2014. Study design Cross-sectional observational analysis using a difference-in-difference design to examine outcomes associated with URI and perinatal episodes of care (EOC) from 2011 to 2014. Key dependent variables include antibiotic use, emergency department visits, physician visits, hospitalizations, readmission, and preventive screenings. Data collection Claims data from the Medicaid Analytic Extract for Arkansas, Mississippi, and Missouri from 2010 to 2014 with supplemental county-level data from the Area Health Resource File (AHRF). Principal findings The URI EOC reduced the probability of antibiotic use (marginal effect [ME] = -1.8, 90% CI -2.2, -1.4), physician visits (ME = 0.6, 90% CI -0.8, -0.4)onsequences.Tertiary hyperparathyroidism is a common cause of hypercalcemia after kidney transplantation (KT) and has been associated with renal dysfunction, bone mineral density loss, and increased risk of fracture and cardiovascular events. In a previous 12-month clinical trial, we demonstrated that subtotal parathyroidectomy was more effective than cinacalcet for controlling hypercalcemia. In the current study, we retrospectively evaluate whether this effect is maintained after 5 years of follow-up. In total, 24 patients had data available at 5 years, 13 in the cinacalcet group and 11 in the parathyroidectomy group. At 5 years, 7 of 11 patients (64%) in the parathyroidectomy group and 6 of 13 patients (46%) in the cinacalcet group (P = .44) showed normocalcemia. However, recurrence of hypercalcemia was only observed in the cinacalcet group (P = .016). Subtotal parathyroidectomy retained a greater reduction in intact parathyroid hormone (iPTH) compared with cinacalcet group. However, such correlation has not been fully investigated in Asian patients. In this study, the association of CNs with CD was evaluated in a multicenter investigation. Methods We enrolled 5633 patients who underwent both colonoscopy and esophagogastroduodenoscopy due to annual follow-up, screening for positive occult blood testing and abdominal symptoms between January 2016 and December 2017 at 3 institutions. The relationship between presence of CNs and CD was investigated, and predictors for presence of CNs were determined by multivariate logistic analysis. Results The enrolled patients consisted of 1799 (31.9%) with CD (average age 70.0 years, male 64.0%) and 3834 without CD (66.0 years, male 52.9%), with the coexistence rates of CNs in those groups 46.6% and 44.2%, respectively (P=0.090). Predictors for early colon cancer were shown to be age (OR 1.02, 95% CI 1.01-1.04, P=0.010), laxatives use (OR 1.76, 95% CI 1.17-2.64, P=0.007), gastric neoplasms (OR 2.16, 95% CI 1.23-3.81, P=0.008), and CD (OR 1.64, 95% CIs 1.16-2.31, P=0.005). Early colon cancer in the distal colon was most frequently detected in patients with right-sided CD (RR 2.50, P=0.001). Conclusion In Japanese patients, early colon cancer was more frequently found in those with as compared to without CD. The presence of CD may be an important indicator for an index colonoscopy examination to detect colon cancer. (Clinical-trial-registry UMIN000038985).We have read with great interest the article by Fernandez-Nieto et al. showing histopathological pictures of one of the two biopsied cases from their 24 patients diagnosed of vesicular lesions related to COVID-19.1 They additionally performed a real-time PCR for SARS-CoV-2 that was negative in four tested cases.1 Regarding their case histopathological pictures, we agree with their description as we can observe acantholytic cells, some of them with dyskeratotic features or grouped without a clear molding of nucleus but reminiscent of the multinucleated giant cells typically observed in herpetic lesions, a picture that looks to our eyes, most likely as an infrequent entity named pseudoherpetic Grover disease (GD),2 first described as vesicular GD by Fernandez-Figueras.3.Background A substantial body of research has emerged suggesting that depression is strongly linked to poor physical health outcomes, which may be partly due to increased allostatic load across stress response systems. Interestingly, health risks associated with depression are also borne by the offspring of depressed persons. Our aim was to simultaneously investigate whether maternal depression is associated not only with increased allostatic load across cardiac control, inflammation, cellular aging, but also if this is transmitted to adolescent children, possibly increasing the risk for early onset of psychiatric conditions and disease in these offspring. Methods A preregistered, case-control study of 180 low-income mothers (50% mothers depressed, 50% mothers nondepressed) and their adolescent offspring was conducted to determine how depressed mothers and their adolescent offspring systematically differ in terms of autonomic, sympathetic, and parasympathetic cardiac control; inflammation; cellular aging; and behavioral health in offspring, which are indicators suggestive of higher allostatic load.
