We present a 52-year-old male with chronic complete tetraplegia who presented to the emergency room with unremitting autonomic dysreflexia (AD). He was admitted for medical management of the AD when a source of the AD could not be identified. After an extensive workup, the patient was found to have bilateral sacral insufficiency fractures. He subsequently underwent sacroplasty with immediate and full resolution of AD symptoms and return to full premorbid function within 24 h.

Although it is not life-threatening, the accompanying rapid and irregular ventricular rate can lead to hemodynamic deterioration and obvious symptoms, especially the risk of cerebrovascular embolism. Our study aimed to identify novel and promising genes that could explain the underlying mechanism of AF development. MATERIAL AND METHODS Expression profiles GSE41177, GSE79768, and GSE14975 were acquired from the Gene Expression Omnibus Database. R software was used for identifying differentially expressed genes (DEGs), and Gene Ontology and Kyoto Encyclopedia of Genes and Genomes enrichment analyses were subsequently performed. A protein-protein interaction network was constructed in Cytoscape software. Next, a least absolute shrinkage and selection operator (LASSO) model was constructed and receiver-operating characteristic curve analysis was conducted to assess the specificity and sensitivity of the key genes. RESULTS We obtained 204 DEGs from the datasets. The DEGs were mostly involved in immune response and cell communication. The primary pathways of the DEGs were related to the course or maintenance of autoimmune and chronic inflammatory diseases. The top 20 hub genes (high scores in cytoHubba) were selected in the PPI network. Finally, we identified 6 key genes (FCGR3B, CLEC10A, FPR2, IGSF6, S100A9, and S100A12) via the LASSO model. CONCLUSIONS We present 6 target genes that are potentially involved in the molecular mechanisms of AF development. In addition, these genes are likely to serve as potential therapeutic targets.BACKGROUND In patients admitted to the hospital owing to suspected acute coronary syndrome and where the first electrocardiogram (ECG) is non-diagnostic of ST-elevated myocardial infarction (STEMI), international guidelines recommend that sequential ECGs are performed to identify new or intermittent ST-deviations. Continued monitoring for ST-segment alterations is rarely performed, in contrast to the continued monitoring for arrhythmias. Continued monitoring for ST-segment alteration may detect intermittent ST-segment elevations not captured by routine 12-lead ECGs. We present a case in which continued ST-segment monitoring revealed intermittent ST-segment elevations and led to expedited coronary angiography and revascularization. CASE REPORT A 66-year-old woman was admitted to a regional hospital in Denmark with intermittent retrosternal chest pain and dyspnea. Her risk profile included a family history of cardiovascular disease, hypercholesterolemia, active smoking, and obesity. click here The results of 3 sets of high-sensitivity cardiac troponins and sequential 12-lead ECG measurements were normal. Because of clinical suspicion of unstable angina, the patient remained admitted. Continued rhythm and ST-segment monitoring was initiated. During continued ST-segment monitoring, asymptomatic intermittent ST-elevations in the inferior leads were detected. The patient was referred for an acute coronary angiography at the nearest cardiac center. Coronary catheterization showed right coronary artery stenosis, and complete revascularization was obtained by percutaneous coronary intervention with stent implantation. CONCLUSIONS Continued ST-segment monitoring can detect intermittent ST-segment elevations and thereby substantiate the need for acute coronary angiography.
Boxing is a popular combat sport in which competitors may sustain high impact blows to the face. For this reason, they are at high risk for craniofacial injuries; however, data on facial injuries specific to boxing remains sparse. Studies on safety measures, such as headgear, to prevent such injuries in boxing have been inconclusive. Boxing is popular with a wide audience. However, there is no consensus on safety measures across different populations involved in boxing due to lack of data. The objective of this study is to characterize the demography and incidence of injury types of patients presenting to emergency departments with boxing-related craniofacial injuries on a national scale in order to facilitate the establishment of evidence-based safety guidelines for prevention of boxing-related injuries.
The National Electronic Injury Surveillance System database was searched for boxing-related craniofacial injuries from the last 10 years (2010-2019). Injuries involving boxing were isolated and organizede required to develop algorithms for management of boxing-related craniofacial injuries and to evaluate the safety and efficacy of protective equipment such as boxing headgear on concussions.
