RESULTS When 40 radiomics features were used in the random forest classification, in-house software achieved superior sensitivity (1.00) and accuracy (0.992) compared to the commercially available research prototype (sensitivity = 0.950, accuracy = 0.968). When the number of features was reduced to five features, diagnostic performance of the in-house software decreased to sensitivity (0.950), specificity (0.923), and accuracy (0.936). Diagnostic performance of the commercially available research prototype was unchanged. selleck inhibitor CONCLUSION Commercially available and in-house radiomics software achieve similar diagnostic performance, which may lower the barrier of entry for radiomics research and allow more clinician-scientists to perform radiomics research.AIM To compare clinico-radiological pattern of non-cirrhotic versus cirrhotic HCC and correlate them with histopathological tumor grade. MATERIALS AND METHODS This prospective study was carried out on 94 patients enrolled following ultrasound diagnosis of a liver mass measuring > 3 cm. Multiphasic MDCT was performed on all treatment-naïve cases and 56 cases with imaging pattern consistent with unifocal HCC were selected. Background liver parenchyma was assessed on ultrasound for cirrhosis and NAFLD. Cases were categorized into cirrhotic liver (CL) and non-cirrhotic liver (NCL) groups with 26 and 30 cases, respectively, and guided biopsy of each liver mass was performed. AFP levels were compared in both groups. Serum markers for hepatitis B and C were assessed. Masses in both groups were compared for morphology, attenuation on each phase and washout time. Presence of capsule, corona enhancement, satellite nodules and portal vein invasion was noted. RESULTS AFP level was higher in CL group. HBV serum marker was raised in both groups. Most HCCs in NCL were moderately differentiated (histopathology), larger, had well-defined margins, showed mosaic pattern of enhancement, complete capsule and delayed phase washout. Majority in CL group were poorly differentiated, smaller, had ill-defined margins, showed heterogeneous enhancement, absent capsule and portal venous phase washout. Time of washout correlated with histopathological differentiation of masses, with earlier washout indicating poorer differentiation. CONCLUSION HCCs in NCL have different clinico-radiological characteristics than HCCs in CL. Time of contrast washout correlates with histopathological grade of HCC. Non-cirrhotic NAFLD may require formulation of new screening guidelines for HCC.BACKGROUND Wilms' tumor (WT) is the most common renal malignant tumor of childhood. Metastatic WT has a worse prognosis than localized disease. This study aims to assess the clinical outcome and different prognostic factors that influence treatment outcome of pediatric metastatic WT cases treated at National Cancer Institute (NCI), Egypt, between January 2008 and December 2015. Medical records were retrospectively reviewed for clinical, radiological and histopathological data, treatment received, and survival outcome. RESULTS In the specified study period, 24/103 (23.3%) patients with WT were metastatic at presentation. The mean age was 5.25 ± 2.87 years (range 2.0-12.7). Abdominal swelling/mass was the commonest presentation (70.8%). Only 3 patients (12.5%) had combined lung and liver metastases while 21 patients (87.5%) had pulmonary-only metastases. All patients had favorable histology tumors with no anaplasia. Nine patients (37.5%) underwent upfront nephrectomy. Majority of patients (91.7%) had local stage III disease. Surgical complications were reported in 4 patients; 3 of them had up-front nephrectomy. Only 7/21 patients achieved rapid complete response of pulmonary nodules after 6 weeks of chemotherapy (CTH), and they had a better survival outcome. Patients were followed up till December 2017. Thirteen patients (54.1%) experienced events during the study period including 5 relapses, 6 cases with disease progression, and 2 patients died out of sepsis. The 3-year event-free and overall survival rates were 48.2% and 54.2%, respectively. CONCLUSION Neo-adjuvant CTH followed by delayed nephrectomy seems more suitable approach in our institute. Pulmonary response to neo-adjuvant CTH appears to be a strong predictor for outcome.Human tissue plasminogen activator was the first recombinant therapy protein that successfully produced in Chinese hamster ovary cells in 1986 and approved for clinical use. Since then, more and more therapeutic proteins are being manufactured in mammalian cells, and the technologies for recombinant protein production in this expression system have developed rapidly, with the optimization of both upstream and downstream processes. One of the most promising strategies is expression vector cassette optimization based on the expression vector cassette. In this review paper, these approaches and developments are summarized, and the future strategy on the utilizing of expression cassettes for the production of recombinant therapeutic proteins in mammalian cells is discussed.The latest WHO report estimates about 1.6 million global deaths annually from TB, which is further exacerbated by drug-resistant (DR) TB and comorbidities with diabetes and HIV. Exiguous dosing, incomplete treatment course, and the ability of the tuberculosis bacilli to tolerate and survive current first-line and second-line anti-TB drugs, in either their latent state or active state, has resulted in an increased prevalence of multidrug-resistant (MDR), extensively drug-resistant (XDR), and totally drug-resistant TB (TDR-TB). Although a better understanding of the TB microanatomy, genome, transcriptome, proteome, and metabolome, has resulted in the discovery of a few novel promising anti-TB drug targets and diagnostic biomarkers of late, no new anti-TB drug candidates have been approved for routine therapy in over 50 years, with only bedaquiline, delamanid, and pretomanid recently receiving tentative regulatory approval. Considering this, alternative approaches for identifying possible new anti-TB drug candidates, for effectively eradicating both replicating and non-replicating Mycobacterium tuberculosis, are still urgently required. Subsequently, several antibiotic and non-antibiotic drugs with known treatment indications (TB targeted and non-TB targeted) are now being repurposed and/or derivatized as novel antibiotics for possible use in TB therapy. Insights gathered here reveal that more studies focused on drug-drug interactions between licensed and potential lead anti-TB drug candidates need to be prioritized. This write-up encapsulates the most recent findings regarding investigational compounds with promising anti-TB potential and drugs with repurposing potential in TB therapy.
