PAX6 is a transcription factor involved in embryonic development of many organs, including the eyes and the pancreas. Mutations of PAX6 gene is the main cause of a rare disease, congenital aniridia (CA). This case-control study aims to investigate the effects of PAX6 mutations on glucose metabolism and insulin secretion in families with CA.

In all, 21 families with CA were screened by Sanger sequencing. Patients with PAX6 mutations and CA (cases) and age-matched healthy family members (controls) were enrolled. Oral glucose tolerance test (OGTT) was performed to detect diabetes or impaired glucose tolerance (IGT). Insulin and proinsulin secretion were evaluated.

Among 21 CA families, heterozygous PAX6 mutations were detected in five families. Among cases (n=10) from the five families, two were diagnosed with newly identified diabetes and another two were diagnosed with IGT. Among controls (n=12), two had IGT. The levels of haemoglobin A1c were 36±4mmol/mol (5.57±0.46%) and 32±5mmol/L (5.21±0.54%) in the cases and the controls, respectively (p=0.049). More importantly, levels of proinsulin in the cases were significantly higher than that of the controls, despite similar levels of total insulin. The areas under the curve of proinsulin in the cases (6425±4390) were significantly higher than that of the controls (3709±1769) (p=0.032).

Patient-prosthesis mismatch (PPM) is associated with poor outcomes after aortic valve replacement. The aim of this study was to assess the accuracy of indexed effective orifice area (EOAi) charts in predicting PPM after transcatheter aortic valve replacement (TAVR).

A retrospective review of 346 TAVR patients from January 2017 to November 2018 was performed. EOAi was predicted for patients based on published predictive tables using valve type, annulus diameter, and body surface area. . The accuracy of predicted PPM was assessed. Differences in clinical outcomes, including mean gradient, length of stay, mortality, complications, and change in Kansas City cardiomyopathy questionnaire score as an indicator of quality of life, were evaluated based on actual PPM.

Of the 346 patients analyzed, 44 (12.7%) of patients had PPM on intraoperative TEE. Of the 182 patients who received Sapien 3 valves, 42 (23.1%) were predicted to have PPM while 25 (13.7%) had actual PPM. EOAi charts had poor sensitivity (40.0% for Sapien 3; 5.25% for Evolut) and positive predictive value (23.8% for Sapien 3; 33.3% for Evolut) for both valve types.

Saline agglutination tests (SATs) are widely recommended for diagnosis of immune-mediated hemolytic anemia in dogs, but there are frequent false-positive results.

Specificity of SATs will improve at higher saline-to-blood ratios.

Prospective diagnostic accuracy study. Immune-mediated hemolysis (IMH) was considered present if a gel direct antiglobulin test (DAT) was positive and there was clinical evidence of hemolysis (n = 9), absent if another mechanism for anemia was identified and the DAT was negative or there was no hemolysis (n = 138), and if IMH status was unclear, dogs were excluded (n = 3). Saline agglutination tests were prepared at 1 1, 4 1, 9 1, and 49 1 saline-to-blood ratios, and microscopic agglutination was considered a positive result.

Specificity for IMH increased from 29% (95% confidence interval 20-38) at a 1 1 dilution to 97% (93-99) at a 49 1 dilution. Sensitivity was 88% (47-100) at 1 1 and 4 1 dilutions and 67% (30-93%) at 9 1 and 49 1 dilutions. Diagnostic accuracy increased from 33% (24-42) at 1 1 dilution to 95% (90-98) at 49 1 dilution.

If performed using a 49 1 saline-to-blood ratio, SATs achieve high specificity for IMH. Based on a gold standard of positive DAT and evidence of hemolysis, lower saline-to-blood ratio results should not be used because false-positive results are common.
