Infantile nephrotic syndrome is a rare disorder which is frequently caused by genetic defects. A 7-month-old girl presented with fever, loose stools and anasarca and was diagnosed with nephrotic syndrome. Work-up for a genetic cause was negative. Cytomegalovirus polymerase chain reaction (CMV PCR) was positive and the infant was treated with ganciclovir for 6 weeks, followed by valganciclovir for 10 weeks. All symptoms resolved within 2 weeks of commencing treatment and she attained complete remission within 4 weeks. CMV PCR was negative within 4 weeks of antiviral therapy. At 18 months follow-up she remained well. Appropriate treatment of infantile nephrotic syndrome secondary to CMV should result in recovery.
The link between the dietary pattern known as the Mediterranean diet (MedDiet) and lower morbidity/mortality is well known, and its efficacy in the primary prevention of cardiovascular diseases has been proven in recent years. However, adherence to the MedDiet seems to be related to socioeconomic status. The objective was to analyze whether their adherence to the MedDiet differs from the rest of the working population. For this purpose, three bacterial strains (Pseudomonas kilonensis MB490, Pseudomonas kilonensis MB498 and Pseudomonas sp. MB504), isolated from cotton fields of Mianwali, Pakistan were investigated for Triazophos degradation and metabolite formation in M-9 broth, soil slurry and soil microcosm after incubation for 9 days. There was 88.4-95.8% Triazophos degradation in M-9 broth, 99.90% degradation in soil slurry and 92.74 to 96% Triazophos degradation in soil microcosm by these bacteria after 9 days. While there was negligible Triazophos degradation (upto 7%) in the controls without bacteria. According to GCMS analysis, 7 unique and novel metabolites (1, 2, 4-Triazole-4-amine, N-(2-Thienylmethyl), Benzene sulfonic acid hydrazide, Benzene sulfonic acid methyl ester, 4H-1,2,4-Triazole-4-benzenesulfonamide, 4, 5 dihydro-N-(O-toyl)-3-furamide, Ethyl 4-phenyldiazenylbenzoate and Dibutyl methanephosphonate) of Triazophos were revealed. Current results strongly suggest the potential of these bacterial strains for the remediation of Triazophos contaminated agricultural soils.
Hyperbilirubinaemia is a common cause of hospital admission of newborn infants; however, maternal visual assessment of jaundice may reduce unnecessary hospital visits.
To investigate the validity of maternal visual assessment of neonatal jaundice to identify infants with hyperbilirubinaemia requiring phototherapy or who have significant hyperbilirubinaemia ≥239.4µmol/L (14 mg/dL).
A prospective study of the diagnostic accuracy of maternal visual assessment of jaundice was conducted at a university hospital in Bangkok. Mothers were trained to assess for neonatal jaundice using their infant's palms as a skin colour reference. Trained mothers who were blinded to transcutaneous bilirubin or serum bilirubin values assessed their infants and reported 'jaundice' or 'no jaundice', and determined jaundice severity using dermal icterus zones. Sensitivity and negative predictive values were used to assess the validity of visual assessment for neonatal jaundice.

The associated risks are needed for optimizing cervical cancer screening. To estimate HPV type-specific prevalence, odds ratio (OR), and positive predictive value (PPV) for cervical cytological abnormalities, we determined 41 different HPV genotypes in cervical samples from a population-based sample of 8351 women aged 18-51 years before HPV vaccination era (V501-033; NCT01077856). Prevalence of HPV16 was 4.9% (95% CI 4.4-5.5) with the PPV for high-grade cytology 11.2%, and OR 11.9 (95% CI 8.5-16.5). Carcinogenic HPVs included in the nonavalent vaccine (HPV16,18,31,33,45,52,58) had a population prevalence of 14.4% (95% CI 13.5-15.4), with PPV of 8.0% (95% CI 6.8-9.3) and OR 23.7 (95% CI 16.0-63.5) for high-grade cytology. HPV types currently included in most screening tests, but not vaccinated against (HPV35,39,51,56,59,66,68) had a joint prevalence of 8.5% (95% CI 7.8-9.2) with PPV of 4.4% (95% CI 3.3-5.7) and OR of 2.9 (95% CI 2.0-4.0) for high-grade cytology. The other 27 non-carcinogenic genotypes had a prevalence of 11.8%, PPV of 2.9% (95% CI2.1-3.9), and OR 1.5 (95% CI 1.1-2.2.) for high-grade cytology. These results suggest that HPV screening tests in the post-vaccination era might perform better if restricted to the HPV types in the nonavalent vaccine and screening for all 14 HPV types might result in suboptimal balance of harms and benefits.Nowadays, as life expectancy grows, the healthcare industry faces growing challenges related to corresponding increases in chronic diseases. Home care services (HCS) are the solution to this growing problem. It's a general premise that information and communication technology (ICT) can address these health issues and enhances HCS. The scope of our study was the active managerial and supervisory roles of these technologies within HCS. The study aimed to extract, accumulate, and classify the challenges of using active ICT for elderly HCS. We employed the keywords, their synonyms, and their combinations into the searching areas of title, keywords, and abstract. More than 300 resources were collected, and found those 33 articles of those 33 articles were eligible for our study. Later, a team of experts provided their opinions on our gatherings, which were collected individually. According to the expert team's opinions, researchers classified challenges into; technology, human factors, and management.Objective To study the prevalence of revision cochlear implant study in a tertiary care referral center. To assess the various indications and surgical outcomes of revision cochlear implant surgery. Methods A retrospective chart review of revision cochlear implant surgery done from June 1997 to December 2019. All the surgeries that were done in children of 12 years and below were included. The revision surgeries were either with or without explantation and reimplantation. The causes included were device failures, electrode extrusion/malposition, magnet migration, persistent foreign body reaction, facial twitching, wound infection, and cholesteatoma. Results A total of 1636 pediatric cochlear implantation surgery were performed during the study period of 22 years. There were 94 (5.7%) revision surgeries done for various indications during this period. Out of them, 67 patients (71.3%) had device failure thus being the commonest indication for revision surgery, followed by infection in 12.8% of the total patients. Among the total revision, 81 (86.2%) patients had explantation and reimplantation of the new device. Conclusion Indications of revision cochlear implant surgery are manifold that can be either device-related or patient-related. Revision surgery needs a highly skilled and experienced team of surgeons, audiologists, and habilitationists for achieving optimal results.In this study, three kinds of modified basalt fibre (MBF) were used as biofilm carrier to treat wastewater, mainly for the removal of organic matter and nutrient pollutants, which was evaluated the feasibility of modification by pollutants removal performance. Polydopamine modified basalt fibre (PAD-BF) via the surface coating method were obtained. Polyacrylamide modified basalt fibre (PAM-BF) and maleic anhydride-modified basalt fibre (MAH-BF) via the surface grafting method were prepared. The surface physicochemical properties, biomass attachment capacity and pollutants removal efficiency of MBF were systematically investigated. Electron microscope scanning (SEM) revealed that the surface roughness of BF was obviously improved by modification. Besides, fourier transform infrared (FTIR) suggested that the MBF had more surface-active functional groups. The results of sludge immobilization tests showed that PDA/PAM/MAH-BF had higher bio-affinity than ordinary BF with 1.5∼2.3 times on immobilization ratio of microorganisms (IRM). Furthermore, the performances of PDA/PAM/MAH-BF as biofilm carrier for pollutants were significantly higher than that of ordinary BF group. Among them, the highest removal efficiency of COD in PAD-BF biofilm reactor was 95.29 ± 0.99%, while that of BF group was 86.30 ± 3.09%. PAM-BF group had the best removal effect of nutrients with the removal efficiency of 90.83 ± 7.69% for TP and 91.25 ± 6.43% for TN, respectively, while the removal rate of BF group was only about 70%. The improvement of dissolved oxygen (DO) in the MBF reactors was consistent with the enhancement of contaminant removal. Therefore, PDA/PAM/MAH-BF can be used as promising biological carrier fillers in wastewater treatment engineering.The outbreak of COVID-19 was first experienced in Wuhan City, China, during December 2019 before it rapidly spread over globally. This paper has proposed a mathematical model for studying its transmission dynamics in the presence of face mask wearing and hospitalization services of human population in Tanzania. Disease-free and endemic equilibria were determined and subsequently their local and global stabilities were carried out. The trace-determinant approach was used in the local stability of disease-free equilibrium point while Lyapunov function technique was used to determine the global stability of both disease-free and endemic equilibrium points. Basic reproduction number, R 0 , was determined in which its numerical results revealed that, in the presence of face masks wearing and medication services or hospitalization as preventive measure for its transmission, R 0 = 0.698 while in their absence R 0 = 3.8 . This supports its analytical solution that the disease-free equilibrium point E 0 is asymptotically stable whenever R 0 1 . Therefore, this paper proves the necessity of face masks wearing and hospitalization services to COVID-19 patients to contain the disease spread to the population.This study investigates the effectiveness of SBBR with low-cost textile fibers-based bio-carrier namely polypropylene fibers for the treatment of real sewage. The influent loading rates of COD, TN, and TP were averaged at 0.2780, 0.0170, and 0.0077 kg/m3.d, respectively. The removal efficiencies of BOD, COD, TN, and TP recorded in SBBR were 98%, 93%, 82%, and 44%, respectively at an aeration time of 4 h. The TN and TP removal achieved in SBBR were 2.05 and 2.75 times, respectively higher than SBR. The COD removal efficiency was more than 90% under all SRT conditions (10, 14, 18, 22, and 26 d) in SBBR, and the highest efficiency of 93% was obtained at an SRT of 22 days. As the SRT increased, the nitrogen and phosphorus removal decreased, because the denitrification rate and phosphorus release and uptake rate decreased at longer SRT. Simultaneous nitrification and denitrification (SND) efficiency was 85% in SBBR and 44% in SBR, indicating the co-existence of aerobic nitrifiers and anoxic denitrifiers in the biofilm reactor.
