This regulatory approach is presented by its advocates as a win-win for all parties involved; as bringing family law in line with the changed social reality of creating families; and as maintaining a non-permissive approach toward commercial surrogacy. In this article, we critically examine these claims. Our analysis suggests that the proposed systems may result in the facilitation of surrogacy practices with commercial, commodifying and exploitative dimensions. Objective To assess the performance of a severity scoring system on admission CXR in predicting hospital outcomes in patients admitted with COVID-19. Methods This retrospective study included 240 patients (142 men, 98 women; median age 65 [50-80]) admitted to the hospital from 3/16/2020 to 4/30/2020 with COVID-19 confirmed by real-time reverse transcriptase-polymerase chain reaction and who underwent CXR within 24 hours of admission. Three chest radiologists and three radiology residents independently scored patients' admission CXR using a 0 to 24-point composite scale (sum of 0-3 scores for extent and severity of disease in upper and lower lung zones). OBJECTIVE To assess whether 10 or more administrations of the mcGBCA gadobutrol is associated with increased signal intensity (SI) in the dentate nucleus (DN) and globus pallidus (GP) on unenhanced T1-weighted imaging (T1WI) in children and to explore clinical variables potentially associated with T1 hyperintensity. METHODS Twenty-five children (13 boys, 12 girls; mean age 7±4 years, range 2-18 years) who underwent at least 10 (mean 15±6; range 10-34) contrast-enhanced MRI examinations using exclusively gadobutrol, and 25 age- and sex-matched control patients without gadolinium exposure who underwent MRI, were identified. Two observers in consensus assessed visual T1 hyperintensity in the DN and GP using a 3-point scale. One observer placed ROIs on T1WI in the DN, G, p=0.36; r=0.047, p=0.82), or cumulative mcGBCA dose (r=0.434, p=0.07; r=0.270, p=0.19). Interobserver and intraobserver agreement was substantial for DN-to-MCP SI and GP-to-TH SI ratios (Ro 0.931-0.974). CONCLUSION Ten or more serial gadobutrol administrations was not associated with T1 hyperintensity in the DN or GP in children. CLINICAL IMPACT Selection of gadobutrol as an MRI contrast agent may reduce risk of gadolinium retention in children. The findings may help guide practices for GBCA administration in children.The American College of Radiology (ACR) Thyroid Imaging Reporting and Data System (TI-RADS) is an ultrasound-based risk stratification system (RSS) for thyroid nodules that was released in 2017. Since publication, research has shown that ACR TI-RADS has a higher specificity than other RSSs and reduces the number of unnecessary biopsies of benign nodules compared with other systems by 19.9-46.5%. The risk of missing significant cancers using ACR TI-RADS is mitigated by the follow-up recommendations for nodules that do not meet criteria for biopsy. In practice, after a nodule's ultrasound features have been enumerated, the ACR TI-RADS points-based approach leads to clear management recommendations. Practices seeking to implement ACR TI-RADS must engage their radiologists in understanding how the system addresses the problems of thyroid cancer overdiagnosis and unnecessary surgeries by reducing unnecessary biopsies. This review compares ACR TI-RADS to other RSSs and explores key clinical questions faced by practices considering its implementation. We also address the challenge of reducing interobserver variability in assigning ultrasound features.
