The long-term risk of definitive stoma after sphincter-saving resection (SSR) for rectal cancer is underestimated and has never been reported for ultralow conservative surgery. We report the 10-year risk of definitive stoma after SSR for low rectal cancer.

From 1994 to 2008, patients with low rectal cancer who were suitable for SSR were analysed retrospectively. Patients were divided into the following four groups low colorectal anastomosis (LCRA); coloanal anastomosis (CAA); partial intersphincteric resection (pISR); and total intersphincteric resection (tISR). The end-point was the risk of a definitive stoma according to the type of anastomosis.

During the study period, 297 patients had SSR for low rectal cancer. The incidence of definitive stoma increased from 11% at 1year to 22% at 10years. The reasons were no closure of the loop ileostomy (4.7%), anastomotic morbidity (6.5%), anal incontinence (8%) and local recurrence (5.2%). The risk of definitive stoma was not influenced by type of surgery 26% vs 18% vs 18% vs 19% (P=0.578) for LCRA, CAA, pISR and tISR, respectively. Independent risk factors for definitive stoma were age >65years and surgical morbidity.

To enhance our knowledge of the diversity of microalgae, a phycological survey of the Canary Islands (Spain) was undertaken. Here we report the discovery of a (semi)terrestrial green filamentous alga isolated from a steep volcanic canyon on La Palma. This alga is continually exposed to changing weather conditions (floods vs. droughts) and thus provides a good opportunity to investigate possible adaptations to a semiterrestrial habitat with large fluctuations of environmental parameters.

Most bird-dispersed fruits are green when unripe and become colored and conspicuous when ripe, signaling that fruits are ready to be consumed and dispersed. The color pattern for fruits of Miconia albicans (Melastomataceae), however, is the opposite, with reddish unripe and green ripe fruits. We (1) verified the maintenance over time of its bicolored display, (2) tested the communicative function of unripe fruits, (3) tested the photoprotective role of anthocyanins in unripe fruits, and (4) verified whether green ripe fruits can assimilate carbon.

Using a paired experiment, we tested whether detection of ripe fruits was higher on infructescences with unripe and ripe fruits compared with infructescences with only ripe fruits. We also measured and compared gas exchange, chlorophyll a fluorescence, and heat dissipation of covered (to prevent anthocyanin synthesis) and uncovered ripe and unripe fruits.

Although the bicolored display was maintained over time, unripe fruits had no influence on bird detection r than in unripe fruits, we could not confirm the photoprotective role of anthocyanins in unripe fruits. Cellulose is the most abundant biomolecule on the planet, yet the mechanism by which it is synthesized by higher plants remains largely unknown. In Arabidopsis thaliana (L.) Heynh, synthesis of cellulose in the primary cell wall requires three different cellulose synthase genes (AtCesA1, AtCesA3, and AtCesA6-related genes [AtCesA2, AtCesA5, and AtCesA6]). The multiple response expansion1 (mre1) mutant contains a hypomorphic AtCesA3 allele that results in significantly shorter, expanded roots. Crosses between mre1 and another allele of AtCesA3 (constitutive expression of VSP1, cev1) yielded an F1 with roots considerably longer and thinner than either parent, suggesting intragenic complementation. The F2 generation resulting from self-crossing these F1 showed three different root phenotypes roots like mre1, roots like cev1, and roots like the F1.

Patients with PCOS and healthy women were recruited. Patients were subdivided into obese and nonobese based on body mass index. Patients with PCOS were also grouped according to homoeostasis model assessment-insulin resistance (HOMA-IR) ≥ 2·69 or <2·69, and by PCOS phenotype. Patients with PCOS-IR were treated with a 6-month course of cyproterone acetate/ethinyloestradiol with or without concomitant metformin.

Inflammatory markers were examined at baseline, and after 6 months of treatment.

A total of 445 women with PCOS (mean age 25·9 ± 2·7 years; 298 obese, 147 nonobese) and 213 normal controls (mean age 24·9 ± 3·0 years) were included. Regardless of obesity status, testosterone, free androgen index (FAI), luteinizing hormone/follicle-stimulating hormone (LH/FSH) ratio, HOMA-IR, TSP-1 and NF-κB in the PCOS groups were significantly higher than in the control group, whereas TSP-1 was lower in the PCOS groups (all, P < 0·05).
