Open fractures are unique in the urgency they impart to the injury and the therapeutic challenge they pose. Non-union and infection are among the major concerns. Open tibial and femoral shaft fractures are among the commonest orthopaedic urgencies. Primary intramedullary (IM) nailing requires a skillset and knowledge of the principles of open fracture management and also a well-equipped operating room with the necessary implants and instruments. In a low resource setting, one or more of these prerequisites may not be met, and the outcome may thus be affected similarly. The objective of this study was to study these outcomes.

We prospectively studied primary IM nailing of open 1, 2 and 3A tibial and femoral fractures done in our hospital, in a simple non-modular operating room without an image intensifier.

There were 106 fractures in 104 patients 93 tibial and 13 femoral. The mean time from injury to surgical debridement was 19.6h. Mean duration of antibiotics therapy was 4.3days, and the mean duration of hospital stay was 4.1days. All patients were followed up to union. Mean time to radiographic union was 6.1months for the tibia and 5.7months for the femur. Non-union was seen in 4 tibial and 1 femoral fractures. There were 3 superficial infections but no deep infection.

The outcomes were comparable to, and in some cases better than, those found in existing literature. We conclude that primary IM nailing in open long bone fractures can be performed in austere environments with good success, in expert hands if the principles are adhered to.

The availability of water from the source depends on numerous climatic and glacier parameters. One key parameter is mass balance, which helps to assess the glacier health and future water availability. We have used the improved accumulation area ratio (IAAR) method to estimate mass balance in Alaknanda and Bhagirathi basins, constituting 1055 glaciers covering ~1609 km2. The mean Equilibrium Line Altitude (ELA) of the Alaknanda and Bhagirathi basins are estimated as 6147 ± 130 and 5985 ± 130 m.a.s.l, respectively. The mass balance is estimated using the accumulation area ratio (AAR)-mass balance relationship. The mean specific mass balance of the Alaknanda and Bhagirathi for 2001-2013 is estimated as -1.1 ± 0.03 m.w.e.a-1 and -1.01 ± 0.07 m.w.e.a-1, respectively. Total mass loss from the study area is calculated as ~21.4 ± 1.1Gt during this period. The loss of glaciers in the mountain area will increase the vulnerability of communities living in the region. It suggests a need for better adaptation strategies to improve the resilience of high mountain communities.Ceratitis capitata (Wiedemann) (Diptera Tephritidae), known as Medfly, is a severe agricultural invasive pest in Argentinian fruit-producing regions. The native habitat disturbance and introduction and spread of exotic host plants strongly favored Medfly proliferation. This scenario is common throughout the northern subtropical citrus-growing region. Environmentally friendly strategies to suppress Medfly populations by the National Fruit Fly Control and Eradication Program have currently been taken. One of these actions involves augmentative biological control through releases of the exotic parasitoid Diachasmimorpha longicaudata (Ashmead) (Hymenoptera Braconidae). Consequently, the hypothesis that the effectiveness of D. longicaudata females, from two different population lines, in controlling Medfly larvae progressively increases as the density of released females increases was tested. One parasitoid line derives from larvae of a biparental Medfly strain. The other hails from irradiated larvae of the genetic sexing Temperature Sensitive Lethal Vienna-8 medfly strain reared at the "BioPlanta San Juan" biofactory. Parasitoids foraged for 24 h on peaches artificially inoculated with naked lab-reared biparental Medfly larvae. Peaches were placed near the roof or on the ground in field cages. Five treatments (20, 40, 80, 160, and 320 females released) and a control (no parasitoids) for each population line were carried out throughout summer and autumn 2016. Host density (200 larvae) remained constant. At 320 released parasitoid females, both D. longicaudata population lines highly increased the Medfly mortality in both testing seasons, and foraged skillfully on peaches at both fruit height levels. These data encourage the application of augmentative biological control against Medfly.Eibesfeldtphora tonhascai Brown (Diptera Phoridae) is a natural enemy of Atta sexdens Linnaeus and Atta laevigata Smith and is considered a promising candidate for the biological control of these ants. The aim of this study was to verify if E. tonhascai has a preference for specific sizes of A. sexdens foragers and whether this parasitoid demonstrated host species preference when comparing parasitism of A. sexdens and A. laevigata. Worker ants were classified into four different size groups and introduced into an attack arena with one E. tonhascai female. To verify host species preference, both A. sexdens and A. laevigata workers were placed in the attack arena together with one E. tonhascai female. A no-choice test was also performed, when each ant species was offered to a phorid female at different times. Eibesfeldtphora tonhascai performed the highest number of inspection flights, attempted attacks, and actual attacks against A. sexdens workers with a cephalic capsule size of 3-5 mm. When comparing attacks on A. sexdens and A. laevigata workers, they were equally inspected, attacked, and the total parasitism rates were similar. However, the phorid adult emergence rate was significantly higher in the A. sexdens parasitized workers. We conclude that E. tonhascai females express preference for parasitizing larger hosts (cephalic capsule width of 3-5 mm), but do not display a preference for host species when comparing A. sexdens versus A. laevigata, and thus can similarly contribute to both leafcutter ants' natural biological control.The neurobiological and neurochemical mechanisms underlying the pathophysiology of bipolar disorder are complex and not yet fully understood. From circadian disruption to neuroinflammation, many pathways and signaling molecules are important contributors to bipolar disorder development, some specific to a disease subtype or a cycling episode. Pharmacological agents for bipolar disorder have shown only partial efficacy, including mood stabilizers and antipsychotics. The purinergic hypothesis for bipolar disorder emerges in this scenario as a promising target for further research and drug development, given its role in neurotransmission and neuroinflammation that results in behavioral and mood regulation. Here, we review the basic concepts of purinergic signaling in the central nervous system and its contribution to bipolar disorder pathophysiology. Allopurinol and novel P2X7 receptor antagonists are promising candidates for treating bipolar disorder. We further explore currently available pharmacotherapies and the emerging new purinergic targets for drug development in bipolar disorder.Older people often suffer from different types of ulcers, with the most prevalent being chronic leg ulcers (CLUs) and diabetic foot ulcers. There are major issues in the current medical approach because these ulcers are hard to heal, and, in the case of CLUs, very painful and with a high frequency of relapse. Older people complain of pain more frequently than young people, frequently due to a combination of painful chronic wounds with other comorbidities (e.g. arthritis, peripheral arterial disease, vertebrogenic pain syndrome). However, chronic pain appears to be managed better by older people because the pain sensitivity is downregulated and the pain threshold is higher in older people. Pain management of chronic wounds is often insufficient, especially in older individuals. It is highly important to use non-traumatic wound dressings and pay attention to patients' feelings and fears because pain in chronic ulcers can impair wound healing. Key factors include good preparation for dressing change and adequate analgesia, ideally a combination of topical and oral agents.

Psychotropic medicine utilisation in older adults continues to be of interest because of overuse and concerns surrounding its safety and efficacy.

This study aimed to characterise the utilisation of psychotropic medicines in older people in New Zealand.

We conducted a repeated cross-sectional analysis of national dispensing data from 1 January, 2005 to 31 December, 2019. We defined utilisation using the Anatomical Therapeutic Chemical classification defined daily dose system. Utilisation was measured in terms of the defined daily dose (DDD) per 1000 older people per day (TOPD).

Overall, the utilisation of psychotropic medicines increased marginally by 0.42% between 2005 and 2019. The utilisation increased for antidepressants (72.42 to 75.21 DDD/TOPD) and antipsychotics (6.06-19.04 DDD/TOPD). In contrast, the utilisation of hypnotics and sedatives (53.74-38.90 DDD/TOPD) and anxiolytics decreased (10.20-9.87 DDD/TOPD). The utilisation of atypical antipsychotics increased (4.06-18.72 DDD/TOPD), with the terest to understand the utilisation of antidepressants and antipsychotic medicines during the coronavirus disease 2019 pandemic year.

To examine data from studies supporting the clinical efficacy of medical approaches from India traditional systems of medicines like Ayurveda, Unani, Siddha, and Homeopathy for psoriasis using outcome indicators employed in clinical practice and research.

Searches were conducted between December 2019 and September 2020 in databases PubMed, Scopus, Web of Science and Ovid Medline using search terms including traditional, complementary, psoriasis, Kushtha, Ayurveda, Siddha, Unani, Homeopathy and clinical. Controlled trials, case series and case reports published from India were included.

Data of 17 selected studies were extracted. Treatment efficacy in terms of improvement in Psoriasis Area and Severity Index (PASI) score or/and percentage reduction in score (PASI 50, PASI 75 and PASI 90) or/and patient-reported outcomes using instruments like Dermatology Life Quality Index and Psoriasis Disability Index were noted. All studies reported good improvement as per the study specific outcome. find more However, study characteristics, including study design, sample size, follow-up period, inclusion and exclusion criteria were heterogeneous, and the choice of outcome measures was not adequate to conclude the effectiveness of intervention.
