In feature modeling, we propose an affinity module to measure frame-specific similarities among neighboring frames and dynamically attend to informative neighbors when calculating temporal convolution. Experiments on three benchmarks demonstrate the high performance of our BackTAL.Text based Visual Question Answering (TextVQA) is a recently raised challenge requiring models to read text in images and answer natural language questions by jointly reasoning over the question, textual information and visual content. Introduction of this new modality - Optical Character Recognition (OCR) tokens ushers in demanding reasoning requirements. Most of the state-of-the-art (SoTA) VQA methods fail when answer these questions because of three reasons (1) poor text reading ability; (2) lack of textual-visual reasoning capacity; and (3) choosing discriminative answering mechanism over generative couterpart (although this has been further addressed by M4C). In this paper, we propose an end-to-end structured multimodal attention (SMA) neural network to mainly solve the first two issues above. SMA first uses a structural graph representation to encode the object-object, object-text and text-text relationships appearing in the image, and then designs a multimodal graph attention network to reason over it. Finally, the outputs from the above modules are processed by a global-local attentional answering module to produce an answer splicing together tokens from both OCR and general vocabulary iteratively by following M4C.Objective The feasibility of conducting in vivo non-localized 31P Magnetic Resonance Spectroscopy (MRS) with a 1.0T extremity scanner and the potential to increase accessibility of this important diagnostic tool for low cost applications is revisited. Methods This work presents a custom transmit-only quadrature birdcage, four-element receive coil array, and spectrometer interfaced to a commercial ONI 1.0T magnet for enabling multi-channel, non-1H frequency capabilities. A custom, magnetic resonance compatible plantar flexion-extension exercise device was also developed to enable exercise protocols. The coils were assessed with bench measurements and 31P phantom studies before an in vivo demonstration. Results In pulse and acquire spectroscopy of a phantom, the array was found to improve the signal-to-noise ratio (SNR) by a factor of 1.31 and reduce the linewidth by 13.9% when compared to a large loop coil of the same overall size. selleck chemical In vivo testing results show that two averages and a four second repetition time for a temporal resolution of eight seconds was sufficient to obtain phosphocreatine recovery values and baseline pH levels aligned with expected literature values. Conclusion Initial in vivo human skeletal muscle 31P MRS allowed successful monitoring of metabolic changes during an 18-minute exercise protocol. Significance Adding an array coil and multinuclear capability to a commercial low-cost 1.0T extremity scanner enabled the observation of characteristic 31P metabolic information, such as the phosphocreatine recovery rate and underlying baseline pH.Goal To develop a low-cost magnetic resonance imaging (MRI)-free transcranial focused ultrasound (FUS) system for microbubble-mediated therapy. Methods A 128-element 11 MHz array for skull localization was integrated within a 256-module multi-frequency (306/612/1224 kHz) dual-mode phased array. The systems transcranial transmit and receive performance was evaluated with ex-vivo human skullcaps using phase aberration corrections calculated from computed tomography (CT)-based simulations via ultrasound-based (USCT) and landmark-based (LMCT) registrations, and a gold-standard fixed source emitter (FSE)-based method. Results Displacement and rotation registration errors of 1.4 0.4 mm and 2.1 0.2 were obtained using USCT, resulting in sub-millimeter transmit targeting errors driven at 306 kHz (0.9 0.2 mm) and 612 kHz (0.9 0.3 mm), and source localization errors of 1.0 0.3 mm and 0.6 0.2 mm at receive frequencies of 306 kHz and 612 kHz, respectively (mean SD). Similar errors were obtained using LMCT and no significant differences between these two approaches were found on either transmit (p = 0.64/0.99) or receive (p = 0.45/0.36) at 306 kHz/612kHz. During volumetric multi-point exposures, approximately 70% and 60% of the transmit frames in which microbubble activity was detected via FSE were recovered using USCT when imaging at the second-harmonic and half-harmonic, respectively, compared to 60% and 69% using LMCT. Conclusion This low-cost ultrasound-guided transcranial FUS system affords USCT skull registration with accuracy comparable to LMCT methods. Significance Such systems have great potential to advance the adoption of microbubble-mediated FUS brain therapy by improving access to the technology.

The objectives of this study were to (1) develop a multi-channel trans-impedance leadforming method for beat-to-beat stroke volume (SV) and breath-by-breath tidal volume (TV) measurements and (2) assess its feasibility on an existing in vivo animal dataset.

A deterministic leadforming algorithm was developed to extract a cardiac volume signal (CVS) and a respiratory volume signal (RVS) from 208-channel trans-impedance data acquired every 20 ms by an electrical impedance tomography (EIT) device. SV

and TV

values were computed as a valley-to-peak value in the CVS and RVS, respectively. The method was applied to the existing dataset from five mechanically-ventilated pigs undergoing ten mini-fluid challenges. An invasive hemodynamic monitor was used in the arterial pressure-based cardiac output (APCO) mode to simultaneously measure SV

values while a mechanical ventilator provided TV

values.

The leadforming method could reliably extract the CVS and RVS from the 208-channel trans-impedance data measurand TV noninvasively, which would be beneficial to patients. The repressor-element 1-silencing transcription/neuron-restrictive silencer factor (REST/NRSF) controls hundreds of neuron-specific genes. We showed that REST/NRSF downregulates glutamatergic transmission in response to hyperactivity, thus contributing to neuronal homeostasis. However, whether GABAergic transmission is also implicated in the homeostatic action of REST/NRSF is unknown. Here, we show that hyperactivity-induced REST/NRSF activation, triggers a homeostatic rearrangement of GABAergic inhibition, with increased frequency of miniature inhibitory postsynaptic currents (IPSCs) and amplitude of evoked IPSCs in mouse cultured hippocampal neurons. Notably, this effect is limited to inhibitory-onto-excitatory neuron synapses, whose density increases at somatic level and decreases in dendritic regions, demonstrating a complex target- and area-selectivity. The upscaling of perisomatic inhibition was occluded by TrkB receptor inhibition and resulted from a coordinated and sequential activation of the Npas4 and Bdnf gene programs. On the opposite, the downscaling of dendritic inhibition was REST-dependent, but BDNF-independent. The findings highlight the central role of REST/NRSF in the complex transcriptional responses aimed at rescuing physiological levels of network activity in front of the ever-changing environment. This report highlights the association of tacrolimus use with acute macular neuroretinopathy (AMN). A 27-year-old woman, a known case of diffuse proliferative membranous glomerulonephritis, developed abnormal body movements, loss of consciousness, and blurring of vision in the left eye, after 3 months of starting tacrolimus. Blood investigations revealed anemia, thrombocytopenia, raised urea and creatinine levels, and raised LDH levels. A diagnosis of tacrolimus induced hemolytic uremic syndrome (HUS) with posterior reversible encephalopathy syndrome (PRES) was made. Ocular examination revealed a reddish-brown petaloid retinal lesion, which was better appreciated on red-free imaging as dark grey area pointing towards the fovea. OCT-A and SD-OCT revealed flow voids in deep retinal plexus, and disruption of ellipsoid and interdigitation zone, respectively, findings consistent with AMN. To the best of our knowledge, it is the first report of association of tacrolimus with AMN.Community pharmacies have made significant advances in digital technology; however, mobile systems are only emerging in this sector and mostly focusing patient-centric connections. This study reveals a case of digital transformation in a mobile connected pharmacy, balancing efficient pharmaceutical services and digital innovation. A mobile connected pharmacy solution (mPharmaCare) is developed for a community of near 100.000. The first stage includes a bibliometric analysis and a structured literature review of the mobile connected pharmacy. In the second stage, action research was conducted to evaluate mPharmaCare adoption. A dual organizational structure was tested to cope with innovation and efficient exploration of pharmacy services. Community Pharmacy 5.0 is an inspiring vision that will take advantage of mobility. However, there are tensions between the core pharmacy business and the new technology layers of community connections. Community pharmacies require both client-centric and community-centric approaches to achieve individualization of patient care and horizontal and end-to-end digital integration of pharmacy data. Digital transformation can remove silos in the community pharmacy. Creating an - internal or outsourced - innovation division may be suitable for medium and large community pharmacies. Moreover, pharmacies must consider shifting to a product-service system offer, deploying synchronization mechanisms with different stakeholders.Purpose To monitor intrafraction motion during spine stereotactic body radiotherapy(SBRT) treatment delivery with readily available technology, we implemented triggered kV imaging using the on-board imager(OBI) of a modern medical linear accelerator with an advanced imaging package. Methods Triggered kV imaging for intrafraction motion management was tested with an anthropomorphic phantom and simulated spine SBRT treatments to the thoracic and lumbar spine. The vertebral bodies and spinous processes were contoured as the image guided radiotherapy(IGRT) structures specific to this technique. Upon each triggered kV image acquisition, 2D projections of the IGRT structures were automatically calculated and updated at arbitrary angles for display on the kV images. Various shifts/rotations were introduced in x, y, z, pitch, and yaw. Gantry-angle-based triggering was set to acquire kV images every 45°. A group of physicists/physicians(n = 10) participated in a survey to evaluate clinical efficiency and accuracy of c This technique allows efficient, non-invasive monitoring of patient position using the OBI and patient anatomy as a direct visual guide.

The purpose of this work was to employ a semi-automatic method for measuring spinal cord cross-sectional area (SCCSA) and investigate the correlations between diffusion tensor imaging (DTI) metrics and SCCSA for the cervical and thoracic spinal cord for typically developing pediatric subjects and pediatric subject with spinal cord injury.

Ten typically developing (TD) pediatric subjects and ten pediatric subjects with spinal cord injury (SCI) were imaged using a Siemens Verio 3 T MR scanner to acquire DTI and high-resolution anatomic scans covering the cervical and thoracic spinal cord (C1-T12). SCCSA was measured using a semi-automated edge detection algorithm for the entire spinal cord. DTI metrics were obtained from whole cord axial ROIs at each vertebral level. SCCSA measures were compared to DTI metrics by vertebral level throughout the entire cord, and above and below the injury site. Correlation analysis was performed to compare SCCSA, DTI and clinical measures as determined by the International Standards for Neurological Classification of Spinal Cord Injury (ISNCSCI) examination.
