Detecting matter at a single-molecule level is the ultimate target in many branches of study. Nanosensors based on plasmonics have garnered significant interest owing to their ultrahigh sensitivity even at single-molecule level. However, currently, plasmonic-enhanced nanosensors have not achieved excellent performances in practical applications and their detection at femtomolar or attomolar concentrations remains highly challenging. Here we show a plasmonic sensing strategy, called buoyant plasmonic-particulate-based few-to-single particle-nanosensors. Large-sized floating particles combined with a slippery surface may prevent the coffee-ring effect and enhance the spatial enrichment capability of the analyte in plasmonic sensitive sites via the aggregation and lifting effect. Dimer and single particle-nanosensors demonstrate an enhanced surface-enhanced Raman spectroscopy (SERS) and a high fluorescence sensitivity with an enrichment factor up to an order of ∼104 and the limit of detection of CV molecules down to femto- or attomolar levels. The current buoyant particulate strategy can be exploited in a wide range of plasmonic enhanced sensing applications for a cost-effective, simple, fast, flexible, and portable detection.BACKGROUND Hyperlactatemia is a common phenomenon following cardiac surgeries and is associated with prolonged ICU stay and higher morbidity and mortality rates, but such analyses have never focused on patients undergoing heart transplantation (HTX), in whom hyperlactatemia defined with the traditional threshold is observed in nearly every individual. The present study aimed to assess the prognostic value and clinical usefulness of postoperative serum lactate level measurements for in-hospital mortality prediction following HTX. GSK-3 cancer MATERIAL AND METHODS Forty-six consecutive patients who underwent HTX in the Department of Cardiovascular Surgery and Transplantology between 2010 and 2015 were enrolled into a retrospective analysis. Serum lactate level measurements within the first 48 h after HTX were obtained from arterial blood gas analyses, that were routinely conducted every 6 h. Lactate clearance was determined for each patient individually throughout 3 different time frames the first 24-h (Lac clear 0-24) and second 24-h period (Lac clear 24-48), and the first 48 h after surgery (Lac clear 0-48). RESULTS The ICU admission serum lactate levels differed between the deceased and survivors (7.6 vs. 4.3 mmol/L; p=0.000). Among all tested postoperative lactate level measurements, only the measurement taken upon ICU admission predicted in-hospital mortality (OR 1.94 95% CI [1.09-3.43]; p=0.024). The receiving operating characteristic (ROC) curve for in-hospital mortality was constructed for ICU admission measurement, with the optimal cut-off point estimated at 7.0 mmol/L. CONCLUSIONS Serum lactate level measurement upon ICU admission can be used as a predictive parameter for in-hospital mortality among heart transplant recipients. Values greater than 7.0 mmol/L can predict in-hospital mortality with 90% accuracy.BACKGROUND Aerobic exercise is uniformly accepted as one of the most important modifiable factors to improve cardiovascular health, but endurance athletic training poses a significant risk factor for development of atrial fibrillation (AFib) in middle-aged and older adults. Ubiquitous risk assessment tools (CHADS2 and CHA2DS2-VASc) and US Preventive Services Task Force guidelines do not presently account for this association. A case is presented which illustrates a dire outcome of undiagnosed AFib in an elderly male patient who had run many marathons. CASE REPORT An 80-year-old male with well-controlled hypertension and hypercholesterolemia and a history significant for running many marathons throughout his life was brought in via ambulance after being found down with head trauma by his wife at home. A short run of AFib was recorded on telemetry and electrocardiogram (ECG) and a review of previous ECGs revealed evidence of interatrial block (Bayes Syndrome), though the patient had no history of AFib or anticoagulation. This coupled with imaging indicated thromboembolic stroke to the left middle cerebral artery leading to right-sided hemiplegia and a subsequent fall to the right, causing right-sided head trauma and intracranial hemorrhage. His clinical course did not improve, and on his fifth day of admission he was transferred to comfort care, extubated, and succumbed to his injuries. CONCLUSIONS This case and the accompanying summary of evidence strongly encourage further investigation and a higher index of suspicion for AFib in asymptomatic older adults with a history of endurance athletic training.BACKGROUND This study was designed to investigate the effect of high-glucose and high-fat condition on estrogen receptor- and sexual precocity-related genes in GT1-7 cells. MATERIAL AND METHODS In this study, CCK8 was used to detect cell viability, and TUNEL assay was used to detect apoptosis levels of GT1-7 cells after treatment with glucosamine and palmitate. The expression level of GnRH was measured by ELISA and RT-qPCR. RT-qPCR and Western blot were used to detect the expression of ERß, CD36, and GPR54 in GT1-7 cells, and the expression of ERß was detected using immunohistochemistry analysis. Finally, after adding the intervening drug tamoxifen to GT1-7 cells, the expression level of GnRH was measured by ELISA and Western blot analysis was used to detect the expression of GPR54 and GnRH. RESULTS GnRH secretion in the high-fat and high-glucose group increased continuously over time and peaked at 18 h, and GnRH gene expression peaked at 12 h. High-fat and high-glucose conditions also significantly increased the levels of estrogen receptors ß (ERß), fatty acid translocase protein (CD36), and G Protein-Coupled Receptors 54 (GPR54) in GT1-7 cells. After estrogen receptors ß (ER) was inhibited, GnRH secretion and GPR54 expression were decreased at 12 h and 18 h. CONCLUSIONS Our study demonstrates that high-glucose and high-fat conditions promote the secretion of GnRH and ER and the expression of genes related to sexual precocity in GT1-7 cells.
