To investigate patient preferences for treatment settings in the light of the current pandemic.

Patients, who prior to the pandemic had attended two different outpatient clinics in a university hospital for their infusions or injections, were interviewed by telephone. The semi-structured interviews were analyzed using qualitative and quantitative methods.

Out of 49 patients with either anti-inflammatory or immunoglobulin treatments (response rate 83 %), 24 (49.0 %) switched from subcutaneous (sc) injections in the hospital to the community setting, 18 (36.7 %) from intravenous infusions (iv) in the hospital to sc administration at home and 7 (14.3 %) moved to iv at home. During the pandemic 38 (80.9 %) wanted to continue their treatment at home, but after the pandemic 22 (46.8 %) would opt to go back to the hospital. Satisfaction was high with both settings, slightly favoring drug administration in hospital. Qualitative data shows that patients while emphasizing the importance of the relationship with the healthcare team, had increased concerns about safety as a result of COVID-19.

Background The measures initiated as a result of the coronavirus pandemic have far-reaching consequences for the everyday life of people with dementia and their family carers. Both are usually among those who are the most vulnerable and thus are subject to rigorous restrictions. Their everyday life is made more difficult because care and respite services are currently suspended. In addition, people with dementia have difficulty understanding and implementing the restrictions and hygiene rules.

This study aims to describe the current life situation of family carers and people with dementia.

For this purpose, 21 telephone interviews with both family carers and people with dementia have been conducted twice during the spring of 2020 and were subsequently evaluated by a content analysis.

The interviewees experienced the situation differently. Especially the social isolation, the higher amount of care, the uncertainty of the situation and the increase in psychological symptoms are described as being stressful. With regard to coping with the situation, discussions are taking place about the support from the social environment, alternative ways of communication, experiences with comparable crises, the stability of formal care and the handling of information.

Informal support is a particularly important support mechanism.Hyoliths (hyolithids and orthothecids) were one of the most successful early biomineralizing lophotrochozoans and were a key component of the Cambrian evolutionary fauna. However, the morphology, skeletogenesis and anatomy of earliest members of this enigmatic clade, as well as its relationship with other lophotrochozoan phyla remain contentious. Here, we present a new orthothecid, Longxiantheca mira gen. et sp. nov. preserved as part of secondarily phosphatized small shelly fossil assemblage from the lower Cambrian Xinji Formation of North China. Longxiantheca mira retains some ancestral traits of the clade with an undifferentiated disc-shaped operculum, a simple conical conch with apical septa and a two-layered microstructure of aragonitic fibrous bundles. The operculum interior exhibits impressions of soft tissues, including muscle attachment scars, mantle epithelial cells and a central kidney-shaped platform interpreted as a support structure in association with its presumptive feeding apparatus. The muscular system in orthothecids appears to be similar to that in hyolithids, suggesting a consistent anatomical configuration among the total group of hyoliths. The new finding of shell secreting cells demonstrates a mantle regulating the mode of growth for the operculum. Investigations of shell microstructures support the placement of hyoliths as total group molluscs with an unsettled position within the phylum Mollusca.Coral bleaching events are increasing in frequency and severity, resulting in widespread losses in coral cover. However, branching corals native to highly variable (HV) thermal environments can have higher bleaching resistance than corals from more moderate habitats. Here, we investigated the response of two massive corals, Porites lobata and Goniastrea retiformis, from a moderately variable (MV) and a low variability (LV) pool transplanted into a HV pool on Ofu Island in American Samoa. Paired transplant and native ramets were exposed to an acute thermal stress after 6 and 12 months of exposure to the HV pool to evaluate changes in thermal tolerance limits. For both species, photosynthetic efficiency and chlorophyll loss following acute heat stress did not differ between ramets transplanted into the HV pool and respective native pool. Moreover, HV native P. find more lobata exhibited the greatest bleaching susceptibility compared to MV and LV natives and there was no effect of acute heat stress on MV P. lobata. There was also a thermal anomaly during the study, where Ofu's backreef thermal regime surpassed historical records-2015 had 8 degree heating weeks (DHW) and 2016 had up to 5 DHW (in comparison to less than or equal to 3 over the last 10 years)-which may have exceeded the upper thermal limits of HV native P.
