Secure messaging (SM) between patients and primary care teams has expanded care access but may impact other clinical encounters.

To study associations between SM use and primary care in-person and telephone visits in the Veterans Health Administration (VHA).

The SM feature of VHA's patient portal, MyHealtheVet, supports asynchronous communication between patients and primary care teams. To study the impact of SM on in-person and telephone visits, two analyses were performed (1) a retrospective pre-/post-analysis comparing changes after initiating SM use and (2) a difference-in-difference comparison among SM users and non-users 1 year before and after index SM use. To investigate potential relationships, we sampled multi-floral honey from 30 Apis mellifera hives from urban, agricultural and semi-natural habitats (SNH), identified and quantified three neonicotinoids present (clothianidin, imidacloprid and thiacloprid) using UHPLC-MS, and classified surrounding land use up to 5 km around hive sites. Neonicotinoids were most frequently detected in honeys from hives in agricultural habitats, and 70% of all samples contained at least one of the three neonicotinoid compounds. Imidacloprid was the most frequently detected neonicotinoid (found in 43% of honey samples) followed by clothianidin (40%) and thiacloprid (37%). Almost half (48%) of samples contained at least two neonicotinoids, and two of the 30 samples contained all three. Clothianidin and thiacloprid were more frequently detected in honeys from urban habitats, highlighting that exposure to pesticides does not just occur in agricultural settings. To achieve the purpose of this research paper, available data and literature are analyzed, evaluated, and used, in order to provide a comprehensive understanding of the status of SMI, in light of sustainable development. This relates to various perspectives, including limestone geology, geopolitics, socioeconomics, culture, technology, legislation, as well as climate change, acid rain, and harmful effects of the SMI scale on public health and safety, environmental well-being, and challenges facing the industry.
