The hybrid molecules bearing heterocyclic structures in the A or D rings of steroids have significant biological activity. 16 (E)-Hetereoarylidene steroids were synthesized from the reaction of different heteroaromatic carbaldehydes and trans-Dehydroepiandrosterone (DHEA) in a basic medium. Then, synthesis of the N-formyl pyrazoline substituted new DHEA derivatives were carried out from the reaction of hydrazine hydrate and 16 (E)-hetereoarylidene steroids. The structures of the synthesized compounds were elucidated by elemental analysis, FT-IR, 1H NMR, and 13C NMR spectroscopy. To investigate the activation pathway of synthesized N-formyl pyrazoline substituted steroid derivatives, a molecular docking study was performed on human cytochrome P450-(CYP17A1 PDB ID 5IRQ) with the help of the free AutoDock Vina. 100 ns molecular dynamic simulation process was performed to monitor the behavior of the complex structure formed by CYP17A1 and to calculate the stability over time of 2a and 2d (-9.8 kcal/mol), which gave the lowest value according to the results obtained in the molecular docking study with AutoDock Vina. Accordingly, RMSD, RMSF, Rg, and SASA analyzes of 2a and 2d were performed, and MMPBSA was calculated. Lastly, the ADMET (absorption, distribution, metabolism, excretion, and toxicity) analyses of the novel steroid derivatives were investigated. Communicated by Ramaswamy H. This study leverages the random variation in sex at birth to assess whether the sex of nieces and nephews, in combination with own children, matters for fertility choices. Using three-generational data from the US Panel Study of Income Dynamics (PSID), I show that extended families (including grandparents, their children, and their grandchildren) are collectively more likely to have three or more grandchildren when lacking sex mix, whether the first two grandchildren are siblings or cousins. I explore the pathways for these offspring sex preferences, finding support for a preference for an uninterrupted line of male descendants. This multigenerational approach also contributes a new estimation strategy that causally estimates the effects of family sizes on outcomes beyond fertility.

The growing burden of hypertension is emerging as one of the major healthcare challenges in low- and middle-income countries (LMICs), such as Nepal. Given that they are struggling to deliver adequate health services, some LMICs have significant gaps in the cascade of hypertension care (including screening, awareness, treatment, and control). This results in uncontrolled hypertension, placing a high burden on both patients and healthcare providers.

The objective of this study was to quantify the gaps in hypertension screening, awareness, treatment, and control in the Nepalese population.

We used the data from a pooled sample of 9682 participants collected through two consecutive STEPwise approach to Surveillance (STEPS) surveys conducted in Nepal in 2013 and 2019. A multistage cluster sampling method was applied in the surveys, to select nationally representative samples of 15- to 69-year-old Nepalese individuals. Prevalence ratios were calculated using multivariable Poisson regression.

Among the hypernsion care in Nepal are large. These gaps are particularly pronounced among the poor, persons living in Lumbini and Sudurpaschim provinces, those who sought treatment in public hospitals, those who did not have health insurance, and young people. In the present study, we conducted a population coverage analysis referring to the diverse Indian population. From the Immune epitope database (IEDB), HLA- distribution analysis was performed to find the most promiscuous T-cell epitope out of In silico determined epitope of Spike protein from SARS-CoV-2. Epitopes were selected based on their binding affinity with the maximum number of HLA alleles belonging to the highest population coverage rate values for the chosen geographical area in India. 404 cleavage sites within the 1288 amino acids sequence of spike glycoprotein were determined by NetChop proteasomal cleavage prediction suggesting the presence of adequate sites in the protein sequence for cleaving into appropriate epitopes. For population coverage analysis, 179 selected epitopes present the projected population coverage up to 97.45% with 56.16 average hit and 15.07 pc90. 54 epitopes are found with the highest coverage among the Indian population and highly conserved within the given spike RBD domain sequence. Among all the predicted epitopes, 9-mer TRFASVYAW and RFDNPVLPF along with 12-mer LLAGTITSGWTF and VSQPFLMDLEGK epitopes are observed as the best due to their decent docking score and best binding affinity to corresponding HLA alleles during MD simulations. Outcomes from this study could be critical to design a vaccine against SARS-CoV-2 for a different set of populations within the country.Communicated by Ramaswamy H. Sarma.Fear of cancer recurrence (FCR) is the most common psychosocial issue amongst cancer survivors. However, fear of progression (FoP) has rarely been studied outside of the cancer context. This review aimed to (1) meta-synthesise qualitative studies of FoP in illnesses other than cancer; and (2) quantify the relationship between FoP and anxiety, depression, and quality of life (QoL) in non-cancer chronic illnesses. 2-DG We identified 25 qualitative and 11 quantitative studies in a range of chronic illnesses. Participants described fears of progression and recurrence of their illness, including fears of dying, and fears of becoming a burden to family. Fears were often triggered by downward comparison (i.e., seeing people worse off than themselves). Participants coped in different ways, including by accepting the illness or seeking knowledge. Those for whom these fears caused distress reported hypervigilance to physical symptoms and avoidance. Distress, and seeking information, were associated with adherence. In quantitative analyses, FoP was moderately associated with QoL, and strongly associated with anxiety and depression. These results suggest that FoP in illnesses other than cancer is similar to FCR. FoP appears to be an important transdiagnostic construct associated with distress. Evidence-based FCR interventions could be adapted to better manage FoP in other illnesses.The highway-rail grade crossings (HRGCs) across the United States have been experiencing about 2500 crashes each year. Previous studies analyzed crash frequencies and fatalities; however, factors pertaining to drivers' gate violation behaviors are little known. Also, applied methodologies for gate violation behaviors analysis did not consider their heterogeneity across regions. This study uses 20-year of crash data (1999-2018) to evaluate pre-crash drivers' behaviors at HRGCs. A mixed multinomial logit model was developed to associate such behaviors with demographic factors, vehicle characteristics, temporal and environmental factors, as well as crossing-related factors. The study results indicated a high intra-class correlation coefficient which signifies the importance of including the random-effect parameter in the model. Further, the study found that male drivers are more likely to drive around the gate, while older drivers are more likely to stop and proceed before a train has passed. Furthermore, compared to trucks, all other vehicle types are more likely to drive around the gate. The influence of train speed, vehicle occupancy, visibility, among others, on drivers' pre-crash behaviors, is also presented. Understanding the impact of these factors on pre-crash behaviors may assist in improving the motorist's safety at the highway-rail grade crossings across the United States.

Suicide rates among Hispanics in the United States have steadily risen over the last 2 decades, especially among youth and adults in midlife. Social disconnection (low belonging) is associated with suicidal ideation; however, little is known about the demographic and social factors that impact a sense of belonging among Hispanic adults in midlife. The current study sought to examine (a) the association between demographic and social factors (acculturative stress, community integration and engagement, religiosity, ethnic cohesion) and neighborhood belonging as well as (b) whether neighborhood belonging is associated with passive suicide ideation (thoughts of death) among a community sample of Hispanic adults.

This study uses a sample of 968 Hispanic adults in midlife, from Puerto Rican, Mexican, and Dominican backgrounds, from the Midlife in the United Sates (MIDUS) Survey of Minority Groups.

Insight into patients' experiences of monitoring physical activity is needed to inform future healthcare practice and policy utilizing this technology to support long-term positive health behavior change. This scoping review aimed to explore the experiences of using technology for monitoring physical activity among people with COPD. The Joanna Briggs Institute scoping review methodological framework was used. Relevant scientific databases (CINAHL Complete, MEDLINE, PsycINFO, SPORTDiscus, Cochrane Library and Scopus) were searched from 1st January 2016 to 16th March 2021. Thematic synthesis was used to analyze the data. Twelve studies exploring the experiences of people with COPD using technology for monitoring physical activity were included in the synthesis. Seven themes were developed and summarize experiences 1) Monitoring and keeping track of their activity and health, 2) Supporting motivation to be active, 3) Acceptability of the device, 4) Experiencing technical issues with the device, 5) Setting appropriate and achievable goals for their health condition, 6) Integrating the device into their life and daily routine, and 7) Perceived physical and psychological benefits of using the device. Further high-quality research is needed to understand the experiences of people with COPD using technology to monitor physical activity in everyday life and better self-manage their health condition. Supporting people with COPD to monitor their physical activity could enable them to better self-manage their health condition.
