Guillain-Barré's syndrome in childhood can follow an atypical course, increasing the challenges in diagnosis and decisions regarding immunomodulatory treatment. Here, we report the case of on a 13-year-old boy with acute onset Guillain-Barré's syndrome progressing over 40 days to a very severe, locked-in-like syndrome despite intensive immunomodulatory treatment. After a plateau phase lasting 3 months and characterized by fluctuating signs of ongoing inflammatory disease activity, we were prompted to perform repeated and maintenance immunomodulatory treatment, which resulted in a continuous and nearly complete recovery of function. Atypical features at disease onset, the severe "total" loss of all peripheral and cranial nerve function, and an apparent late response to treatment give rise to reviewing the dilemmas of diagnosis and treatment in such severe and protracted courses of Guillain-Barré syndrome.

The purpose of our study was to suggest an imaging strategy and guidelines for the selection of the children with mild intellectual disability (ID) for magnetic resonance imaging (MRI), to avoid unnecessary imaging.

The brain MRIs and patient reports of 471 children were reviewed for the imaging findings and ID severity. The correlation between the clinical and brain MRI findings was analyzed in the 305 children with mild ID.

Thirty-eight (12.5%) of the children with mild ID had significant abnormal brain MRI findings. Thirty-five of these had other neurological symptoms or diseases in addition to ID, which were an indication for brain MRI. In the logistic regression analysis, seizures (in patients without an epilepsy diagnosis), epilepsy, movement disorders, dysmorphia, encephalitis, traumatic brain injury, and abnormal head size were statistically significant symptoms or comorbidities associated with abnormal MRI findings. Only three children (1.0%) with mild ID had a significant MRI finding without any other clinical symptoms or disease.

Routine MRI in children with mild ID without specific neurological symptoms, dysmorphic features, or related diseases is not suggested for revealing an etiology of mild ID. Since children with ID usually need to be sedated for MRI, routine imaging in the diagnostic evaluation of mild ID should be carefully considered. Clinical examination, other symptoms, and related diseases should be carefully assessed to decide the need for MRI.

Transoral robotic surgery (TORS) in particular has been pushing the boundaries towards transoral resection in recent years. Nevertheless, there are still limitations to this approach. Transcervical surgery is then the therapy of choice. It is used in case of insufficient visibility of the entire larynx and especially the tumor-bearing region, cartilage infiltration, a lack of mobility of both arytenoid cartilages or a lack of patient compliance. Even with the transcervical approach, from an oncological point of view, the procedure must be as radical as necessary on the one hand and as function-preserving as possible on the other. In case of a transcervical appraoch, however, additional requirements, side effects and consequences must be considered during planning, implementation and follow-up treatment. In addition to our previous publications, this article is intended to provide an overview of transcervical surgery for malignancies of the larynx.Rhinophyma is a benign disease involving the skin of the nose, which is characterized by skin thickening and hypertrophy of the sebaceous glands and connective tissue. It occurs predominantly by Caucasians in their fifth to seventh life decades and is 12-30 times more likely to affect males. The etiology and pathogenesis of Rhinophyma remains unclear, however it is generally stated to be the final stage of rosacea. A causative relationship between rhinophyma and alcohol consumption has not yet been proven. This review highlights the treatment options of rhinophyma. Surgery is gold standard for management of advanced rhinophyma. Each technique has pros and cons, but using surgical instruments with monopolar energy as loop and ball electrode seem to combine several goals best - first of all simultaneous removal and hemostasis followed by nasal contour refinement. Due to possible coincidence of skin cancer such as a basal- or squamous cell carcinoma, histopathological examination of all removed tissue is recommended.

The aim of the present study was to evaluate the surface roughness (SR) of various nickel-titanium (NiTi) rotary endodontic instruments (ProTaper Next [PTN], WaveOne Gold [WOG], and ProTaper Gold [PTG]) before and after root canal instrumentation.

For each type (PTN, WOG, and PTG), the endodontic instrumentation was performed using extracted mandibular molar teeth's curved mesial root canals (curvature 20-40 degrees) after determining the working length. Each NiTi file was cleaned, and sterilized following preparation of four root canals and characterized for surface properties before and after endodontic instrumentation using a contact-mode three-dimensional surface profiler. The data were analyzed statistically using Statistical Package for the Social Sciences for SR parameters including average surface roughness value (Sa), root mean square roughness (Sq), and peak to valley height (Sz).

Preinstrumentation assessment revealed a significant difference for all the three SR variables

< 0.05) for the cutting blade and the flute area. WOG instruments showed the highest SR values (

= 0.000). The postinstrumentation assessment revealed significant differences in SR values in the blade and the flute between the three groups (

< 0.05), with WOG and PTG exhibiting the highest values in the blade and flute sections, respectively.

The SR parameters of intact PTN, WOG, and PTG NiTi files vary and that was increased following the endodontic instrumentation.

To evaluate the surface roughness and gloss of three nanohybrid resin composites after polishing with three different polishing systems.

A total number of 112 disc specimens (10 × 3 mm) were prepared from nanohybrid-Empress Direct (ID), Grandio (GR), Filtek Z550 (ZN) and a microhybrid resin composite restorative materials-Filtek Z250 (ZM). Following 24-hour storage in 37°C distilled water, each composite group (

= 28) was assigned into four groups (

= 7) according to finishing/polishing (F/P) system Mylar strip, Optrapol, Politip, and Sof-Lex (SL). The surface roughness (Ra, mm) was measured by a novel three-dimensional method using an image analysis software attached to an environmental scanning electron microscope. A glossometer was used to measure the surface gloss.

Statistical analysis used was ANOVA test. Two-way Anova test revealed that the "type of composite" and "F/P techniques" had a significant effect on both surface roughness and gloss of the tested resin composite materials (

< 0.05). Tukey's post hoc test showed that ID, GR, and ZN revealed lower surface roughness and higher surface gloss than ZM within the same polishing system (

< 0.05). Sof-Lex polishing discs produced the lowest surface roughness and highest surface gloss values compared with Optrapol and Politip (

< 0.05).

The tested F/P systems provided comparable surface roughness and gloss for nanohybrid composites. The Sof-Lex system provided the best surface roughness and gloss for nanohybrid composites.

Disc-shaped specimens of four resin composites (Aura bulk fil [SDI], IPS Impress Direct [Ivoclar], Filtek Z250, and Filtek P60 [3M/ESPE]) were prepared and divided in two groups either uncoated or coated with G-coat plus (

= 10). The specimens were obtained and stored in distilled water (DW) at 37°C. For water sorption and solubility, the specimens were stored for 60 days in DW and measured according to ISO 4049 instructions. The specimens were subjected to color change (ΔE), microhardness, and flexural strength (FS) measurements after 24 hours and 60 days of immersion in DW.

ANOVA, post hoc Tukey's, and independent

-tests were used to analyze data.

Mean solubility values of coated groups were significantly lower than that of uncoated ones (

< 0.001). No significant differences were detected between neither water sorption nor FS of coated and uncoated groups (

= 0.502 and

= 0.510, respectively). For all materials except IPS Empress, the mean values for ΔE were not statistically different between coated and uncoated groups, and mean microhardness values of the uncoated groups were significantly greater than the coated groups (

< 0.001).

Coating resin composites improved water sorption and solubility but increased ΔE, decreased microhardness, and had no significant effect on FS in the long run.

The results of treatment and rehabilitation made it possible to achieve the optimal distribution of loads on the dentition. It was shown that it is necessary to analyze casts in an articulator to reproduce a three-dimensional model of mandibular movements taking into account the individual features of the patient to obtain a satisfactory assessment and treatment planning of patients with edentulism requiring full mouth reconstruction. This case confirms that the developed algorithm is anatomically and pathogenetically justified, as it considers all changes and interrelationships of structures of the dentition.

Hypoxic-ischemic (HI)-related brain injury, especially HI encephalopathy (HIE) is a leading cause of morbidity and disability in newborns. Long noncoding RNAs (lncRNAs) are implicated in the progress of HI brain damage. However, the mechanisms underlying the regulatory effects of lncRNA H19 on autophagy in HIE remain unknown. This study was designed to identify the potential mechanisms involving lncRNA H19 in HIE.

We selected three HIE newborns and three healthy newborns for neonatal behavioral neurological assessment and screened the differentially expressed lncRNAs by microarray analysis and detected H19 expression in serum. After that, neonatal HIE rats were established and injected with H19 overexpression lentivirus vector or autophagy activator Rapa. USP25/28 inhibitor AZ1 nmr The structure and apoptotic levels of brain tissue were observed, and righting reflex and geotaxis reflex were utilized to evaluate the short-term neurological function of HIE rats. The Morris water maze was performed to measure the long-term neurological functions of HIE rats.
