A total of 103 patients were analyzed, and the renal colic diagnosis was retained in 85 cases after the CT examination. The accuracy of PoCUS was 91% (86; 95%) for detecting urinary tract dilatation, 83% (76; 90%) for detecting perinephric fluid, and 54% (44; 64%) for detecting lithiasis. Only high urinary tract stones with ≥ 6mm diameter were surgically managed (p < 0.01). Conversely, distal ureteral stones with a diameter of < 6mm were managed with medical ambulatory treatment (p < 0.05).

The extrafloral nectaries of S. occidentalis were studied structurally and anatomically (at secretory and post-secretory developmental stages). Role of extrafloral nectaries as a common plant-adoptive characteristic in context to diversity and phylogenetic pattern was also speculated while exploring other collaborative evolutionary implications of this plant. Extrafloral nectaries (EFNs) are widespread and evolutionarily labile traits that have repeatedly and remarkably evolved in vascular plants. Morphological descriptions of the EFNs of certain plant species are common in the literature, but they rarely relate morphology with histology, gland distribution and secretory characteristics. Studies relating EFNs features, i.e., morphology and distribution with their differential visitation by insects, viz. ants and the cost of maintenance to the plants are important to understand the evolution of these glands. Therefore, in this study a morphological, anatomical (structure and ultrastructure) and secretory chad from a functional perspective. S. occidentalis L. (Caesalpiniaceae) is an economically important species from industrial, medicinal and agricultural perspective. Observations from the result showed that shape of the EFNs (size 1-2 mm) ranged to globular, ovoid-conical, dome-shaped, fusiform or cylindrical with conical tip. The EFNs were sessile, positioned interpetiolar or seated at the base of petiole. Light and transmission electron microscopic studies showed the specific internal structures of the extrafloral nectary. Two developmental stages of the EFNs (secretory and post-secretory) were recognized. Our current understanding of the phylogenetic patterns of EFNs makes them powerful candidates for future work exploring the drivers of their evolutionary origins, shifts, and losses.This paper aims to uncover the explanatory profile of an idealized version of Karl Ernst von Baer's notion of individuation, wherein the special develops from the general. Second, because historical why-questions concern the diachronic unity or genidentity of the events under consideration, I argue that the von Baerian pattern elicits a distinctive response to such questions, wherein we are inclined to simultaneously affirm and reject the temporal unity of these events. I buttress this claim by considering non-biological expressions of the von Baerian principle, drawn from institutional history and literature. In the second half of the paper, I consider the implications of my findings for ontogenetic and phylogenetic sequences. I argue that the explanatory profile of von Baer's principle neatly describes the distinctive speciation events that characterize deep metazoan phylogeny, as described by Stuart Newman. I also argue that parallel considerations should move us to accept a sense in which ontogenetic stages are not diachronically unified.

Talar neck and body fractures are uncommon injuries that are challenging to manage with high reported complication rates, including post-traumatic arthritis, avascular necrosis, and poor functional outcomes. The aim of this study was to assess the complication rates for patients with talus fractures across three major trauma centres (MTCs) in England.

A retrospective analysis was performed of prospectively collected trauma databases. Data were collected from three English MTCs. Patients with talar neck and/or body fractures sustained between August 2015 and August 2019 were identified and their clinical course reviewed radiologically and clinically. Isolated process fractures, osteochondral defects and paediatric patients were excluded. Patients were analysed by fracture type and for definitive treatment method with separation into non-operative and operative management groups. Procedure type was identified in the operative group. Superficial infection, deep infection, non-union, avascular necrosis, post-traumatic arthritis and removal of metalwork rates were analysed.

Eighty-five patients with talar neck and/or body fractures were included. Seventy-five patients received operative management, 10 non-operative. The overall AVN rate was 5.9% (five patients), overall post-traumatic arthritis rate was 18.8% (16 patients), deep infection rate 1.2% (one patient), non-union rate 4.7% (four patients). Removal of metalwork rate was 9.4% (eight patients).

Multiple sclerosis (MS) causes neuro-cognitive symptoms involving dysfunctional large-scale brain networks. Yet, whether healthy laterality patterns of RSNs are maintained in MS and whether altered laterality patterns explain disease symptoms has not been explicitly investigated. We analysed functional MRI and diffusion tensor imaging data from 24 relapsing-remitting MS patients and 25 healthy participants. We performed group-level independent component analysis and used dual regression to estimate individual versions of well-established RSNs. Voxelwise laterality indices were calculated for each RSN. Group differences were assessed via a general linear model-based approach. The relationship between functional laterality and white matter microstructural asymmetry was assessed using Tract-Based Spatial Statistics. Spearman's correlation was calculated between laterality indices and Brief International Cognitive Assessment for Multiple Sclerosis scores. Functional laterality of the dorsal attention network showed a significant leftward shift in the MS group in the posterior intraparietal sulcus (p  less then  0.033). Default-mode network laterality showed a significant leftward shift in the MS group in the angular gyrus (p  less then  0.005). Diminished dorsal attention network laterality was associated with increased fractional anisotropy asymmetry in the superior longitudinal fasciculus (p  less then  0.02). In the default-mode network, leftward laterality of the angular gyrus was associated with higher BVMT-R scores (R = - 0.52, p  less then  0.023). Our results confirm previous descriptions of RSN dysfunction in relapsing-remitting MS and show that altered functional connectivity lateralisation patterns of RSNs might contibute to cognitive performance and structural remodellation even in patients with mild clinical symptoms.

Multidisciplinary treatment for esophageal squamous cell carcinoma (ESCC) has improved outcomes, but the prognosis for ESCC remains poor. Nutritional and inflammatory indicators are reported to be associated with cancer prognosis. The combination of albumin and the derived neutrophil-to-lymphocyte ratio (Alb-dNLR) score was established to measure the immune system and nutritional status. The authors hypothesized that the Alb-dNLR score could be a new reliable prognostic factor for ESCC patients.
