A fiber laser system for standoff detection of chemical and biological species by coherent anti-Stokes Raman scattering is presented. The system is based on an ytterbium fiber laser and a hollow-core photonic crystal fiber for generation of broadband pump/Stokes pulses. High-resolution Raman spectra encompassing the fingerprint region (600-1600cm-1) are obtained for toluene, and two simulants of chemical and biological warfare agents, specifically dimethyl methylphosphonate and sodium dipicolinate. The system is operated at standoff distances of 2 m and integration times of 8 ms. The fiber technology makes the approach suitable for implementation as a compact standoff detection and identification system.Free-space quantum key distribution (QKD) is attractive for the establishment of future global-scale quantum networks. However, it can be quite difficult for dynamic polarization control required in continuous-variable QKD systems to work properly in the presence of channel fading. Here we propose a dynamic polarization control scheme and verify its validity via simulations and an experiment performed over a 150 m free-space channel. Here, we experimentally demonstrate a significant enhancement of the magneto-optical effect and nonreciprocal chiral transmission in low-biased gyrotropic media. A designer magneto-optical metasurface consists of a gyrotropy-near-zero slab doped with magnetic resonant inclusions. The immersed magnetic dopants enable efficient nonreciprocal light-matter interactions at the subwavelength scale, providing a giant macroscopic nonreciprocity and strong robustness against the bias disturbance. Microwave measurements reveal that the metasurface can act as a chiral isolator for circular polarization, with extremely weak intrinsic gyromagnetic activity. We also demonstrate its capability of signal isolation for circularly polarized antennas. The exact analytical equations for the design of the toroidal multi-pass cell were derived based on analytical vector calculations. Furthermore, the fringe effect is substantially reduced to less than 0.5% by the usage of our designed mask.BACKGROUND Clozapine plays a unique role in the management of treatment-resistant schizophrenia (TRS). Clozapine re-challenge following an episode of myocarditis is controversial, with a very limited literature, although it may be crucial in the recovery of certain patients. To date and to the best of our knowledge, only 10 of 22 studied cases reported successful clozapine retrial after myocarditis. CASE REPORT We present the case of a 22-year-old Hispanic man with treatment-resistant schizophrenia and polysubstance use disorder (methamphetamine, cannabis, and alcohol) initiated on aggressive clozapine titration after lack of response to several other therapies. Approximately 16 days after clozapine trial, the patient developed cardiac function impairment, presenting with chest pain, notable elevation in several biomarkers (troponin 0.72 ng/ml, ESR >100 mm/h, CRP 20.8 mg/dl, and BNP 999 ng/ml), and a depressed ejection fraction at 25%. Further assessments also showed positive hepatitis A serology. Following discontinuation of clozapine and providing supportive care, the patient's physical symptoms resolved. He had a relapse of psychotic symptoms, which were refractory to treatment with other antipsychotic agents. Subsequently, the patient underwent a second clozapine trial under close monitoring, with resolution of his psychosis. Repeated echocardiography demonstrated improved EF to 50%, transaminitis was resolved, repeat blood test results were normalized, and the patient was discharged while he was stabilized and asymptomatic. CONCLUSIONS This case adds to the previous case reports and suggests that clinicians may consider clozapine re-challenge following an episode of myocarditis based on clinical judgment, on a case-by-case basis, and under close monitoring. We highlight the need for development of clinical guidelines for clozapine re-challenge.BACKGROUND Temporomandibular disorders (TMD) are accompanied by masticatory muscle-related pain, making it meaningful to assess the stiffness of the masticatory muscles. 2-D08 investigated the intra- and inter-operator reliabilities of MyotonPRO for assessing the elasticity of masseter muscles, to determine minimal detectable changes, and to quantify changes in stiffness from conditions of relaxation to maximal contraction. MATERIAL AND METHODS Twenty healthy subjects (10 men and 10 women) were recruited. The stiffness of their masseter muscles was quantified with MyotonPRO in both relaxed and maximal contraction conditions. Two experienced operators (A and B) measured stiffness on the same day, and operator A repeated this procedure 5 days later. RESULTS Intra-rater reliability was good (ICC=0.78) and inter-operator reliability was excellent (ICC=0.95) for assessing masseter muscle stiffness with MyotonPRO. The mean stiffness of the masseter muscle on the dominant side was 369.5 N/m under relaxed conditions and 618.3 N/m at maximum bite force, an increase of 67.4%. Stiffness on the dominant and non-dominant sides did not differ significantly under both conditions (P>0.05). CONCLUSIONS MyotonPRO is a reliable method for quantifying the stiffness of the masseter muscle and monitoring its changes under different contraction conditions.

The purpose of this study was to determine whether ostomy patients are receiving ostomy care pre- and postoperatively in accordance with the United Ostomy Associations of America Ostomy and Continent Diversion Patient Bill of Rights.

Descriptive, quantitative study.

The sample comprised 325 participants with an ostomy living in the United States. link3 All participants underwent ostomy surgery in the United States.

Participants were recruited through a purposive, nonprobability sampling method based on the presence of an ostomy and the ability to read and understand English. Data were collected between summer 2017 and fall 2018 from ostomy support groups (online and in-person support groups). Respondents completed a self-administered online questionnaire, which included closed-ended and open-ended questions.

Forty percent (n = 127) of participants reported not being supervised changing their pouch before discharge. Forty-two percent (n = 137) reported receiving only hands-on instruction on how to care for their ostomy after surgery (no reading materials, nor video tutorial). Slightly more than half (52%; n = 170) reported not receiving support group information, and only 14% (n = 46) indicated participating in a discussion on intimacy concerns.

The study demonstrates that ostomy care at the hospital can improve. Receiving an ostomy is a life-changing operation, and the first line of support for ostomy patients is their provider. Future research should examine ostomy care from providers' perspectives and, prospectively, in order to test care measures on patient health outcomes.

The National Average Drug Acquisition Cost (NADAC) database (2013-2019) was used analyze per-unit drug price. Medicare Part D prescriber profile was used to identify eye-care providers prescribing patterns between 2013 and 2017.

Brand-name medication prices increased by 59% between 2013 and 2019, while generic medications decreased by 22%. Brand-name drugs were 13 to 162 times more expensive than their generic counterparts. Eye-care Providers prescribed 25% less brand name medications in 2017 compared with 2013.
