Cardiac resynchronization therapy (CRT) in patients with symptomatic heart failure and left bundle branch block (LBBB) can be achieved with His-bundle pacing correcting the bundle branch block (His-CRT). The present study is the largest randomized study comparing His-CRT and biventricular pacing (BiV-CRT) to date.

Fifty patients with symptomatic heart failure, left ventricular ejection fraction (LVEF)≤35% and LBBB according to electrocardiography were randomized 11 to His-CRT or BiV-CRT and followed for 6months. At implantation, 7 patients crossed over from His-pacing to LV-pacing in the His-CRT group and 1 patient crossed over from LV-pacing to His-pacing in the BiV-CRT group.

His-corrective pacing was achieved in 72% of the patients in the His-CRT group. Intention-to-treat 6-month follow-up LVEF increased by 16 ± 7% in the His-CRT group compared with 13 ± 6% in the BiV-CRT group (nonsignificant) and improvements were seen in clinical and physical parameters in both treatment arms with no significant differences between the groups. Pacing thresholds were higher for His-CRT compared with BiV-CRT both at implantation (1.8 ± 1.2V vs. 1.2 ± 0.8 V; p<0.01) and at 6-month follow-up (2.3 ± 1.4V vs. 1.4 ± 0.5 V; p<0.01). The per-protocol LVEF was significantly higher at 6months (48 ± 8% vs. 42 ± 8%; p<0.05) and the end-systolic volume was lower (65 ± 22ml vs. 83 ± 27ml; p<0.05) in His-CRT patients compared with BiV-CRT.

This paper aims to evaluate the prevalence of REs in a clinic from Aguascalientes, Mexico by analysing clinical records from the local public health system. Refractive errors (REs) are quite common globally, but no data have been published regarding their frequency in clinics from Mexico. A priori, the frequency of ametropias should be high as admixture ancestry from this region is mainly European and Amerindian, the regions with high prevalence worldwide.

This cross-sectional study was conducted on 2195 subjects from records of public optometry services during the year 2018. Information obtained included age, gender, sphere, cylinder and axis. The prevalence of myopia, hyperopia and astigmatism was determined by gender and age groups in paediatric and adult patients. Chi-square testing was applied to determine significant differences in prevalence across age groups and gender. A p-value <0.05 was considered significant.

In subjects under 18 years of age, the prevalence of emmetropia, astigmatism, myopia and hyperopia was 20.1%, 51.1%, 7.0% and 11.8%, respectively. In adults, emmetropia was present at a frequency of 20.1%, while 57.1% presented astigmatism, 12.4% hyperopia and 8.6% presented myopia. A significant association was observed between the presence of REs and age and gender.

Prostate-specific membrane antigen (PSMA) is a promising, novel theranostic target in advanced prostate cancer (PCa). Multiple PSMA-targeted therapies are currently in clinical development, with some agents showing impressive antitumour activity, although optimal patient selection and therapeutic resistance remain ongoing challenges.

To review the biology of PSMA and recent advances in PSMA-targeted therapies in PCa, and to discuss potential strategies for patient selection and further therapeutic development.

A comprehensive literature search was performed using PubMed and review of American Society of Clinical Oncology and European Society of Medical Oncology annual meeting abstracts up to April 2021.

Glucagon-like peptide-1 receptor agonists (GLP-1-RAs) have significant cardiovascular and renal benefits for patients with T2DM and related comorbidities. Their anti-inflammatory properties could be beneficial in these patients. This work provides less-biased estimates regarding the risk for respiratory tract infections and acute respiratory distress syndrome by performing the first significant meta-analysis of cardiovascular outcome trials in the literature. Notably, GLP-1-RAs do not seem to increase the risk for respiratory tract infection, pneumonia, or acute respiratory distress syndrome in patients with T2DM and cardiovascular comorbidities.

Therapeutic patient education (TPE) is effective and essential in the context of the growing prevalence of chronic diseases, in which tools are needed for planning structured programs. The objective of this project was to develop guidelines for designing and assessing a TPE program.

1) We assembled a multidisciplinary group of 8 leaders in TPE, chronicity, quality and safety from the hospital and the university. 2) We conducted an exhaustive review of the scientific literature on the planning of TPE programs directed at chronically ill patients, their relatives and caregivers. 3) The final text underwent comments and suggestions by participants from the hospital and primary care centre during a course on information and TPE methodology. The recommendations were unanimously agreed upon by the writing group.

We obtained a standardised work procedure targeted at professionals involved in planning TPE programs, based on international recommendations. The document is structured into sections a) Definition of the health problem and analysis of the situation; b) Program structure (human resources and materials); objectives (health-related, behaviour-related and educational) and methodology; c) Path the patient and family/caregiver follows in the program; and d) Assessment and indicators. The assessment of the procedure, in the framework of the methodology courses, was favourable.

Assess the quality of water language in California school district wellness policies and examine how language quality relates to school drinking water access.

Cross-sectional study.

Random sample of 240 schools selected from all California public schools, stratified by geography and grades served.

Policies (2016-2018) were coded for quality (strength and comprehensiveness) using an adapted school wellness policy tool. School administrators completed phone interviews about drinking water access on their campus.

Descriptive statistics (eg, means, standard deviations) summarized school-level characteristics and main outcomes. Mixed-effects linear regression models were used to examine the relationship between the strength and comprehensiveness of water policy language and water access.

On a scale of 0-100, mean strength was 11.3 (SD, 5.7), and mean comprehensiveness was 28.8 (SD, 8.7). There was an inverse association between the quality of water language in policies and excellence in drinking water access in schools.
