Copyright © 2020 Fred Poordad et al.Background Fecal microbiota transplantation (FMT) is a well-established therapeutic option for patients with antibiotic resistant Clostridioides difficile infection (CDI). However, the efficacy of FMT in patients with chronic liver disease remains elusive. Aims We studied the effect of FMT on chronic liver disease (CLD) patients with CDI at our tertiary medical center. Methods A cohort of all patients who received FMT from December 2012 to May 2014 for refractory or recurrent CDI was identified. Patients were monitored for a year after FMT. Descriptive analysis was conducted to compare the effect of FMT in patients with and without CLD. Results A total of 201 patients with CDI received FMT, 14 of which had a history of CLD. Nine of these patients exhibited cirrhosis of the liver with a mean Child-Turcotte-Pugh score of 8. CDI development in these patients was associated with recent exposure to antibiotics and was observed to be significantly different between both groups (17% of CLD patients vs. 58% in the general cohort, p = 0.01). Four patients with CLD received >1 FMT, of which 2 did not respond to treatment. There was no significant difference between patients with liver disease and the rest of the cohort with regard to FMT response (12/14 (87%) vs. 164/187 (88%), p = 0.01). MicroRNAs (miRNAs) and the SDF-1/CXCR4 axis are essential factors of chondrocyte apoptosis and ECM degeneration. However, very few studies have investigated the correlation between miRNAs and the SDF-1/CXCR4 axis in osteoarthritis so far. Here, through miRNAs microarray and bioinformatics analyses, we identified miR-142-5p as a CXCR4-targeted and dramatically downregulated miRNA in cartilage from OA patients, as well as in SDF-1-induced OA chondrocytes in vitro. In SDF-1-treated primary human OA chondrocytes that were transfected with a miR-142-5p mimic or inhibitor, the expression of CXCR4 was found to be inversely correlated with the expression of miR-142-5p. The dual luciferase reporter assay further verified the target relationship between miR-142-5p and CXCR4. Overexpression of miR-142-5p alleviated OA pathology by suppressing chondrocyte apoptosis, even in CXCR4 overexpressed OA chondrocytes. This was associated with decreased cartilage matrix degradation, reduced cartilage inflammation, and inactivated MAPK signaling pathway. Our study suggests that upregulated expression of CXCR4-targeted miR-142-5p can inhibit apoptosis, inflammation, and matrix catabolism and inactivate the MAPK signaling pathway in OA chondrocytes. As opposed to passive case finding, it involves systematically searching for TB in individuals who would not spontaneously present for care. The National TB Program (NTP) of India has initiated ACF for TB through the existing general health system since the end of 2017. However, prior to scale-up, there is need for exploring the implementation challenges and solutions to improve the efficiency of this program. Objectives (1) To explore the enablers and challenges in the implementation of ACF for TB by NTP in the Bengaluru rural district of Karnataka, South India, and (2) to explore the perceived solutions to improve the efficiency of ACF activity. Methods A qualitative descriptive study was conducted in the Bengaluru rural district during July 2018. In-depth interviews using purposively selected health care providers involved in active case finding (n = 9) and presumptive TB patients (n = 9) and presumptive TB patients (. Results The challenges in conduct of ACF were as follows inadequate training of health care workers, shortage of staff, indifferent attitude of community due to stigma, lack of awareness about TB, illiteracy, inability to convince patients for sputum test, and delay in getting CBNAAT results. The field staff recommended the installation of mobile CBNAAT machine, involvement of general health staff in activity, training of health workers on counseling of patients, and issue of identity cards for community health workers/volunteers so that people recognize them. Iron deficiency anemia (IDA) is a challenging public health problem across the globe instigating from a broader sociocultural background. It is more prevalent among pregnant women, children under the age of five years, and adolescent girls. Adolescent girls are vulnerable to develop IDA because of additional nutritional demand of the body needed for growth spurt, blood loss due to onset of menarche, malnourishment, and poor dietary iron intake. In this study, we explore the societal determinants of anemia among adolescent girls in Azad Jammu and Kashmir (AJK), Pakistan. A cross-sectional study was conducted in the Muzaffarabad division of AJK on randomly selected 626 adolescent girls. The data were collected using a pretested self-administered interview schedule comprising mainly closed-ended questions with a few open-ended questions. Descriptive statistics was computed for describing the data, and bivariate regression and logistic regression were used to determine the association of anemia with its societal determinants. Multiple linear regression is used to determine the relationship of different determinants (independent variables) with the hemoglobin level (dependent variable) of the respondents. The prevalence of anemia among adolescent girls is 47.9%, of which 47.7% have mild anemia, 51.7% have moderate anemia, and 5.7% have severe anemia, which reveals that anemia is a severe public health problem among adolescent girls in the study area. The findings aver that anemia occurrence was significantly associated with the respondent's and her parental education, economic well-being, prevalence of communicable diseases, menstrual disorder, exercise habits, meals regularity, and type of sewerage system.
