We undertook an online, voluntary, anonymous survey of ACEM fellows and trainees.

One hundred and thirty-nine (46.5%) respondents used a medication under examination. Sleep aids included melatonin (19.1% of respondents) and benzodiazepines (8.7%). Medications to improve performance included modafinil (4.7%), pseudoephedrine (2.0%), melatonin (2.0%) and beta blockers (1.3%). Some medications were taken prior to shifts. Medications to manage stress and anxiety included benzodiazepines (3.0%) and beta blockers (2.0%).

Medication use is common and support for some doctors may be required.Short-TE proton MRS is used to study metabolism in the human brain. Common analysis methods model the data as a linear combination of metabolite basis spectra. This large-scale multi-site study compares the levels of the four major metabolite complexes in short-TE spectra estimated by three linear-combination modeling (LCM) algorithms. 277 medial parietal lobe short-TE PRESS spectra (TE = 35 ms) from a recent 3 T multi-site study were preprocessed with the Osprey software. The resulting spectra were modeled with Osprey, Tarquin and LCModel, using the same three vendor-specific basis sets (GE, Philips and Siemens) for each algorithm. 2,3-Butanedione-2-monoxime chemical structure Levels of total N-acetylaspartate (tNAA), total choline (tCho), myo-inositol (mI) and glutamate + glutamine (Glx) were quantified with respect to total creatine (tCr). Group means and coefficient of variations of metabolite estimates agreed well for tNAA and tCho across vendors and algorithms, but substantially less so for Glx and mI, with mI systematically estimated as lower by Tarquin. The cohort mean coefficient of determination for all pairs of LCM algorithms across all datasets and metabolites was R 2 ¯ = 0.39, indicating generally only moderate agreement of individual metabolite estimates between algorithms. There was a significant correlation between local baseline amplitude and metabolite estimates (cohort mean R 2 ¯ = 0.10). While mean estimates of major metabolite complexes broadly agree between linear-combination modeling algorithms at group level, correlations between algorithms are only weak-to-moderate, despite standardized preprocessing, a large sample of young, healthy and cooperative subjects, and high spectral quality. This study aimed to examine the personality changes before and after an FV incident using Weibo data. Samples were selected from 1.16 million Weibo users in China who had posted their own FV experience as victims. We used Linguistic Inquiry and Word Count (LIWC) to extract the linguistic features of these unstructured texts as the scores of participants' personality. We built prediction models to measure and compare personality differences between the victim group and control group in Sample 1; and personality changes between the victim group and control group before and after an FV incident in Sample 2. Results showed that the victims' neuroticism increased and conscientiousness decreased after experiencing FV. link2 At the same time, their agreeableness and openness levels were lower than those of the control group. Implications and limitations are also discussed.During the early stage of biomarker discovery, high throughput technologies allow for simultaneous input of thousands of biomarkers that attempt to discriminate between healthy and diseased subjects. In such cases, proper ranking of biomarkers is highly important. Common measures, such as the area under the receiver operating characteristic (ROC) curve (AUC), as well as affordable sensitivity and specificity levels, are often taken into consideration. Strictly speaking, such measures are appropriate under a stochastic ordering assumption, which implies, without loss of generality, that higher measurements are more indicative for the disease. Such an assumption is not always plausible and may lead to rejection of extremely useful biomarkers at this early discovery stage. We explore the length of a smooth ROC curve as a measure for biomarker ranking, which is not subject to directionality. We show that the length corresponds to a ϕ divergence, is identical to the corresponding length of the optimal (likelihood ratio) ROC curve, and is an appropriate measure for ranking biomarkers. We explore the relationship between the length measure and the AUC of the optimal ROC curve. We then provide a complete framework for the evaluation of a biomarker in terms of sensitivity and specificity through a proposed ROC analogue for use in improper settings. In the absence of any clinical insight regarding the appropriate cutoffs, we estimate the sensitivity and specificity under a two-cutoff extension of the Youden index and we further take into account the implied costs.  In spring 2020, the COVID-19 pandemic required far-reaching changes and measures of unprecedented extent. These measures were implemented to reduce virus spread and to ensure the continuity of nation-wide medical care, in particular with a view to having sufficient intensive-care capacities in case of a large caseload of patients infected with COVID-19. With regard to surgical specialties, this implied a temporary hold on elective cases for an indefinite period of time. The aim of this study was to assess the impact of these measures on the caseload of a level-three plastic surgery unit.

 This study retrospectively assessed the caseload at the Division of Plastic, Aesthetic and Reconstructive Surgery at Medical University Graz during the so-called lockdown from 16 March 2020 to 27 April 2020 (6 weeks) as well as two weeks before. The Doppler device settings should include a low pulse repetition frequency ( less then  4 cm/s), a low wall filter ( less then  100 Hz), and adequate gain. Comparison with the unaffected healthy testis is essential. The most important of the three diseases is torsion of the spermatic cord because it requires immediate surgical intervention and detorsion. The affected testis is enlarged and has an inhomogeneous echotexture with hypoechoic and hyperechoic areas as well as an associated hydrocele. The preconception, early pregnancy, and interconception periods represent opportune windows to engage women in preventive and health promotion interventions. This review provides an overview of findings from observational and intervention studies on the role of diet, physical activity, and weight (change) during these periods in the primary prevention of GDM. Current evidence suggests that supporting women to increase physical activity and achieve appropriate weight gain during early pregnancy and enabling women to optimize their weight and health behaviors prior to and between pregnancies have the potential to reduce rates of GDM. Translation of current evidence into practice requires further development and evaluation of co-designed interventions across community, health service, and policy levels to determine how women can be reached and supported to optimize their health behaviors before, during, and between pregnancies to reduce GDM risk.Singapore currently has one of highest number of confirmed COVID-19 cases in Southeast Asia. To curb the further spread of COVID-19, Singapore government announced a temporary nationwide lockdown (circuit breaker). In view of restrictions of patients' mobility and the enforcement of safe distancing measures, usual in-person visits were discouraged. Here we describe how diabetes care delivery was ad hoc redesigned applying a telehealth strategy. We describe a retrospective assessment of subjects with diabetes, with and without COVID-19 infection, during the circuit breaker period of 7th April to 1st June 2020 managed through Tan Tock Seng Hospital's telehealth platform. The virtual health applications consisted of telephone consultations, video telehealth visits via smartphones, and remote patient monitoring. The TTSH team intensively managed 298 diabetes patients using a telehealth strategy. The group comprised of (1) 84 inpatient COVID-19 patients with diabetes who received virtual diabetes education and blood glucose management during their hospitalisation and follow-up via phone calls after discharge and (2) 214 (n=192 non-COVID; n=22 COVID-positive) outpatient subjects with suboptimal glycaemic control who received intensive diabetes care through telehealth approaches.
