Severe coronavirus disease (COVID-19) is characterized by an excessive proinflammatory cytokine storm, resulting in acute lung injury and development of acute respiratory distress syndrome (ARDS). The role of corticosteroids is controversial in severe COVID-19 pneumonia and associated hyper-inflammatory syndrome. We reported a case series of six consecutive COVID-19 patients with severe pneumonia, ARDS and laboratory indices of hyper-inflammatory syndrome. All patients were treated early with a short course of corticosteroids, and clinical outcomes were compared before and after corticosteroids administration. All patients evaded intubation and intensive care admission, ARDS resolved within 11.8 days (median), viral clearance was achieved in four patients within 17.2 days (median), and all patients were discharged from the hospital in 16.8 days (median). Early administration of short course corticosteroids improves clinical outcome of patients with severe COVID-19 pneumonia and evidence of immune hyperreactivity.Background Insulin resistance is associated with higher all-cause and cancer-specific mortality in postmenopausal women. However, to the authors' knowledge, information regarding insulin resistance and breast cancer mortality risk is limited. Therefore, the authors examined associations between insulin resistance and breast cancer incidence and mortality in a subsample of Women's Health Initiative participants. Methods A total of 22,837 postmenopausal women with fasting baseline glucose and insulin levels were followed for incident breast cancer and breast cancer mortality. Breast cancers were verified by medical record review and serial National Death Index linkage-enhanced mortality findings. Insulin resistance was estimated using the homeostatic model assessment of insulin resistance (HOMA-IR). Multivariable Cox proportional hazards models were used to compute hazard ratios (HRs) with 95% confidence intervals (95% CIs) for quartile comparisons. Outcomes included breast cancer incidence, deaths from breast cancer, and deaths after breast cancer (breast cancer followed by death from any cause). Results During a median of 19.8 years of follow-up of 1328 breast cancer cases, there were 512 deaths reported, 151 of which were from breast cancer. Breast cancer incidence was higher in women in the highest HOMA-IR quartile (HR, 1.34; 95% CI, 1.12-1.61 [P for trend = .003]). Although HOMA-IR was not found to be associated with risk of death from breast cancer (HR, 1.04; 95% CI, 0.60-1.79), women in the highest versus those in the lowest HOMA-IR quartile were at a higher risk of death after breast cancer (HR, 1.78; 95% CI, 1.32-2.39 [P for trend less then .001]). All patients were enrolled from the Department of Dermatology between February 12 and May 8, 2020, the duration of 4 weeks before COVID-19 and eight weeks after were analyzed in 3 parts consisting of 4 weeks. Data obtained from the database such as age, gender, diagnoses were anonymized. Repeated applications with the same diagnosis in ten days after the first presentation were ignored. Compared to the pre-outbreak, there was a 3.5-fold decrease in dermatology applications in the first month after COVID-19 and an 8.8-fold in the second month. Herein, a fibrous hydroxylated MXene/carbon nanotubes (h-Ti 3 C 2 /CNTs) composite is designed as a scaffold for dendrite-free Na metal electrode. This composite displays fast Na + /electron transport kinetics, satisfied thermal conductivity and mechanical property. The h-Ti 3 C 2 contains abundant sodiophilic functional groups, which play a significant role in inducing homogeneous nucleation of Na. Meanwhile, CNTs provide high tensile strength and ease film-forming. As a result, h-Ti 3 C 2 /CNTs exhibit a high average Coulombic efficiency of 99.2% and no dendrite after 1000 cycles. The h-Ti 3 C 2 /CNTs/Na based symmetric cells show a long lifespan over 4000 h at 1.0 mA·cm -2 with a capacity of 1.0 mAh·cm -2 . Furthermore, Na-O 2 batteries with h-Ti 3 C 2 /CNTs/Na anode exhibit a low potential gap of 0.11 V after an initial 70 cycles. This work paves the way to construct stable Na anode by hydroxylated MXene materials.Background & aims Previous small studies have appraised the gut microbiome (GM) in steatosis, but large-scale studies are lacking. We studied the association of GM diversity and composition, plasma metabolites, predicted functional metagenomics and steatosis. Approach & Results This is a cross-sectional analysis of the prospective population-based Rotterdam Study. We used 16Sribosomal-RNA gene sequencing and determined taxonomy using the Silva-reference database. Alpha-and beta-diversity were calculated using Shannon-index and Bray-Curtis dissimilarities. Differences were tested across steatosis using PerMANOVA. Hepatic steatosis was diagnosed by ultrasonography. We subsequently selected genera using regularized regression. The functional metagenome was predicted based on the GM using KEGG-pathways. Serum metabolomics were assessed using high-throughput proton nuclear magnetic resonance. All analyses were adjusted for age, sex, BMI, alcohol, diet, and proton-pump inhibitors. We included 1355 participants of which 472 had steatosis. Alpha-diversity was lower in steatosis (P=1.1∙10-9 ) and beta-diversity varied across steatosis strata (P=0.001). Lasso selected 37 genera of which three remained significantly associated after adjustment (Coprococcus3 β=-65; Ruminococcus Gauvreauiigroup β=62; and Ruminococcus Gnavusgroup β=45, Q-value =0.037). Predicted metagenome analyses revealed that pathways of secondary bile-acid synthesis and biotin metabolism were present and D-alanine metabolism was absent in steatosis. Metabolic profiles showed positive associations for aromatic-and branched chain amino acids and glycoprotein acetyls with steatosis and R. Gnavusgroup, whereas these metabolites were inversely associated with alpha-diversity and Coprococcus3. Conclusions We confirmed, for the first time on a large-scale, the lower microbial diversity and association of Coprococcus and Ruminococcus Gnavus with steatosis. We additionally showed that steatosis and alpha-diversity share -opposite- metabolic profiles.In this study, we performed a single-centered study of 307 severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infected patients. Many patients with PSF experience cognitive difficulties, but studies aiming to identify cognitive correlates of PSF have been largely inconclusive. With the aim of characterizing the relationship between subjective fatigue and attentional function, we collected behavioral data using the attention network test (ANT) and self-reported fatigue scores using the fatigue severity scale (FSS) from 53 stroke patients. In order to evaluate the utility and added value of computational modeling for delineating specific underpinnings of response time (RT) distributions we fitted a hierarchical drift diffusion model (hDDM) to the ANT data. Results revealed a relationship between fatigue and RT distributions. Specifically, there was a positive interaction between FSS score and elapsed time on RT. Group analyses suggested that patients without PSF increased speed during the course of the session, while patients with PSF did not show any changes.
