Adaptation to military operational stress is a complex physiological response that calls upon the sympathetic nervous system (SNS), hypothalamic pituitary adrenal (HPA) axis and immune system, to create a delicate balance between anabolism and catabolism and meet the demands of an ever-changing environment. As such, resilience, the ability to withstand and overcome the negative impact of stress on military performance, is likely grounded in an appropriate biological adaptation to encountered stressors. Neuroendocrine [i.e. cortisol, epinephrine (EPI), norepinephrine (NE), neuropeptide-Y (NPY), and brain derived neurotropic factor (BDNF)], inflammatory [i.e. interleukin 6 (IL-6), IL-1β, IL-4, IL-10 and tumour necrosis factor (TNF)-α], as well as growth and anabolic [i.e. insulin-like growth factor-I (IGF-I), testosterone, and dehydroepiandrosterone (DHEA)] biomarkers independently and interactively function in stress adaptations that are associated with a soldier's physical and psychological performance. In this narrative review, we detail biomarkers across neuroendocrine, inflammatory, and growth stimulating domains to better elucidate the biological basis of a resilient soldier. The findings from the reviewed studies indicate that military readiness and resiliency may be enhanced through better homeostatic control, better regulated inflammatory responses, and balanced anabolic/catabolic processes. It is unlikely that one class of biomarkers is better for assessing physiological resilience. Therefore, a biomarker panel that can account for appropriate balance across these domains may be superior in developing monitoring frameworks. Real-time physiological monitoring to assess biomarkers associated with resilience will be possible pending more sophisticated technologies and provide a field-expedient application for early identification and intervention of at-risk soldiers to improve military resiliency.

Hallux sesamoid injuries are well described and can be debilitating and chronically disabling. The role of orthobiologics such as platelet-rich plasma (PRP) in sesamoid injuries has not been reported. This study describes three cases of recalcitrant hallux sesamoid injuries in teenage athletes who returned to impact activities, pain free, following one treatment of PRP.

This is a case-series study describing three teenage athletes presenting to a tertiary level pediatric sports medicine practice with chronic hallux sesamoid injuries.

The three patients (two female, one male) described in this case series were 13-, 16-, and 17-year-old athletes. Their primary sports were ballet, basketball, and Irish step dance, respectively. All three athletes received PRP two received unilateral treatment (one tibial sesamoid, one fibular sesamoid) and one received treatment to bilateral tibial sesamoids. The average duration of symptoms prior to PRP was 52.5weeks (14-128weeks). The average time out of their primary spributed to return to impact activities, this case series sets the groundwork for future research investigating the role of PRP with needle fenestration in the treatment of sesamoid injuries.

Medical records of dogs (n = 5) that had been treated for patellar fracture with transarticular external skeletal fixation (TA-ESF) to augment internal fixation, at a single referral hospital in the United Kingdom between 2015 and 2017, were reviewed.

At presentation, two dogs had polar patellar fractures, two had comminuted fractures and one dog had a transverse fracture. The median age at the time of the surgery was 21 (min 8, max 132) months and the median body weight was 19.0 (min 8.3, max 28.6) kg. In all cases, TA-ESF (Type IA lateral triangulated or modified Type II) was used in combination with internal fixation with pins and/or orthopaedic wire, supported by nylon leader line (patella-to-tibia mattress suture) and/or absorbable suture in a locking loop and/or circum-patellar pattern. All cases had short-term (6-12 weeks) post-operative radiographic follow-up, which showed evidence of fracture healing in 2/5 cases. All TA-ESF were removed 6 or 7 weeks post-operatively and four dogs had minor complications related to TA-ESF. More than 2 years post-operatively, the owners of all dogs were contacted and questioned using the Liverpool Osteoarthritis in Dogs questionnaire regarding the mobility of their pet. Four of the five cases were re-examined to evaluate their long-term outcomes. Based on the results of goniometric measurement of stifle range of motion, subjective gait assessment and objective gait analysis with a pressure-sensitive walkway, all dogs showed a satisfactory outcome at the final follow-up.

For overhead throws, execution of the 'eggbeater kick' (cyclical movement of the legs) is required to lift the body out of the water. Although a symmetrical action, inter-limb differences in task execution could lead to knee frontal plane projection (FPPA) differences. The present study examined imbalances shoulder and knee FPPA in female players. Eighteen competitive female field players (24.1 ± 5.5 years, 1.68 ± 0.06 m, 72.9 ± 13.3 kg) had their shoulder strength assessed in a shot-mimicking position with a portable dynamometer, standing and seated (isolating the shoulder contribution). Anterior posterior and shooting non-shooting shoulder comparison were made. Additionally, players performed a drop jump. Knee FPPA was recorded from digitising and comparing the frames just before landing and at stance phase. During standing, players exhibited higher shooting non-shooting asymmetry (p = 0.032) in the anterior contraction direction, while during seated the shooting shoulder anterior posterior asymmetry was higher (p = 0.032). Interlimb knee FPPA asymmetry was higher in the stance phase (p = 0.02). Participants were then asked to make a series of judgments on the basis of the photos alone. It was found that the degree of muscularity of the targets shaped participants' estimates of perceived outcomes and of the targets' traits in non-bodybuilding domains (e.g., expected positive life outcomes, gender role traits, and sexual orientation). Although lncRNA MCM3AP antisense RNA 1 (MCM3AP-AS1) has been reported to be an oncogenic factor in NSCLC, its detailed mechanism in NSCLC is unknown. In this study, quantitative real-time polymerase chain reaction (qRT-PCR) was performed to determine MCM3AP-AS1, microRNA (miR)-195-5p and E2F transcription factor 3 (E2F3) mRNA expressions in NSCLC tissues and cells. Western blot was utilized to determine the expression levels of E2F3, BCL2-associated X protein (Bax), B-cell lymphoma-2 (Bcl-2), E-cadherin and N-cadherin. CCK-8 and Transwell assays were conducted to examine cell proliferation, migration and invasion, respectively. Dual-luciferase reporter assay and RNA immunoprecipitation experiments were used to determine the regulatory relationships between MCM3AP-AS1 and miR-195-5p, and miR-195-5p and E2F3. We demonstrated that MCM3AP-AS1 was overexpressed in NSCLC tissues and cells, and MCM3AP-AS1 overexpression accelerated the proliferation, migration and invasion of NSCLC cells. In addition, MCM3AP-AS1 overexpression markedly up-modulated Bcl-2 expression and repressed Bax expression; MCM3AP-AS1 overexpression also significantly up-regulated N-cadherin expression and suppressed E-cadherin expression in NSCLC cells. What is more, in NSCLC cells, miR-195-5p was a target of MCM3AP-AS1, and the latter worked as a molecular sponge for miR-195-5p to regulate E2F3 expression. Collectively, MCM3AP-AS1, serving as a competitive endogenous RNA (ceRNA) to regulate miR-195-5p/E2F3 axis, promotes NSCLC progression, which is a promising therapeutic target for NSCLC.This study aimed to systematically review training methods prescribed to develop lower-body power, determine their effectiveness for the development of lower-body mechanical power and their implementation in an annual training cycle amongst team-sport athletes. The absolute and relative outcome values of concentric mean power, peak power and mean propulsive power were extracted from 19 studies. Outcomes were assessed using baseline to post intervention percent change, effect sizes, and the level of evidence concerning the method's effectiveness. A thorough analysis of the literature indicated that, based on the high level of evidence, traditional (e.g., strength training alone) and combination training (e.g., complex and contrast) methods should be considered. Further, optimal load and velocity-based training can be implemented if coaches have access to the appropriate equipment to monitor movement velocity and mechanical power in every session.
