Long noncoding RNAs (lncRNAs) play crucial roles in cardiovascular diseases. To date, only limited studies have reported the role of mitochondria-derived lncRNAs in heart failure (HF). In the current study, recombinant adeno-associated virus 9 was used to manipulate lncRNA cytb (lnccytb) expression in vivo. Fluorescence in situ hybridization (FISH) assay was used to determine the location of lnccytb, while microRNA (miRNA) sequencing and bioinformatics analyses were applied to identify the downstream targets. The competitive endogenous RNA (ceRNA) function of lnccytb was evaluated by biotin-coupled miRNA pull-down assays and luciferase reporter assays. Results showed that lnccytb expression was decreased in the heart of mice with transverse aortic constriction (TAC), as well as in the heart and plasma of patients with HF. FISH assay and absolute RNA quantification via real-time reverse transcription PCR suggested that the reduction of the lnccytb transcripts mainly occurred in the cytosol. Upregulation of cytosolic lnccytb attenuated cardiac dysfunction in TAC mice. Moreover, overexpression of cytosolic lnccytb in cardiomyocytes alleviated isoprenaline-induced reactive oxidative species (ROS) production and hypertrophy. Mechanistically, lnccytb acted as a ceRNA via sponging miR-103-3p, ultimately mitigating the suppression of PTEN by miR-103-3p. In summary, we demonstrated that the overexpression of cytosolic lnccytb could ameliorate HF.Synthetic siRNA guide strands are typically designed with perfect complementarity to the passenger strand and the target mRNA. We examined whether siRNAs with intentional guide-strand bulges are functional in vitro and in vivo. Importantly, this was done by systematic shortening of the passenger strand, evaluating identical 19-mer guide-strand sequences but forcing them into conformations with 1- to 4-nt bulges after annealing. We demonstrate that guide-strand bulges can be well tolerated at several positions of unmodified and modified siRNAs. Beyond that, we show that GalNAc-conjugated siRNAs with bulges at certain positions of the guide strand repress transthyretin in murine primary hepatocytes and in vivo in mice. In vivo, a GalNAc-conjugated siRNA with a 1-nt bulge at position 14 of the guide strand was as active as the perfectly complementary siRNA. Finally, in a luciferase reporter system, mRNA target sequences were systematically shortened so that RNA-induced silencing complex activity could only occur with a guide-strand bulge. Here, luciferase reporters were repressed when 1- and 2-nt deletions of the reporter were applied to the edges of the sequence. We conclude that some guide-strand bulges versus target transcript can result in target repression and therefore should be evaluated as off-target risks.

Assessment of medical students' clinical skills (CS) remains a challenge. Little is known about early predictors of future CS performance. This study examines the relationship between students' pre-clerkship clinical skills (PCCS) performance and year 3 clerkship performance measures.

The authors performed a retrospective analysis of four medical student cohorts who matriculated between 2014 and 2017 and participated in a longitudinal pre-clerkship CS curriculum. A total of 440 students were included in the analyses. Students' clinical skills were assessed through a series of PCCS exams, each consisting of a single standardized patient encounter. First-year PCCS exams assessed history taking, physical examination, professionalism, and communication skills; second-year PCCS exams also assessed clinical documentation and clinical reasoning skills. Evaluators assigned a grade of "satisfactory," "borderline," or "unsatisfactory" for each skill set. Regression analyses compared year 3 performance outcomes betwcy in the clerkship environment and future medical training.

This study aimed to perform a critical appraisal of WhatsApp use by undergraduate students in dental education.

A PRISMA-Scoping Review was carried out to explain (i) how has WhatsApp been used as a learning tool in dental education? and (ii) how has WhatsApp been evaluated as a learning tool in dental education? Inclusion criteria are specified as the population (dental students), concept (use of WhatsApp), context (institutions/universities of dental education), and types of sources of evidence (peer-reviewed publication).

Nine studies published between 2016 and 2020 were included in this scoping review. Mostly had a cross-sectional design.

However, the low contrast of areas of inflammation on CT images, areas of infection are difficult to identify. The purpose of this study is to develop a post-image-processing method for quantitative analysis of COVID-19 pneumonia-related changes in CT attenuation values using a pixel-based analysis rather than more commonly used clustered focal pneumonia volumes. The COVID-19 pneumonia burden is determined by experienced radiologists in the clinic. Previous AI software was developed for the measurement of COVID-19 lesions based on the extraction of local pneumonia features. In this respect, changes in the pixel levels beyond the clusters may be overlooked by deep learning algorithms. The proposed technique focuses on the quantitative measurement of COVID-19 related pneumonia over the entire lung in pixel-by-pixel fashion rather than only clustered focal pneumonia volumes. Pixel-based techniques produce more sensitive results than AI techniques. Despite the high success rate of radiofrequency ablation of the left lateral accessory pathways, a rather uncommon manifestation is intra-atrial conduction block at the level of the mitral isthmus. We report 2 cases of orthodromic atrioventricular reentrant tachycardia using a concealed left-sided accessory pathway with an abrupt change in the activation of the coronary sinus from an eccentric to concentric sequence after ablation delivery. The electrophysiological characteristics and the underlying mechanism of the intra-atrial conduction block are commented on. Careful mapping and assessment of relative conduction are helpful to document the diagnosis of intra-atrial conduction block. Familiarity with the likelihood of intra-atrial block during left lateral accessory pathway ablation is needed to avoid the erroneous elucidation that a second accessory pathway is present and to identify correctly the ablation site of interest.To accommodate the surge in patients with coronavirus disease 2019 during the spring of 2020, outpatient areas in our health system were repurposed as inpatient units. These spaces often lacked the same resources as the standard inpatient unit, including telemetry equipment. We utilized mobile cardiac outpatient telemetry (MCOT) in place of traditional telemetry and suggest that MCOT is an appropriate substitution only for patients at low risk of developing arrhythmia given the prolonged time to notification of the care team regarding events and imprecise measurements of the corrected QT interval when compared to 12-lead electrocardiography.
