Obstructive sleep apnea (OSA) is a common cause of hypertension. Previous studies have demonstrated beneficial short-term effects of continuous positive airway pressure (CPAP) therapy on blood pressure. However, long-term antihypertensive effects of CPAP have not been properly verified. This study examined the longitudinal effect of CPAP therapy adherence on blood pressure among OSA patients. All patients diagnosed with OSA and undergoing subsequent CPAP therapy at a Kanagawa-area sleep clinic were clinically followed for 24 months to examine CPAP adherence, as well as longitudinal changes in blood pressure and body weight because it may become a confound factor for changes in blood pressure. The hours of CPAP usage were collected over the course of 30 nights prior to each follow-up visit (1st, 3rd, 6th, 12th, and 24th month). The relationship between CPAP adherence and blood pressure was analyzed using mixed-effect logistic regression models. A total of 918 OSA patients were enrolled in the study. We found a significant reduction in diastolic blood pressure among patients with good CPAP adherence during the 24-month follow-up period (β = - 0.13, p = 0.03), when compared to the group with poor CPAP adherence. No significant association was found between CPAP adherence and weight loss (β = - 0.02, p = 0.59). Long-term, good CPAP therapy adherence was associated with lower diastolic blood pressure without significant weight loss.Animal models have expanded our understanding of temporal lobe epilepsy (TLE). However, translating these to cell-specific druggable hypotheses is not explored. Herein, we conducted an integrative insilico-analysis of an available transcriptomics dataset obtained from animals with pilocarpine-induced-TLE. A set of 119 genes with subtle-to-moderate impact predicted most forms of epilepsy with ~ 97% accuracy and characteristically mapped to upregulated homeostatic and downregulated synaptic pathways. The deconvolution of cellular proportions revealed opposing changes in diverse cell types. The proportion of nonneuronal cells increased whereas that of interneurons, except for those expressing vasoactive intestinal peptide (Vip), decreased, and pyramidal neurons of the cornu-ammonis (CA) subfields showed the highest variation in proportion. A probabilistic Bayesian-network demonstrated an aberrant and oscillating physiological interaction between nonneuronal cells involved in the blood-brain-barrier and Vip interneurons in driving seizures, and their role was evaluated insilico using transcriptomic changes induced by valproic-acid, which showed opposing effects in the two cell-types. AZD5991 Additionally, we revealed novel epileptic and antiepileptic mechanisms and predicted drugs using causal inference, outperforming the present drug repurposing approaches. These well-powered findings not only expand the understanding of TLE and seizure oscillation, but also provide predictive biomarkers of epilepsy, cellular and causal micro-circuitry changes associated with it, and a drug-discovery method focusing on these events.Seaweeds are now recognized as a treasure of bioactive compounds. However, the bioactivity of seaweed originating in Bangladesh is still unexplored. So, this study was designed to explore the secondary metabolites and antioxidant activities of solvent extracts of Padina tetrastromatica and Gracilaria tenuistipitata. Phytochemical screening and FTIR spectra confirm the diverse type of bioactive compounds. Antioxidant activity of extracts were evaluated by 1,1-diphenyl-2-picrylhydrazyl (DPPH), 2, 2-Azino-bis (3-ethylbenzothiazoline-6-sulfonic acid) (ABTS), reducing power (RP), phosphomolybdenum, hydrogen peroxide and nitric oxide (NO) scavenging assays. Here, methanolic extract of P. tetrastromatica showed highest amount of total phenolic content (85.61 mg of GA/g), total flavonoid content (41.77 mg of quercetin/g), DPPH (77.07%), ABTS (77.65%), RP (53.24 mg AAE/g), phosphomolybdenum (31.58 mg AAE/g), hydrogen peroxide (67.89%) and NO (70.64%) assays compared to its methanolic extracts of G. tenuistipitata. This study concluded that methanol as a solvent extract of brown seaweed (P. tetrastromatica) exhibited bioactivity and antioxidant potentiality which will be useful for pharmacological as well as in functional food application.Pleural mesothelioma is an aggressive malignancy with limited effective therapies. In order to identify therapeutic targets, we integrated SNP genotyping, sequencing and transcriptomics from tumours and low-passage patient-derived cells. Previously unrecognised deletions of SUFU locus (10q24.32), observed in 21% of 118 tumours, resulted in disordered expression of transcripts from Hedgehog pathways and the T-cell synapse including VISTA. Co-deletion of Interferon Type I genes and CDKN2A was present in half of tumours and was a predictor of poor survival. We also found previously unrecognised deletions in RB1 in 26% of cases and show sub-micromolar responses to downstream PLK1, CHEK1 and Aurora Kinase inhibitors in primary mesothelioma cells. Defects in Hippo pathways that included RASSF7 amplification and NF2 or LATS1/2 mutations were present in 50% of tumours and were accompanied by micromolar responses to the YAP1 inhibitor Verteporfin. Our results suggest new therapeutic avenues in mesothelioma and indicate targets and biomarkers for immunotherapy. The purpose of this study is to evaluate utility of MRI in differentiation of uterine low-grade endometrial stromal sarcoma (LGESS) from rare leiomyoma variants. This multi-center retrospective study included consecutive 25 patients with uterine LGESS and 42 patients with rare leiomyoma variants who had pretreatment MRI. Two radiologists (R1/R2) independently evaluated MRI features, which were analyzed statistically using Fisher's exact test or Student's t-test. Subsequently, using a five-point Likert scale, the two radiologists evaluated the diagnostic performance of a pre-defined MRI system using features reported as characteristics of LGESS in previous case series uterine tumor with high signal intensity (SI) on diffusion-weighted images and with either worm-like nodular extension, intra-tumoral low SI bands, or low SI rim on T2-weighted images. Area under the receiver operating characteristic curve (AUC), sensitivity, and specificity of the two readers' Likert scales were analyzed. Intra-tumoral low SI bands (p  less then  0.001), cystic/necrotic change (p ≤ 0.02), absence of speckled appearance (p  less then  0.001) on T2-weighted images, and a low apparent diffusion coefficient value (p ≤ 0.02) were significantly associated with LGESS. The pre-defined MRI system showed very good diagnostic performance AUC 0.86/0.89, sensitivity 0.95/0.95, and specificity 0.67/0.69 for R1/R2. MRI can be useful to differentiate uterine LGESS from rare leiomyoma variants.A black hole laser in analogues of gravity amplifies Hawking radiation, which is unlikely to be measured in real black holes, and makes it observable. There have been proposals to realize such black hole lasers in various systems. However, no progress has been made in electric circuits for a long time, despite their many advantages such as high-precision electromagnetic wave detection. Here we propose a black hole laser in Josephson transmission lines incorporating metamaterial elements capable of producing Hawking-pair propagation modes and a Kerr nonlinearity due to the Josephson nonlinear inductance. A single dark soliton obeying the nonlinear Schrödinger equation produces a black hole-white hole horizon pair that acts as a laser cavity through a change in the refractive index due to the Kerr effect. We show that the resulting laser is a squeezed-state laser characterized by squeezing parameters. We also evaluate the degree of quantum correlation between Hawking and its partner radiations using entanglement entropy which does not require simultaneous measurements between them. As a result, the obtained entanglement entropy depending on the soliton velocity provides strong evidence that the resulting laser is derived from Hawking radiation with quantum correlation generated by pair production from the vacuum.Autoimmune and autoinflammatory inner ear diseases (AIED/AID) are characterized by the symptom of sensorineural hearing loss (SNHL). To date, standardized diagnostic tools for AIED/AID are lacking, and clinically differentiating AIED/AID from chronic otitis media (COM) with SNHL is challenging. This retrospective study aimed to construct a magnetic resonance imaging (MRI)-based decision tree using classification and regression tree (CART) analysis to distinguish AIED/AID from COM. In total, 67 patients were enrolled between January 2004 and October 2019, comprising AIED/AID (n = 18), COM (n = 24), and control groups (n = 25). All patients underwent 3 T temporal bone MRI, including post-contrast T1-weighted images (postT1WI) and post-contrast FLAIR images (postFLAIR). Two radiologists evaluated the presence of otomastoid effusion and inner ear contrast-enhancement on MRI. A CART decision tree model was constructed using MRI features to differentiate AIED/AID from COM and control groups, and diagnostic performance was analyzed. High-intensity bilateral effusion (61.1%) and inner ear enhancement (postFLAIR, 93.8%; postT1WI, 61.1%) were the most common findings in the AIED/AID group. We constructed two CART decision tree models; the first used effusion amount as the first partitioning node and postT1WI-inner ear enhancement as the second node, whereas the second comprised two partitioning nodes with the degree of postFLAIR-enhancement of the inner ear. The first and second models enabled distinction of AIED/AID from COM with high specificity (100% and 94.3%, respectively). The amount of effusion and the degree of inner ear enhancement on MRI may facilitate the distinction between AIED/AID and COM with SNHL using decision tree models, thereby contributing to early diagnosis and intervention.A theoretic model to investigate the dynamics of the longitudinal nucleus-acoustic waves (NAWs) in gyrogravitating electrostatically confined degenerate quantum plasma (DQP) system in spherically symmetric geometry is constructed. The model setup consists of non-degenerate heavy nuclear species (HNS), lighter nuclear species (LNS), and quantum degenerate electronic species (DES). It specifically considers the influences of the Bohm potential, Coriolis rotation, viscoelasticity, and electrostatic confinement pressure (ECP, scaling quadratically in density). A standard normal spherical mode analysis gives a generalized dispersion relation (septic). It highlights the dependency of various atypical instability response on the equilibrium plasma parameters. A numerical illustrative platform portrays that the relative nuclear charge-to-mass coupling parameter ([Formula see text]) acts as a destabilizing agency and the heavy-to-light nuclear charge density ratio ([Formula see text]) acts as a stabilizing agency in both the non-relativistic (NR) and ultra-relativistic (UR) limits. Another interesting conjuncture is that the Coriolis rotation introduces a destabilizing influence on the system in both the limits. The progressive analysis presented herein has correlations and consistencies in the dynamic growth backdrop of various compact astro objects and their circumvent atmospheres, such as white dwarfs, neutron stars, etc.
