In 3 patients, aspirated material was suctioned from the trachea and bronchi using the ultra-slim gastroscope. Immediate post-procedure extubation was successful in all endoscopist-facilitated intubation patients and none exhibited radiographic evidence of aspiration pneumonia. Conclusions Endoscopist-facilitated intubation using an ultra-slim flexible gastroscope is feasible and expeditious for MAC to GA conversion during ERCP. This technique is readily accomplished in the semi-prone position, while standard intubation requires patient transfer from fluoroscopy table to gurney, with associated delay/risks. These data suggest that further study of this approach is warranted, and this may be the most favorable approach for intubation during ERCP.For Rokitansky, pathological anatomy was not only a descriptive but also an explanatory science. Since the changes in the solidis (morphological conditions) did not always adequately explain the cause of death, Rokitansky considered that the answers to his research questions were to be found in a combination of solidar pathology and humoral pathology. Many reviewers claim that Rokitansky had fallen back into the "old" humoral pathology. However, Rokitansky's approach to humoral pathology also included the analysis of blood, blood plasma, blood serum, secretions, excretions, lymph and exudates in pathological anatomy. In his research, Rokitansky referred to "the oxidation of protein to fiber", "illnesses of the protein" and "illnesses of the fibrin". Rokitansky postulated that the progression of a disease was determined by the different forms of protein and fibrin in the blood. From his point of view, diseased blood components (dyscrasias) influence the tissue in its cellular and intercellular dimensions. He sought to generate awareness of this interaction in his Krasenlehre, which was criticised by Rudolf Virchow. Only in the past thirty years has research confirmed the visionary dimension of Rokitansky's humoral pathology."A quick glance at selected topics in this issue" aims to highlight contents of the Journal and provide a quick review to the readers.Lung cancer is the deadliest cancer worldwide. It has been shown that early detection using low-dose computer tomography (LDCT) scans can reduce deaths caused by this disease. We present a general framework for the detection of lung cancer in chest LDCT images. Our method consists of a nodule detector trained on the LIDC-IDRI dataset followed by a cancer predictor trained on the Kaggle DSB 2017 dataset and evaluated on the IEEE International Symposium on Biomedical Imaging (ISBI) 2018 Lung Nodule Malignancy Prediction test set. Our candidate extraction approach is effective to produce accurate candidates with a recall of 99.6%. In addition, our false positive reduction stage classifies successfully the candidates and increases precision by a factor of 2000. Our cancer predictor obtained a ROC AUC of 0.913 and was ranked 1st place at the ISBI 2018 Lung Nodule Malignancy Prediction challenge. Graphical abstract.Muscle fatigue negatively affects gait, and the changes in gait pattern due to muscle fatigue is influenced by which muscles are fatigued and pre-existing physical activity levels. However, how these factors alter gait stability and variability, measures related to risk of fall, remains unclear. To explore how muscular fatigue affects linear and nonlinear gait features in young adults, the effects of triceps surae fatigue and weight training level on gait variability and local stability, as well as a 12-min recovery time of post-fatigue period, were evaluated in young adults (trained and untrained groups). Some features were estimated, i.e., (i) step length (SL) and step frequency (SF), (ii) average standard deviation of trunk acceleration along strides (VAR), and (iii) local dynamic stability (LDS; maximum Lyapunov exponent). LDS presented a significant increase in the anterior-posterior direction with recovery to trained group. Liraglutide Glucagon Receptor agonist SL and SF changed immediately post-fatigue and recovered for both groups, while VAR increased significantly in all directions, with a recovery in the vertical direction for both groups and in the medial-lateral direction for trained group. Localized fatigue affected the analyzed gait variables independent of the participant's training condition, and an interval of 12 min does not seem to be enough for a complete recovery, suggesting a longer recovery period after tasks involving localized triceps surae fatigue to guarantee basal levels of gait variability and local stability. Graphical abstract Flow chart of the experimental protocol. A) Pre-fatigue 4 min walking at PWS. B) Post-fatigue first 4 min walking after fatigue protocol. C) Post-fatigue second 4 min walking after fatigue protocol. D) Post-fatigue third 4 min walking after fatigue protocol (PWS, preferred walking speed; AP, anterior-posterior; V, vertical; ML, medial-lateral).Tissue-specific occurrence and formation of endogenous sesquiterpene lactones has been assessed and suggests physiological function as antagonists of auxin-induced plant growth in sunflower. Sunflower, Helianthus annuus, accumulate high concentrations of bioactive sesquiterpene lactones (STL) in glandular trichomes, but in addition, structurally different STL occur in only trace amounts in the inner tissues. The spatial and temporal production of these endogenous STL during early phases of plant development is widely unknown and their physiological function as putative natural growth regulators is yet speculative. By means of HPLC and MS analysis it was shown that costunolide, dehydrocostuslactone, 8-epixanthatin and tomentosin are already present in dry seeds and can be extracted in low amounts from cotyledons, hypocotyls and roots of seedlings during the first days after germination. Semi-quantitative and RT-qPCR experiments with genes of the key enzymes of two independent routes of the endogenous STL biosynthesis confirmed the early and individual expression in these organs and revealed a gradual down regulation during the first 72-96 h after germination. Light irradiation of the plants led to a fast, but transient increase of STL in parts of the hypocotyl which correlated with growth retardation of the stem. One-sided external application of costunolide on hypocotyls conferred reduced growth of the treated side, thus resulting in the curving of the stem towards the side of the application. This indicates the inhibiting effects of STL on plant growth. The putative function of endogenous STL in sunflower as antagonists of auxin in growth processes is discussed.
