Objectives This study first examined whether real-time conscious postural control (reinvestment) and postural sway increase with different postural difficulties on a compliant surface among older adults. The second objective was to investigate the effect of an externally focused dual-task on real-time reinvestment and postural sway under a relatively challenging standing position. Method Thirty-two community-dwelling older adults (mean age = 72.09, SD = 4.18 years) were recruited. Participants performed balance tasks in four standing positions in a randomized order on a balance foam pad. The four positions included wide-based standing on foam (WBF), narrow-based standing on foam (NBF), tandem-based standing on foam (TBF) and tandem-based standing on foam with an externally focused dual-task (TBFE). Throughout all the balance tasks, participants' real-time reinvestment and body sway were indicated by the Alpha2 T3-Fz Electroencephalogram (EEG) coherence and the total sway length (TSL), respectively. Results and Discussion Our results revealed no significant difference in real-time reinvestment among different standing positions while postural sway increased from WBF to NBF and reduced from NBF to TBF. We also demonstrated that when performing a relatively challenging standing task on a compliant surface, an externally focused dual-task (TBFE), compared to a baseline single task (TBF), can neither mitigate real-time reinvestment nor improve balance performance in community-dwelling older adults with good balance capability. Potential explanations and implications are discussed.

Tangwang Mingmu granule (TWMM), a traditional Chinese medicine, has been widely used in the treatment of diabetic retinopathy (DR), the most common microvascular complication in diabetes mellitus.

To establish a method to select target compounds from herbs for a pharmacokinetic study using network pharmacology, which could be applied in clinical settings.

First, UPLC/Q Exactive Q-Orbitrap and GCMS 2010 were used to determine the non-volatile and volatile ingredients of TWMM. Based on the identified compounds, network pharmacology was used to screen the key compounds and targets of TWMM in the treatment of DR. Based on the compound-target-pathway network and identification of components emigrant into blood, the potential compound markers

were chosen. Then, Sprague-Dawley (SD) rats were administrated of TWMM at a 9.6 g/kg dose to investigating pharmacokinetic parameters using the UPLC-QQQ-MS.

Ninety and forty-five compounds were identified by UPLC-MS and GC-MS, respectively. Based on the network phars strategy provides a clinically relevant methodology that allows for the screening of pharmacokinetic markers in Chinese medicines.

Fab antivenom (FabAV) halts progression of tissue injury and improves recovery in copperhead snakebite. It is unknown if F(ab')

AV does as well. The objective of this study was to compare control of tissue injury in copperhead snakebite patients treated with F(ab')

AV versus FabAV.

We performed a post hoc analysis of copperhead envenomated patients in a clinical trial comparing F(ab')

AV to FabAV. The outcomes for this analysis are the number of repeat doses required to obtain initial control, the number of patients requiring unscheduled doses during maintenance, and the time from antivenom administration to initial control.

Twenty-one (13 F(ab')

AV, 8 FabAV) were copperhead patients. Median age was 46 years with a male predominance. Baseline severity was similar. One (8%) F(ab')

AV and 2(25%) FabAV patients required repeat initial dosing, difference = 17%, (95%CI -18, 57%). One (8%) F(ab')

AV and 1(13%) FabAV patients required additional doses after maintenance, difference = 5%, (95%CI -27, 45%). Median time to initial control was 2.7 range (2.0, 9.3) hours and 3.5 range (2.0, 7.4) for F(ab')

AV and FabAV respectively, difference -0.8 h (95% CI -2.6, 0.9).

This exploratory analysis suggests that the available measures of the control of venom-induced tissue injury are similar between antivenom subgroups.

This exploratory analysis suggests that the available measures of the control of venom-induced tissue injury are similar between antivenom subgroups.Road traffic injuries in Nepal are increasing despite being largely preventable. Little evidence exists regarding the barriers and facilitators to a safer road system. This study aimed to explore the perspectives of professionals whose jobs had the potential to influence road safety in Nepal regarding challenges and potential solutions. Semi-structured interviews with eight informants from diverse roles were analysed thematically. Three themes were identified Modifying behaviours of road users; Road planning, construction and maintenance; and the Governance of roads and traffic. All participants considered the primary cause of crashes to be the negligent behavior of the road users, suggesting that improved knowledge would influence their decisions. Poor road design, building and maintenance, together with poor vehicle standards, and lack of investment and enforcement of existing road safety legislation, needed to be addressed through greater coordination of the agencies. The study identified a range of areas for future inquiry and action. The highly contagious whooping cough agent Bordetella pertussis has evolved as a human-restricted pathogen from a progenitor which also gave rise to Bordetella parapertussis and Bordetella bronchiseptica. While the latter colonizes a broad range of mammals and is able to survive in the environment, B. pertussis has lost its ability to survive outside its host through massive genome decay. Instead, it has become a highly successful human pathogen by the acquisition of tightly regulated virulence factors and evolutionary adaptation of its metabolism to its particular niche. By the deployment of an arsenal of highly sophisticated virulence factors it overcomes many of the innate immune defenses. It also interferes with vaccine-induced adaptive immunity by various mechanisms. Here, we review data from in vitro, human and animal models to illustrate the mechanisms of adaptation to the human respiratory tract and provide evidence of ongoing evolutionary adaptation as a highly successful human pathogen.Workplace violence (WPV) is a serious endemic phenomenon in healthcare settings, and it has been escalating during the COVID-19 pandemic. In this cross-sectional study, healthcare workers (HCWs) (105 physicians and 104 nurses) working at two public hospitals accepting patients with COVID-19 in Egypt were included. Using a self-administered questionnaire distributed in January 2021, data about HCWs' sociodemographic and occupational characteristics and their exposure to psychological and physical WPV during the past six months were collected. The results showed that the prevalence of psychological and physical WPV was 42.6% and 9.6%, respectively. Relatives of patients were the perpetrators in most WPV incidents. HCWs did not report 57.3% of psychological and 10.0% of physical WPV incidents. Female sex, having physical contact with patients, and working rotational shifts were associated with the increased exposure to psychological and physical WPV. In conclusion, this study showed a high prevalence of WPV against HCWs in Egyptian public hospitals during the COVID-19 pandemic. We conducted a preregistered study (N = 609) to conceptually replicate and extend prior research regarding the effects of facial redness on emotion perception. In a within-subjects design, participants saw emotion faces (anger, happiness, fear, neutral) of a random female and a random male target with default facial colouration and increased facial redness and were asked to simultaneously rate the intensity of six emotions (happiness, surprise, sadness, fear, disgust, anger) for each emotion face. The emotion intensity was rated higher, when the emotion face and the rated emotion matched than when the emotion face and the rated emotion did not match. However, increased facial redness did not influence the intensity of the rated emotion. The results of this conceptual replication limit the generalisability of previous findings, challenge the assumption that facial redness is used as a cue to infer emotions, and point to the necessity to develop a more nuanced theoretical account of contextual boundaries.

American Indians (AIs) experience substance use disorder and sexual risk related inequities with elevated rates of sexually transmitted infections, unintended pregnancy, and fetal alcohol spectrum disorders. learn more Substance misuse is associated with risky sexual behaviors including condomless sex.

The current study aimed to test the validity of the Condom Use Self-Efficacy Scale (CUSES) and the relationship between self-efficacy and condom use behaviors in AI individuals with substance use disorders (SUDs). Exploratory analyses also examined changes in condom use self-efficacy across SUD treatment.

As part of a larger randomized controlled trial, AI individuals (

= 79) seeking SUD treatment completed baseline measures of condom use self-efficacy and sexual risk behaviors.

Confirmatory factor analysis (CFA) indicated that a 3-factor, 10-item version of the CUSES was a reliable and valid measure in this population. Condom use self-efficacy was associated with condom use behavior and increased during SUD elated health inequities.

The ABILHAND questionnaire is recommended to assess perceived manual ability after stroke; however, more knowledge on interpretability is needed to improve the clinical applicability.

To determine clinically meaningful cutoff scores for different levels of perceived manual ability, assessed by ABILHAND, corresponding to established observed and perceived upper extremity assessments post stroke.

This cross-sectional study, part of the Stroke Arm Longitudinal Study (SALGOT) at the University of Gothenburg, included 80 participants with upper extremity impairments after stroke. The self-reported upper extremity functioning was assessed with ABILHAND and Stroke Impact Scale Hand (SIS Hand), and the observed functioning was assessed by Fugl-Meyer Assessment for Upper Extremity (FMA-UE) and Action Research Arm Test (ARAT) at 3months after stroke. Receiver operating characteristic curve, sensitivity, and specificity analyses were used to determine the cutoffs.

The overall discriminating accuracy was excellent (AUC > 0.
