Base catalysed exchange with sodium hydroxide, calcium oxide or N,N,N,N-tetramethylguanidine in deuterium oxide is a viable procedure for the preparation of terminally deuterated alkynes for those alkynes stable to strong base. The use of silver perchlorate as a catalyst is an alternative practical option when labelling alkynes which are sensitive to base or contain functionalities which would lead to labelling elsewhere in the molecule. Labelling with this catalyst takes place smoothly at ambient temperature in a mixture of N,N-dimethylformamide and deuterium oxide.The premature ventricular contractions (PVCs) have usually good prognosis in patients without structural heart disease. In case of left ventricular ejection fraction depression or symptoms, antiarrhythmic drugs or cardiac ablations could be an option for management. We present a case of a patient with high burden of PVC admitted for cardiac ablation. Preoperative assessment revealed hyperkalemia and metabolic acidosis which ended up with type-4 renal tubular acidosis (RTA). Its rare cause and management may draw attention to the possibility of type -4 RTA as the cause of the PVC, and hyperkalemia.In nutritional epidemiology, measurement error in covariates is a well-known problem since dietary intakes are usually assessed through self-reporting. In this article, we consider an additive error model in which error variables are highly correlated, and propose a new method called approximate profile likelihood estimation (APLE) for covariates measured with error in the Cox regression. Asymptotic normality of this estimator is established under regularity conditions, and simulation studies are conducted to examine the finite sample performance of the proposed estimator empirically. Moreover, the popular correction method called regression calibration is shown to be a special case of APLE. We then apply APLE to deal with measurement error in some nutrients of interest in the EPIC-InterAct Study under a sensitivity analysis framework.

Sport workforce strategy in the United Kingdom (UK) has identified the occupational therapy profession as being ideally positioned to contribute to public health agendas relating to tackling physical inactivity amongst marginalised populations, such as disabled people and people with experience of mental distress. However, a robust understanding of the enablers, restrictions, and exclusions such groups encounter when seeking to participate in sport and physical activity is currently lacking.

This study aimed to gain an in-depth understanding of the different ways people with experience of mental distress talked about their participation in a community-based football league in England, in the UK. Nine people took part in this strand of a larger participatory action research (PAR) study, which used go-along interviews as the method of data collection. In alignment with PAR seeking to address power imbalances, the data from the go-along interviews were analysed through a Foucauldian lens using a collaborativfor wider social change beyond specialist services.

For many patients with primary immune deficiency (PID), stem-cell transplantation (SCT) may be life-saving.

To review our experience of 11years transplanting children with PID in Mexico.

Chart review of patients who underwent SCT from 2008 to 2018, to describe their diagnoses, time to transplant, conditioning regime, survival rate and outcomes. All patients received post-transplant cyclophosphamide as graft-versus-host-disease (GVHD) prophylaxis.

19 patients with combined, phagocytic or syndromic PID from 5 states. Twelve of them were male (58%) and 14 survive (79%). Mean age at HSCT was 41.9months; mean time from diagnosis was 31.2months. Seven grafts were umbilical cord and 12 haploidentical. The conditioning regime was myeloablative, with five primary graft failures. Two patients had partial and 10 full chimerism. Five patients died within 2months after transplant. Immune reconstitution was complete in 11 of 19 patients. We found a prevalence of 21% GVHD.

The influence of psychological disturbances on OLP has been widely discussed but still bears the controversy. This study aimed at assessing scientific evidence between the OLP and psychological alterations of the patient. We searched seven important databases for studies on OLP and psychological factors (anxiety, depression, stress, sleep disorders, etc.) published between 1 January 2000, and 1 October 2020. Case-control and cross-sectional studies were incorporated into this study. The meta-analysis used a random-effects model assessed by using the I2 statistic. Dichotomous variables used the odds ratio and 95% confidence interval (CI), and continuous variables used the mean difference with 95% CI. Finally, 26 studies were included in the review. A meta-analysis of 13 studies showed there was a significant association of OLP with anxiety, depression and stress compared with healthy controls. Certain personality characteristics and sleep disorders also influence the patient of OLP. Therefore, psychological and psychiatric examinations should be carried out routinely for patients with OLP and improve the prognosis of the disease. © 2022 Australian Dental Association.DNA topoisomerase 1 (TOP1) plays general roles in DNA replication and transcription by regulating DNA topology in land plants and metazoans. TOP1 is also involved in specific developmental events; however, whether TOP1 plays a conserved developmental role among multicellular organisms is unknown. Here, we investigated the developmental roles of TOP1 in the moss Physcomitrium (Physcomitrella) patens with gene targeting, microscopy, 3D image segmentation and crossing experiments. We discovered that the disruption of TOP1α, but not its paralogue TOP1β, leads to a defect in fertilisation and subsequent sporophyte formation in P. patens. In the top1α mutant, the egg cell was functional for fertilisation, while sperm cells were fewer and infertile with disordered structures. We observed that the nuclei volume of wild-type sperm cells synchronously decreases during antheridium development, indicating chromatin condensation towards the compact sperm head. By contrast, the top1α mutant exhibited attenuated cell divisions and asynchronous and defective contraction of the nuclei of sperm cells throughout spermatogenesis. These results indicate that TOP1α is involved in cell division and chromatin condensation during spermatogenesis in P. patens. Our results suggest that the regulation of DNA topology by TOP1 plays a key role in spermatogenesis in both land plants and metazoans.Riclinoctaose was produced by enzymatic hydrolysis of a succinoglycan-type exopolysaccharide riclin. It can be used as a prebiotic to regulate the composition of gut microbiota. Therefore, a safety evaluation is needed. Here, we reported the safety data generated on riclinoctaose. Standard in vitro genotoxicity tests such as the bacterial reverse mutation assay and in vivo micronucleus assay were performed and no mutagenic or clastogenic potential was found. In the acute toxicity study, ICR mice were administered with riclinoctaose via gavage in 14-day studies at the level corresponding to 3000 mg/kg BW/day. In the subchronic study, the diets containing 0%, 1.0%, 2.5%, and 5.0% of riclinoctaose (weight/weight) were prepared for ICR mice for 13 weeks. No test item-related adverse effects were observed in the acute and subchronic studies. No riclinoctaose-induced differences in the overall health, body weight gain, food and water consumption, hematology, blood chemistry, gross pathology, histopathology, or animal death were observed. A no-observed-adverse-effect level of 8842 mg/kg BW/day for male and 9230 mg/kg BW/day for female mice was identified for riclinoctaose when administered for 13 weeks. In conclusion, these findings demonstrated the safety of riclinoctaose and indicated the possibility that riclinoctaose may be used as a functional food. The role of the Purkinje network in triggering ventricular fibrillation (VF) has been studied; however, its involvement after onset and in early maintenance of VF is controversial.

We studied the role of the Purkinje-muscle junctions (PMJ) on epicardial-endocardial activation gradients during early VF.

In a healthy, porcine, beating-heart Langendorff model [control, n = 5; ablation, n = 5], simultaneous epicardial-endocardial dominant frequent mapping was used (224 unipolar electrograms) to calculate activation rate gradients during the onset and early phase of VF. Selective Purkinje ablation was performed using Lugol's solution, followed by VF re-induction and mapping and finally, histological evaluation.

Epicardial activation rates were faster than endocardial rates for both onset and early VF. After PMJ ablation, activation rates decreased epicardially and endocardially for both onset and early VF [Epi 9.7 ± 0.2 to 8.3 ± 0.2 Hz (p <.0001) and 10.9 ± 0.4 to 8.8 ± 0.3 Hz (p < .0001), respectively; Endo 8.2 ± 0.3 Hz to 7.4 ± 0.2 Hz (p < .0001) and 7.0 ± 0.4 Hz to 6.6 ± 0.3 Hz (p = .0002), respectively]. In controls, epicardial-endocardial activation rate gradients during onset and early VF were 1.7 ± 0.3 Hz and 4.5 ± 0.4 Hz (p < .001), respectively. After endocardial ablation of PMJs, these gradients were reduced to 0.9 ± 0.3 Hz (onset VF, p < .001) and to 2.2 ± 0.3Hz (early VF, p <.001). Endocardial-epicardial Purkinje fiber arborization and selective Purkinje fiber extinction after only endocardial ablation (not with epicardial ablation) was confirmed on histological analysis.

Prevalence of hypertension and hypertension-mediated organ damage (HMOD) had not been well studied in patients with 11β-hydroxylase deficiency (11β-OHD).

The study was to assess the prevalence and risk factors of hypertension and HMOD in patients with 11β-OHD.

Retrospective cohort analysis in a single medical centre.

Twenty-eight patients with 11β-OHD were recruited between January 2003 and June 2021, and their diagnosis had been confirmed by Sanger sequencing.

Blood pressure and clinical indicators for the assessment of HMOD occurrence were collected from the medical records. Medication adherence of antihypertensive drugs and glucocorticoids were determined by the patients' biochemistry. Logistic regression was used to identify factors associated with HMOD.

Prevalence of hypertension and HMOD in the cohort was 100% and 50%, respectively. The kidneys (71.43%) are the organ most commonly damaged by high blood pressure, followed by the heart (64.29%), eyes (57.14%) and brain (21.43%). Risk factors of HMOD were hypokalemia (odds ratio [OR] 9.
