Valid causal inferences are necessary to use developmental research to improve adolescent outcomes. What type of change should be analyzed to approximate causal inferences from longitudinal data? Difference-score and ANCOVA-type analyses often produce contradictory results, a problem known as Lord's paradox. This study investigates 2-group, 2-wave difference-score analyses and ANCOVA, and introduces a method that produces consistent results, namely dual-centered ANCOVA, which is compared to pretest matching.

These methods are tested first on two datasets simulated to fit each of Lord's contrasting results. The methods are then applied to data investigating the longitudinal associations of parent-adolescent discussions about sexual risks on subsequent unprotected sexual behaviors in 4753 American adolescents (62.2% whites).

The results replicate Lord's contradictory results for all datasets. Dual-centered ANCOVA and pretest matching both produce consistent results, but dual-centered ANCOVA replicates theype analyses, dual-centered ANCOVA estimates the difference-score effect while retaining the advantages of ANCOVA in statistical power and covariate inclusion. Recurrent arrhythmia following catheter ablation of atrial fibrillation (AF) may present early, during a standard 3-month blanking period. Early recurrence has been correlated to late recurrence, but the degree to which its absence predicts longer-term success has not been quantified.

The purpose of this study was to explore and quantify the relationship between early and late arrhythmia recurrence, specifically the negative predictive value, that is, the degree to which absence of blanking period recurrence predicts absence of late recurrence.

A systematic literature review and meta-analysis were conducted using statistical methods of a diagnostic test accuracy review. Studies of AF ablation using point-by-point radiofrequency, with repeated monitoring of arrhythmia recurrence including asymptomatic recurrence, and with separate data by AF type, were eligible.

Nine studies met the prespecified eligibility criteria. For paroxysmal AF, 89% (confidence interval [CI] 82%-94%) of patients free from early recurrence remained free from late recurrence. The estimate for persistent AF was similar (91%; CI 75%-97%). This finding was robust in sensitivity analyses. Patients with early recurrence had a wider range of likely outcomes with longer-term follow-up.

Freedom from AF recurrence during the blanking period is highly predictive of longer-term success in catheter ablation. Clinical trials in this area may be able to leverage these findings to more quickly assess the potential utility of new ablation technologies and methods, for example, by using early surrogate measures of success.

Coronavirus disease-2019 (COVID-19) pandemic had a great impact over all elective neurosurgical activity and important implications in management of neurosurgical urgencies. During the pandemic, some pediatric hospitals reported their experiences. After the emergency phase of the COVID-19 pandemic, the health care system needs to be reorganized to again manage all nonurgent activities, while ensuring safety of both patients and health care workers.

We developed preventive measures to limit any possibility of COVID-19 spread, according to the principles of epidemiologic prevention and suggestions from recent literature. To evaluate the efficacy of these measures, we retrospectively reviewed the neurosurgical activity at our institution from May 4 to July 15,2020.

One hundred nineteen patients were admitted to the neurosurgical ward, and 80 surgical procedures were performed. Furthermore, 130 outpatient clinics were scheduled. A total of 258 nasopharyngeal swabs and 249 specific interviews were performed. We present the posterior approach through which the language tracts and visual tracts at the roof and lateral wall of the temporal horn (Meyer loop) can be avoided, minimizing the risk of neurologic injury. The patient, a 32-year-old man, presented with the chief complaint of experiencing seizures for 6 years with rare, generalized, tonic-clonic seizures. Physical examination showed no neurologic deficits and past medical history was not remarkable. Magnetic resonance imaging revealed a left mesiotemporal lesion, which showed no contrast enhancement and infiltrated the atrium. For surgery, the patient was laid in prone position and a tailored bone flap was lifted. Next the occipital lobe was retracted gently to expose the lesion. Penfield dissectors were used to gradually resect the lesion. The roof and inner wall of the atrium were exposed during resection. These structures were protected. Residues of the lesion and its capsule were seen attached to the lateral ventricle and were carefully coagulated and removed. The hippocampus was also exposed and a small segment resected. Exposure of the roof and inner wall of the atrium confirmed that the entire lesion has been resected. Intraoperative magnetic resonance imaging evaluation confirmed a total resection of the lesion. Pathological analysis confirmed the diagnosis of epidermoid cyst. Cognitive evaluation results showed no postoperative deficiencies and his visual field was also not affected by the surgery. Informed patient content was obtained (Video 1).

Frailty is associated with postoperative morbidity in multiple surgical disciplines. We evaluated the association between frailty and early postoperative outcomes for brain tumor patients using a national database.

We reviewed the Nationwide Readmissions Database from 2010 to 2014. International Classification of Diseases, ninth revision, codes were used to identify benign and malignant brain tumors treated with surgical resection. Pituitary tumors were excluded. Frailty was assessed using the Johns Hopkins Adjusted Clinical Groups frailty indicator tool. Multivariable exact logistic regression was used to conduct analyses assessing the association between frailty and the outcome variables. Statistical significance was defined as P < 0.001.

The criteria for frailty were met for 7209 of 87,835 patients (8.2%). After adjustment for patient and hospital factors, frailty was independently associated with in-hospital surgical complications (odds ratio [OR], 1.48; 95% confidence interval [CI] 1.37-1.59; P on or in-hospital strategies to improve the care and outcomes of these at-risk patients.

In Parkinson's disease (PD), the high burden of motor and non-motor symptoms, such as cognitive impairment or falls, is associated with rapid disease progression and mortality. This is often reflected by an increased drop-out rate of PD patients in longitudinal studies. Active physical behavior can impact the disease course beneficially and has an overall positive effect on health. Contrarily, sedentary behavior is associated with cognitive impairment in PD. The aim of this study was to investigate whether sedentary physical behavior assessed in the home environment and cognitive impairment can predict health-related study attrition due to sickness and death in PD.
