Septic patients often show an acute liver dysfunction during the onset of sepsis in ICU. We found the levels of ALT, AST, TBIL increased significantly in septic patients and returned after recovery from sepsis in our ICU (P less then 0.05), and had a similar trend for HMGB1. To explore the role of hepatic macrophage in acute liver injury, we simulated the process of acute liver injury by cecal ligation and puncture (CLP) in mice. We assessed the inflammatory infiltration of the liver by HE, and examined the levels of ALT and AST in serum and the expression of HMGB1, IL-1β in the serum and the relative expression of mRNA in the liver at the different time of CLP model. Also we found the rate of pyroptosis cells in liver was about 18.19%, while 16.29% in macrophages by Flow cytometry. So our study has demonstrated that HMGB1 may promote the pyroptosis of liver macrophages to mediate acute liver injury in sepsis.The aims of this study are to determine characteristics and factors influencing REE in adult severe burn patients. A prospective study was conducted on 62 adult burn patients admitted during 72 h after burn to burn intensive care unit, National Burn Hospital, Vietnam. REE measurements and REE/BMR calculations were obtained on the 3rd, 7th, 14th, 21st and 28th day after burn. Collected data was analyzed to find out the influence of age, gender, burn extent, inhalation injury to REE. The results indicated that all measured REE was significantly higher than BMR at all time points (REE/BMR ratio > 1) with a peak value on the 7th day then steady decreased but still around 200% in compared with BMR on the 28th day after burn. In compared with females, REE of male patients were significantly higher during the first three weeks after burn. In addition, significantly greater REE were seen in the patients with burn surface area ≥ 60% TBSA or deep burn area ≥ 20% TBSA. Moreover, REE of nonsurvivors was significantly higher in compared with survivor group on the 7th and 14th day after burning. Meanwhile, increased age and presence of inhalation injury did not affect REE. In conclusion, in adult burn patients, increased REE is prolonged, burn size dependent and significantly higher in male and in nonsurvivor. This finding should be considered in nutritional caring for adult burn patients.

To explore the effects of sleep deprivation on perioperative general anesthesia in rats.

45 healthy male Sprague-Dawley (SD) rats were randomly divided into 3 groups, the control group (Group A), the anesthesia group (Group B) and the sleep deprivation anesthesia group (Group C), 15 in each group. The sleep deprivation model was established by improving multi-platform water environment method. The initial importance of mentoring competencies based on early career faculty members' perception and experience had not yet been determined. The Mentoring Competency Assessment validated by Fleming et al. (2013b) was used to rate the perceived importance of competencies and assess senior faculty members' competencies using a seven-point range, Likert-type response scale. Responses were analyzed by presence or absence of a mentor, previous formal mentor training, sex, and health science discipline. Junior faculty (n = 144) rated each competency as important or greater across all categories. A majority (70%) reported not having a current mentor. Junior faculty with current mentors rated senior faculty competencies higher than junior faculty without; participation in formal mentor training as well as a clinical faculty appointment were independently associated with higher assessment scores. This study identifies specific population characteristics that may serve to enhance the effectiveness of OCME workshops and demonstrates that junior faculty identify mentoring as significantly important in their academic career success in a research and clinical health setting.We present two male subjects (6 and 14 years old) with mild dysmorphism, intellectual disability, and/or autism spectrum disorder with chromosome 22q11.2 microduplications of different sizes. We then compared the clinical and genetic findings with similar cases from the literature sharing the same 22q11.2 duplications. These rare duplications in our subjects were identified by high-resolution chromosomal microarray analysis and flanked by low copy repeats in the 22q11.2 region, specifically LCR22A, LCR22B, and LCR22D. The typical 22q11.2 defect generally involves a deletion at breakpoints LCR22A and LCR22D causing DiGeorge or velo-cardio-facial syndrome and not duplications of varying sizes as seen in our male subjects.
