Chronic kidney disease (CKD) is characterized by persistent low-grade inflammation. Soluble CD14 (sCD14) is involved in many pathological conditions including inflammation and atherosclerosis. The present study aimed to assess the relation between sCD14 levels, subclinical atherosclerosis (SCA), inflammation and mortality in Egyptian hemodialysis (HD) patients.

The present longitudinal study included 62 HD patients. All patients were submitted to careful history taking, thorough clinical examination and laboratory assessment for high-sensitivity C-reactive protein (hsCRP) and sCD14. Carotid intima-media thickness (CIMT) was also assessed. Patients were followed for a maximum of 18 months. The primary outcome is patients' mortality. Data were statistically analyzed using standard descriptive, comparative, correlative and regression methods.

The present study was conducted on 62 HD patients. They comprised 34 males and 28 females with an age of 54.6 ± 9.0 years. At the end of follow up, 12 patients (19.4 %) died. It was shown that survivors had significantly lower hsCRP levels (104.2 ± 38.2 versus 134.1 ± 15.3 mg/dL, p <0.001), lower sCD14 levels (32.7 ± 10.3 versus 47.4 ± 18.4 µg/mL, p=0.02) and lower CIMT (1.32 ± 0.5 versus 1.5 ± 0.2 mm, p=0.049). sCD14 levels were significantly correlated with hsCRP (r=0.4, p=0.001) and CIMT (r=0.31, p=0.013). Multivariate analysis identified HD duration [HR (95% CI) 1.02 (1.0-1.04), p= 0.021] and sCD14 levels [HR (95% CI) 1.06 (1.0-1.12), p= 0.026] as significant predictors of patients' survival.

Allergic rhinoconjunctivitis and asthma are the most common IgE-mediated diseases worldwide. Allergen-specific immunotherapy (AIT) is currently the only modifying treatment for these IgE-mediated diseases, in both children and adults. Subcutaneous immunotherapy is widely used, but in patients over 65 years old there may be an increased risk of adverse reactions and a worse response to treatment. Oral immunotherapy (OIT) has been proved to be effective and safe, but currently, in most countries, it has been licensed only for patients up to 65 years old based on its technical datasheet. Air pollution is one of the environmental factors that influences the pathogenesis of systemic autoimmune diseases, followed by the development and spread of inflammation and increased oxidative damage. Only a few studies have been conducted on the impact of air pollution on disease activity in patients with lupus, which mostly have focused on PM2.5 particles.

We longitudinally studied 50 patients with lupus bimonthly in a 6-month period in Mashhad; one of the polluted cities of Iran. Disease activity and quality of life were examined according to SLEDAI2K, SLEQOL, and VAS criteria. The outdoor air pollutant was measured by monitoring the average concentration of nitrogen dioxide (NO2), carbon monoxide (CO), some particles less than 10 and 2.5 micrometers in diameter (PM &lt;10, PM &lt;2.5) and the level of temperature and humidity which were taken from the Meteorological Organization of Mashhad. Confounding factors such as medications were investigated by univariate and multivariate statistical analysis, specifically by GEE method.

The possible relation among various factors to SLEDAI, SLEQOL and VAS by two different univariate and multivariate analysis were studied. Our analysis indicated that spring season, decreased temperature, increased air pollutants including (PM2.5, and NO2) and increased humidity increase SLEDAI2K. Furthermore, the percent of polluted days directly correlates with Anti-dsDNA and NO2 significantly increases SLEQOL.

Based on our findings, air pollution (particularly NO2 and PM2.5) has affected at least some aspects of the disease and the health-related quality of life (HRQL) of lupus patients. Further research is needed to confirm these findings.
