The most striking finding at autopsy was of impacted, moist pale yellow perlite that extended from the epiglottis into the main bronchi resulting in complete obstruction of the larger and smaller airways with cast formation. Perlite inhalation differs from inhalation of inert materials such as sand and wheat due to its hygroscopic properties and ability to expand forming an occlusive cast. In the present study, we developed homology-based radiomic features to predict the prognosis of non-small-cell lung cancer (NSCLC) patients and then evaluated the accuracy of this prediction method. METHODS Four datasets were used two to provide training and test data and two for the selection of robust radiomic features. All the datasets were downloaded from The Cancer Imaging Archive (TCIA). In two-dimensional cases, the Betti numbers consist of two values b0 (zero-dimensional Betti number), which is the number of isolated components, and b1 (one-dimensional Betti number), which is the number of one-dimensional or "circular" holes. For homology-based evaluation, computed tomography (CT) images must be converted to binarized images in which each pixel has two possible values 0 ohat there are some limitations. © 2020 American Association of Physicists in Medicine.BACKGROUND Radiculopathies caused by spinal stenosis in the upper lumbar spinal canal (L1/2, L2/3, L3/4) have not been comprehensively investigated. METHODS This retrospective study reviewed 14 patients from a tertiary hospital outpatient clinic. The inclusion criteria were upper lumbar stenosis seen on MRI and radiculopathies with active denervation confirmed on electromyography. Patients with any other conditions that could explain the clinical or electrophysiological manifestations were excluded. RESULTS Neurogenic findings were predominantly observed in L5 or S1 myotomes on electromyography. Abnormal spontaneous activity was observed in distal muscles in all patients and in proximal muscles in eight patients. Axonal involvement was bilateral in 10 patients and unilaterally in 4 patients. MRI showed redundant nerve roots in 13 patients with chronic reinnervation on electromyography. CONCLUSIONS Upper lumbar spinal stenosis usually causes L5 or S1 radiculopathies with diverse patterns. This discrepancy may cause diagnostic confusion. © 2020 Wiley Periodicals, Inc.OBJECTIVES/HYPOTHESIS The association between smoking and olfactory loss remains a conundrum. Prior studies have found negative and positive effects of smoking on olfactory function in the general population. However, smoking cessation seems to improve both rated and measured olfactory function. The purpose of this study was to investigate the olfactory function and smoking habits in patients with olfactory loss caused by different etiologies to unveil underlying patterns related to smoking. STUDY DESIGN Retrospective observational study. METHODS Patients were included from two specialized taste and smell centers. Patients underwent olfactory testing, clinical examination including rhinoscopy, and the underlying etiology was identified. Patterns of olfactory test scores, demographics, and etiologies were analyzed. RESULTS In total, 3,900 patients with olfactory loss were included. Of these, 521 were current smokers, and 316 were former smokers. Patients with a history of smoking did not have significantly lower olfactory function. Current smokers were more often affected by posttraumatic olfactory loss, but not sinonasal, postviral, or idiopathic olfactory loss. CONCLUSIONS Current smoking, but not former smoking, was associated with posttraumatic olfactory loss. In relation to measured olfactory function, a history of smoking was not associated to lower olfactory scores. Our findings suggest that the general recommendations of smoking cessation for patients with olfactory loss are especially relevant for patients with posttraumatic olfactory loss. The nature of this association between current smoking and posttraumatic olfactory loss has yet to be elucidated. Clinical or molecular data to identify primary resistant cases at diagnosis are not yet available. HGS-EOC biopsies from 105 Pt-sensitive (Pt-s) and 89 Pt-resistant (Pt-r) patients were retrospectively selected from two independent tumor tissue collections. Pathway analysis was done integrating miRNA and mRNA expression profiles. Signatures were further validated in silico on a cohort of 838 HGS-EOC cases from a published dataset. In all, 131 mRNAs and 5 miRNAs belonging to different functionally related molecular pathways distinguish Pt-s from Pt-r cases. Then, 17 out of 23 selected elements were validated by orthogonal approaches (SI signature). link3 As resistance to Pt is associated with a short progression-free survival (PFS) and overall survival (OS), the prognostic role of the SI signature was assessed, and 14 genes associated with PFS and OS, in multivariate analyses (SII signature). The prognostic value of the SII signature was validated in a third extensive cohort. The expression profiles of SDF2L1, PPP1R12A and PRKG1 genes (SIII signature) served as independent prognostic biomarkers of Pt-response and survival. The study identified a prognostic molecular signature based on the combined expression profile of three genes which had never been associated with the clinical outcome of HGS-EOC. This may lead to early identification, at the time of diagnosis, of patients who would not greatly benefit from standard chemotherapy and are thus eligible for novel investigational approaches. © 2020 UICC.Stable genetic transformation of plants is a low-efficiency process, and identification of positive transformants usually relies on screening for expression of a co-transformed marker gene. Often this involves germinating seeds on solid media containing a selection reagent. Germination on solid media requires surface sterilization of seeds and careful aseptic technique to prevent microbial contamination, but surface sterilization techniques are time consuming and can cause seed mortality if not performed carefully. We developed an antimicrobial cocktail that can be added to solid media to inhibit bacterial and fungal growth without impairing germination, allowing us to bypass the surface sterilization step. Adding a combination of terbinafine (1 μM) and timentin (200 mg l-1 ) to Murashige and Skoog agar delayed the onset of observable microbial growth and did not affect germination of non-sterile seeds from 10 different wild-type and mutant Arabidopsis thaliana accessions. We named this antimicrobial solid medium "MSTT agar". Seedlings sown in non-sterile conditions could be maintained on MSTT agar for up to a week without observable contamination. This medium was compatible with rapid screening methods for hygromycin B, phosphinothricin (BASTA) and nourseothricin resistance genes, meaning that positive transformants can be identified from non-sterile seeds in as little as 4 days after stratification, and transferred to soil before the onset of visible microbial contamination. By using MSTT agar we were able to select genetic transformants on solid media without seed surface sterilization, eliminating a tedious and time-consuming step. © 2020 The Authors. This study investigated whether VTA DAergic afferents modulate glutamatergic transmission-dependent GABAergic neurons in dorsolateral bed nucleus of stria terminalis (dlBNST) to affect the activity of the HPA-axis. Herein, we demonstrate that systemic administration of the neurotoxicant 1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine (MPTP) or the VTA-injection of 1-methyl-4-phenylpyridinium ion (MPP+) in male mice (MPTP-mice and MPP+mice) caused a decline of tyrosine hydroxylase positive (TH+) cells in VTA with a reduction in TH+fibers in the dlBNST. MPTP-mice and MPP+mice displayed a clear increase in serum levels of corticosterone (CORT) and adrenocorticotropic hormone, corticotropin-releasing hormone (CRH) expression, and CRH neuron activity in PVN. The presynaptic glutamate release, glutamatergic synaptic transmission and induction of long-term potentiation in dlBNST of MPTP-mice were suppressed, and these effects were rescued by a D1-like DAergic receptor (D1R) agonist and mimicked in control dlBNST by blockade of D1R. MPTP-mice exhibited low expression of glutamic acid decarboxylase and dysfunction of the excitatory-dependent GABAergic circuit in dlBNST, and these effects were recovered by the administration of D1R agonist. Furthermore, either dlBNST-injection of D1R agonist or PVN-injection of GABAA receptor (GABAA R) agonist could correct the increased secretion and expression of CRH in MPTP-mice. The results indicate that the DAergic afferents from VTA enhance excitatory-dependent activation of GABAergic neurons in dlBNST, which suppress the activity of the HPA-axis. © 2020 International Society for Neurochemistry.Women presenting with amenorrhea and elevated serum levels of FSH before the age of 40 may suffer from premature ovarian insufficiency (POI). POI can be caused by ovarian damage through surgery, irradiation and chemotherapy. Autoimmune disease and genetic causes are also possible causes for development of POI. Since today we have a wide diagnostic arsenal available for patients, all women should be offered extended investigation. It is also important that affected women receive sufficient estrogen supplementation to avoid complications of POI, such as cardiovascular disease, osteoporosis and cognitive impairment. Supplementation should be given at least until the normal age of menopause, about 51 years of age. Although the condition is often complicated by infertility, it is important to inform patients that some residual fertility potential may exist in the case of secondary amenorrhea and POI.
