Individuals with coronary artery disease (CAD) have worse executive function compared to the general population but the mechanisms are unknown.
To investigate the role of acute mental stress (MS) on the executive function of patients with CAD.
Participants with stable CAD underwent acute MS testing with simultaneous peripheral vascular function measurements and brain imaging using high resolution-positron emission tomography. Digital pulse wave amplitude was continuously measured using peripheral artery tonometry (PAT, Itamar Inc). Stress/rest PAT ratio (sPAT) of pulse wave amplitude during MS/baseline was calculated as a measure of microvascular constriction during MS. Plasma levels of catecholamine and interleukin-6 were assessed at baseline and after MS. Executive function was assessed both at baseline and at 2 years follow-up using the Trail Making Test parts A and B.
We studied 389 individuals with brain data available for 148 participants. Of this population follow-up cognitive assessments were performed in 226 individuals (121 with brain imaging). After multivariable adjustment for baseline demographics, risk factors, and medication use, a lower sPAT, indicating greater vasoconstriction, a higher inferior frontal lobe activation with MS, and increases in norepinephrine and IL-6 levels with MS were all independently associated with greater time to complete Trail B test.-38.4pt.

The z scores measured from these hexagonal bins mimic the risk realness or risk reality phenomenon happening in Albay Province, Philippines. The objective of the study is to assess risk reality phenomena that generate risk knowledge originated from applying the seven metatheorems based on the Schoen Golden Triangle and the Fibonacci Golden Ratio. Risk assessment in this study uses the stability site selection criteria and hexagonal binning technique to store, sort, and process risk hotspot and coldspot information. This approach led to the disclosure of risk phenomenon on the 14 out of 25 resettlement sites (host environment) that remained at risk and continuously increasing the risk trend. When people are continuously allowed to occupy risk hotspots areas it hints at ineffective risk governance to neutralize the passively exposed population. This study concluded that the risk reality phenomena assessment opens new avenues for scientifically informed land use, nil exposure, and 0-risk policy in addition to the existing 0-casualty goal to get prepared with the right direction, decision and action to sensitively utilize the stable host environments aligned to improve risk governance.Behavioral data, despite being a common index of cognitive activity, is under scrutiny for having poor reliability as a result of noise or lacking replications of reliable effects. Here, we argue that cognitive modeling can be used to enhance the test-retest reliability of the behavioral measures by recovering individual-level parameters from behavioral data. We tested this empirically with the Probabilistic Stimulus Selection (PSS) task, which is used to measure a participant's sensitivity to positive or negative reinforcement. An analysis of 400,000 simulations from an Adaptive Control of Thought-Rational (ACT-R) model of this task showed that the poor reliability of the task is due to the instability of the end-estimates because of the way the task works, the same participants might sometimes end up having apparently opposite scores. To recover the underlying interpretable parameters and enhance reliability, we used a Bayesian Maximum A Posteriori (MAP) procedure. We were able to obtain reliable parameters across sessions (intraclass correlation coefficient ≈ 0.5). A follow-up study on a modified version of the task also found the same pattern of results, with very poor test-retest reliability in behavior but moderate reliability in recovered parameters (intraclass correlation coefficient ≈ 0.4). Collectively, these results imply that this approach can further be used to provide superior measures in terms of reliability, and bring greater insights into individual differences.Diagnostic tests are usually based on some quantitative biomarkers. Predictive values of the disease status based on test results are also of interest in medical research and public health management. In this paper, we study the relations among sensitivity, specificity and predictive values of the test. The core concept is risk function, which is assumed to be an increasing function of the biomarker. Our results show that test sensitivity and specificity change in opposite directions. The positive predictive value and the sensitivity also change in opposite directions. Likewise, the negative predictive value and the specificity change in opposite directions.Maladaptive daydreaming (MD) is an immersive fantasy activity that leads to distress and socio-occupational dysfunction. MD has many features suggestive of behavioural addiction, but research exploring the phenomenology of this clinical construct is limited. MD has also been purported to be a maladaptive strategy to cope with distress, but often leads to uncontrollable absorption in fantasy world, social withdrawal and neglected aspects of everyday life. In this paper, we report the case of a 16-year-old boy who developed internet gaming disorder and MD after experiencing cyberbullying from his peers. The patient engaged in vivid, fanciful imagery that lasted for hours, leading to social dysfunction and academic deterioration. Baseline assessment revealed a Maladaptive Daydreaming Scale-16 score of 65 (cut-off 50) and an Internet Gaming Test-20 score of 86 (cut-off 71). The patient was managed primarily with psychological intervention. The therapy focused on behaviour modification and cognitive restructuring. Patients in clinical setting should be questioned about MD when presenting with excessive use of technology or other forms of behavioural addictions. Further studies are necessary to determine the biopsychosocial factors of MD and its correlates. There is also a need to study treatment and management approaches for treating MD.
There is an urgent need in clinical practice to measure the stress of parenting. The Caregiver Strain Questionnaire (CGSQ) was found to be useful to measure parenting stress, but it has not been validated among the Chinese population.
To assess the reliability and construct validity of the Chinese version of CGSQ among Chinese parents.
From 2016 to 2017, 266 parents (patient group) with a child having DSM-5-defined attention deficit hyperactivity disorder (ADHD) (n=107) or autism spectrum disorder (ASD) (n=159) and 268 parents of healthy children (control group) were recruited to the present study in Kunming, Yunnan province. All the parents were asked to fill out the Chinese version of CGSQ. We conducted exploratory factor analysis and confirmatory factor analysis (CFA) to verify construct validity of CGSQ in both patient and control groups. Cronbach's α coefficient as an index of internal consistency was assessed for each subscale. Fourteen days later, 23 subjects filled out the scale again. Intra-clar ASD.
These findings provide initial evidence to support the construct validity and reliability of CGSQ as a parenting stress measurement tool for Chinese parents, especially for parents of children with ADHD or ASD.
HIV-positive children are at high risk for oral mucosal disorders. Additionally, their low immune status is associated with dental caries. However, little is known about how their dental caries and related risk factors, such as salivary flow, salivary pH level and oral health-related quality of life, differ from those of HIV-negative children. The study aimed to assess (1) dental caries and related risk factors in HIV-positive compared with HIV-negative children and (2) the association between these factors and HIV seropositive status in Phnom Penh, Cambodia.
This was a cross-sectional study conducted as a baseline survey of a randomised controlled trial. The study setting was the National Pediatric Hospital's catchment area. SAR131675 The study population comprised 328 HIV-positive and 154 HIV-negative children aged 3-15 years and their caregivers. We collected clinical oral health data, questionnaire data to assess oral health-related quality of life and growth data.
The mean number of decayed, missing or filled permanent teeth (DMFT) and deciduous teeth (dmft) among HIV-positive children was 4.0 (SD 3.6) and 7.0 (SD 4.9), respectively. Among HIV-negative children, the respective values were 3.3 (SD 3.7) and 7.1 (SD 4.6). Living with HIV was positively associated with DMFT (adjusted OR 1.85, 95% CI 1.14 to 3.01) and salivary flow (β=0.72, 95% CI 0.44 to 1.00) and negatively associated with salivary pH (β=-0.13, 95% CI -0.24 to -0.02). However, HIV-positive status was not significantly associated with dmft or oral health-related quality of life.
HIV-positive children had poorer oral health status regarding DMFT and salivary pH level. Specific strategies and further efforts are required to align their oral health status with that of HIV-negative children.

These consequences are felt by all those involved in bullying (bullies, victims and bully-victims) and are now recognised to propagate deep into adulthood. Cyberbullying is a relatively new type of bullying in addition to the traditional forms of direct physical, direct verbal and indirect bullying. Children who are perceived as being 'different' in any way are at greater risk of victimisation, with physical appearance being the most frequent trigger of childhood bullying. Globally, one in three children have been bullied in the past 30 days, although there is substantial regional variation in the prevalence and type of bullying experienced. The consequences of childhood bullying can be categorised into three broad categories educational consequences during childhood, health consequences during childhood and all consequences during adulthood. Many dose-response relationships exist between the frequency and intensity of bullying experienced and the severity of negative health consequence reported.
