BACKGROUND Acute dyspnoea is common among ambulance patients, but little is known of the patients' experience of symptom. To this end, the dyspnoea scale appears feasible in the prehospital setting.BACKGROUND Triatomine bugs are responsible for the vectorial transmission of the parasite Trypanosoma cruzi, etiological agent of Chagas disease, a zoonosis affecting 10 million people and with 25 million at risk of infection. Several triatomine species of the genus Rhodnius have been found inhabiting palm crowns where insects can find shelter in leaves axils and blood from palm-associated vertebrates. Rhodnius prolixus insects have been collected in oil palms in Colombia, and high T. cruzi infection rates were found. Since pest control is carried out in oil palm plantations, continuous exposure to insecticides could be occurring in these triatomines. Some insecticides suggested for pest control in oil palm plantations are also recommended for triatomine control in human dwellings. In this study, our objective was to assess if triatomines inhabiting oil palms exhibit resistance to deltamethrin, an insecticide used for vector control. METHODS Rhodnius prolixus nymphs were sampled in oil palms located in Taurame from oil palms plantations. In palm oil producer countries such as Colombia, the oil palm plantations are growing constantly during the last years. We suggest that pest control strategies in oil palm crops should include triatomine surveillance and toxicological monitoring, especially in zones with several Chagas disease cases.BACKGROUND Interstitial lung diseases in children (chILD) are rare and consist of many different entities that affect the parenchyma of the lungs, leading to a chronic lung disease. The natural course of many of these diseases is connected with a high morbidity and significant mortality. Symptomatic treatment consists of oxygen supplementation, adequate nutrition adapted to the high energy demand generated by the disease due to the increased breathing effort required, as well as immunization against respiratory pathogens to prevent exacerbations through respiratory infections. No proven pharmacological treatments are available to date. This placebo-controlled study aims to evaluate the efficacy and safety of the mid-term use of hydroxychloroquine in chILD. METHODS AND DESIGN The study is an explorative, prospective, randomized, double-blind, placebo-controlled investigation of hydroxychloroquine (HCQ) in chILD. Patients can be included into the trial when diagnosed with a chronic (≥ 3 weeks' duration) diffusepropriate numbers of patients of all age groups from neonates to adults suffering from these rare diseases. Matthias Griese, University Hospital, Ludwig-Maximilians University of Munich, Germany.BACKGROUND Mesenchymal stem/stromal cells (MSCs) can replenish the aged cells of the musculoskeletal system in adult life. Stem cell exhaustion and decrease in their regenerative potential have been suggested to be hallmarks of aging. Here, we investigated whether muscle- and bone-derived MSCs of patients with osteoarthritis and osteoporosis are affected by this exhaustion, compared to healthy donors. METHODS Patients with primary osteoarthritis, femoral neck fractures due to osteoporosis, and healthy donors (controls) were included. MSCs were isolated from the skeletal muscle and subchondral bone from each patient and compared using ex vivo and in vitro analyses, including immunophenotyping, colony-forming unit fibroblast assays, growth kinetics, cell senescence, multilineage potential, and MSC marker gene expression profiling. RESULTS Freshly isolated cells from muscle from patients with osteoarthritis showed a lower proportion of CD45/CD19/CD14/CD34-negative cells compared to patients with osteoporosis andoperties of both skeletal muscle- and bone-derived MSCs in osteoarthritis and osteoporosis patients, compared to healthy donors. In bone-derived MSCs from patients with osteoarthritis, we also identified a lower expression of the leptin receptor, a marker of MSCs that present a major source of MSCs in the adult bone marrow. This suggests that exhaustion of skeletal muscle- and bone-derived MSCs is a hallmark of osteoarthritis and osteoporosis, which defines the need for further clinical trials of stem cell transplantation in these patients.
