Microplastics (MPs) are ubiquitous in environmental media and human diets and can enrich organic contaminants, including polycyclic aromatic hydrocarbons (PAHs) and their derivatives. The bioaccessibilities and triggering cancer risks of MP-sorbed PAHs and PAH derivatives are closely linked with human health, which, however, were rarely focused on. This study explored the sorption behaviors of phenanthrene (PHE) and PHE derivatives on polyethylene (PE), polypropylene (PP), and polystyrene (PS) MPs, and assessed their bioaccessibilities in gastrointestinal fluids as well as their inducing human cancer risks. PE MPs harbored the highest sorption capacity, secondly the PP MPs, then the PS ones. Sorption of PHE and PHE derivatives on MPs was positively correlated with their hydrophobicities. The bioaccessibilities of sorbed PHE and PHE derivatives could reach 53.59 %±0.46 %-90.28 %±0.92 % in gastrointestinal fluids and 81.34 %±0.77 %-98.72 %±1.44 % in gastrointestinal fluids with the addition of Tenax (more close to the bioavailability). The hydrophobicities also controlled the bioaccessibilities of PHE and PHE derivatives in gastric fluids, and those in intestinal fluids with Tenax for PS MPs. The incremental lifetime cancer risk (ILCR) values for PHE, PHE-Cl, and PHE-NO2 on MPs at tested concentrations were all higher than the USEPA-suggested safety limit (10-6), and most of them were even higher than 10-4, which thus indicates serious cancer risks. This study promoted our understanding of the potential health threats posed by organic pollutant-bearing MPs in the environment.Neurodevelopmental delay is associated with neurodevelopmental disorders. Prenatal metal exposure can potentially cause neurodevelopmental delays in children. This study examines whether prenatal exposure to mercury (Hg) and selenium (Se) is associated with the risk of neurodevelopmental delays in children up to 4 years of age. Children enrolled in a prospective birth cohort of the Japan Environment and Children's Study were examined. Hg and Se levels in maternal (nchild = 48,731) and cord (nchild = 3,083) blood were analyzed by inductively coupled plasma-mass spectrometry. Neurodevelopmental delays were assessed in children between the ages of 0.5 to 4 years using the Ages and Stages Questionnaires, Third Edition. The association between exposure and outcomes was examined using the generalized estimation equation models. In maternal blood, compared to participants with Se levels in the first quartile (83.0 to less then 156 ng/g), the odds ratio (95 % confidence intervals) for problem-solving ability in children of mothers in the third (168 to less then 181 ng/g) and fourth quartiles (181 to 976 ng/g) were 1.08 (1.01 to 1.14) and 1.10 (1.04 to 1.17), respectively. Furthermore, communication, gross and fine motor skills, and problem-solving delays were also observed. However, prenatal Hg levels in maternal and cord blood and Se levels in the latter were not associated with neurodevelopmental delays in children. Thus, the findings of this study suggest an association between Se levels in maternal blood and slightly increased risks of neurodevelopmental delays in children up to the age of 4 years.

Stroke is an independent risk factor for venous thromboembolism (VTE). Although the risk of VTE persists after hospital discharge, information on the utilization of anticoagulants among stroke patients after discharge remains limited.

To evaluate changes in post-discharge thromboprophylaxis among stroke patients between 2006 and 2019.

We conducted a retrospective repeated cross-sectional analysis using a commercial healthcare insurance database in the United States. We included patients aged ≥ 18 years with incident stroke diagnosis and assessed prophylactic use of anticoagulants in the 30 days following hospital discharge including low-molecular-weight heparin (enoxaparin ≤40 mg/day, dalteparin ≤5000IU/day), unfractionated heparin ≤5000IU/ twice daily or 3 times a day, apixaban 2.5mg twice daily, and rivaroxaban 10mg/day. Patients with atrial fibrillation, VTE, mechanical heart valves, cancer, antiphospholipid antibody syndrome, and users of therapeutic doses of anticoagulants were excluded. We used theparin or rivaroxaban was relatively low but the use of apixaban increased over the study period. Further research is needed to determine if these agents are safe and effective for VTE prevention in stroke patients.

Stroke is the leading cause of death in patients with Sickle cell disease (SCD). Here, we detail the burden of Moyamoya syndrome (MMS) as a cause of stroke in patients with SCD.

A review of SCD-related hospital discharges was conducted utilizing the National Inpatient Sample. Rates of stroke hospitalization, risk factors, procedures, and outcomes were compared between patients with SCD-MMS and SCD alone. Univariate analyses including T-test, Wilcoxon Rank-Sum test, Chi-square were performed to compare risk factors and outcomes. Multivariable regression was used to identify predictors of stroke unique to each population.

Stroke occurred in 9.8% of SCD-MMS hospitalizations versus 0.5% of those involving patients with SCD alone (OR=20.71, p < 0.001). Patients with SCD-MMS developed stroke at younger ages and with fewer comorbidities compared to those with SCD alone. Stroke hospitalizations in SCD-MMS involved a greater number of procedures (90.5% vs. 79.3%, p=0.007), but were more likely to result in favorable discharge (58.5% vs. Darolutamide research buy 44.2%, p=0.005). The presence of anemia during hospitalization was a significant risk factor for stroke in both cohorts. Long-term antiplatelet use was protective against stroke (OR=0.42, p=0.008) only in the SCD-MMS cohort.

MMS confers a 20-fold increased risk of stroke among patients with SCD and appears to be an important cause of recurrent stroke in this population. Anemia is one of the most significant risk factors for stroke, while antiplatelet use appears to confer a protective benefit.

The mechanisms of SGLT2i-mediated cardiorenal protection include blood pressure lowering and endothelial function improvements, enhancement of cardiac and renal hemodynamics, optimization of energetic efficiency through metabolic changes and cellular ion exchanges, reduction of inflammation and oxidative stress with consequent fibrosis reduction, and sympathetic activity modulation. This review explores the most recent data regarding the physiological mechanisms of SGLT2i cardiac and renal benefits, which lie at the root of the solid clinical evidence on cardiorenal protection, making SGLT2i a promising new pharmacological approach to the treatment of patients at high risk of cardiorenal syndrome.Novel pharmaceutical therapies such as pegvaliase, phenylalanine ammonia lyase (PAL), have enhanced disease control for many individuals with phenylketonuria (PKU). We present a retrospective chart review to assess pegvaliase doses over time in individuals followed at the Boston Children's Hospital PAL Clinic, including those who started pegvaliase in a clinical trial ("trial patients") and those who started after drug came to market ("post-market patients"). Trial patients were on pegvaliase an average of 4.8 years longer, and their mean current pegvaliase dose was 126 ± 92 compared to 223 ± 147 mg/week for post-market patients (p = 0.0155), suggesting that the pegvaliase dose for target efficacy may decrease over time in adults with PKU. In post-market patients, we demonstrated a significant, inverse correlation with dose change and number of weeks from response (r = -0.46, p = 0.046). The entire cohort showed significant variability in terms of time to achieve a therapeutic response, response dose, and current dose. Our data suggest that patients tolerate a reduction in pegvaliase dose over time while maintaining efficacy. This is a clinically meaningful finding as it indicates that patients may reduce number of weekly injections over time on pegvaliase.Combined vacuum impregnation and ultrasound was proposed as an alternative method to improve the infusion of ascorbic acid in berry fruit. The effect of ultrasound application at different stages of impregnation - vacuum, relaxation, and both stages - on the qualitative characteristics of impregnated cranberries was investigated. The quality assessment was based on porosity, color, antioxidant capacity, anthocyanin, polyphenol and structure compound content. Ultrasound-assisted vacuum impregnation contributed to higher ascorbic acid content, smaller relative color difference, and greater antioxidant properties. It was found that the degree of impregnation varies considerably and depends on the stage of using ultrasound. Due to more favorable quality attributes, the conclusion was reached that ultrasound should be applied during the relaxation stage of vacuum impregnation. This study aimed to investigate the impact of multi-mode S-type ultrasound treatment on the protein extraction level of walnut dregs. The structural properties of the walnut protein (WP) were characterized, and the correlation between protein structure and extraction level was analyzed. The in situ real-time monitoring model for the ultrasound-assisted WP extraction process was established by a miniature fiber near-infrared (NIR) spectrometer. Results showed that the protein yield, purity, and comprehensive extraction index (CEI) of extracted WP were 71.07 %, 72.69 %, and 71.72, respectively, under optimal conditions (dual-frequency 20/28 kHz, ultrasonic treatment duration 30 min, and ultrasound power density 120 W/L). The secondary structure of extracted WP displayed that the proportion of α-helix and β-sheet reduced, while the contents of β-turn and random coil increased after ultrasonic treatment. Besides, sonication decreased the disulfide bond content and increased free sulfhydryl (-SH) and surface hydrophobicity compared to the control. The microstructures of WP confirmed that appropriate sonication could unfold the protein aggregates and reduce the particle size. The extraction level of WP is positively correlated with the -SH content (p less then 0.01). The quantitative prediction model of Si-PLS for -SH content in the ultrasound-assisted WP extraction process was established and performed a good correction and prediction performance (Rc = 0.9736; RMSECV = 0.446 μmol/L; Rp = 0.9342; RMSEP = 0.807 μmol/L). This study exploited a high-efficiency way for the WP extraction industry, and provided theoretical support for the development of the intelligent system in industrial protein extraction process. The scientific interest in radiocesium and plutonium found in the oceans and seas has increased enormously in the past years as a consequence of the Fukushima Dai-ichi Nuclear Power Plant (FDNPP) accident and is expected to be ongoing due to many unresolved questions. Hence, continuous development of new and verification of old analytical methods should be at the top of the list of the community, working on the topic. In this study, we processed and analyzed several seawater samples, collected in different time frames (2011-2015) from the North Pacific Ocean offshore Fukushima, to determine their radiocesium activities, 134Cs/137Cs activity ratios and 240Pu/239Pu isotopic ratios using the sequential scavenging method, gamma spectrometry and accelerator mass spectrometry (AMS). The observed radiocesium levels in seawater (0.07-0.042 Bq L-1) clearly indicated that the investigated region remained impacted by releases from the damaged power plant even after four years after the accident. Regarding plutonium, its successful separation from large volume seawater samples was confirmed by detection of 240Pu by AMS.
