Implementation of lockdown (that includes restricted travel activities) is a prevention strategy executed by various governments to minimize the spread of COVID-19. India went into complete lockdown from 25th March 2020; however, change in commuter's travel behavior was observed from the third week of March (termed as transition to lockdown) due to pandemic fear. In total 1945 participants participated in the travel behaviour survey and their responses with respect to work-based and non-work-based trips during transition period were analysed to understand their adaptation towards COVID-19. The study also attempted to quantify the effects of influencing factors which can explain change in the commuters' travel behaviour. The findings revealed that one-year increment in traveller's age had 2% reduced probability of no travel during transition than pre-transition. For non-work-related travel, chances of lower travel frequency were significantly greater during the transition period as compared to pre-transition. Compared to the non-essential trips, the chances of reduced travel frequency for the essential trips were found to be lower by 92%. By examining these behavioural changes, the present study aims to assist the policymakers in understanding the dynamics of fluctuating travel demand with respect to trip purpose during pandemic situations like COVID-19.Tuberculosis (TB) is a global health problem, in which the majority of cases occur in population-dense developing countries. Despite advances in various diagnostic TB modalities, extrapulmonary TB remains a challenge due to complexities related to its diagnostic approach. Hereby, we present a rare case of endocarditis and spondylodiscitis associated with Mycobacterium tuberculosis (MTB). This case report highlighted the challenges faced in diagnosing blood culture-negative infective endocarditis (BCNIE). We also emphasized the importance of considering MTB as etiology of BCNIE, particularly in endemic TB areas.Bedaquiline (Bdq), a novel TB drug, is referred to the most effective drugs and used for the management of multidrug-resistant tuberculosis (MDR-TB). The drug produces a cardiotoxic effect, and its use is limited to six months. We describe a clinical observation of prolonged bedaquiline use in the treatment for MDR-TB using a restricted number of drugs, to which susceptibility was preserved, in a 12-year-old child. The previous treatment course had failed; the patient remained sputum-positive after eight months of the treatment. We used a personalized approach to chemotherapy correction based on repeat drug susceptibility testing. The treatment regimen only contained those drugs, to which susceptibility was preserved amikacin, cycloserine, linezolid, bedaquiline (AmCsLzdBdq). Amikacin was withdrawn after three months due to the development of sensorineural hearing loss. The treatment was continued with CsLzdBdq. The total chemotherapy course took 18 months. Sputum conversion was observed after one month, cavity closure - by 18 months of treatment. We did not observe cardiotoxic effects due to prolonged bedaquiline use. The administration of prolonged bedaquiline use was based on life-saving considerations. We achieved favourable treatment outcome and demonstrated safety of prolonged bedaquiline use for a child.A 44-year-old woman undergoing therapy for acute promyelocytic leukemia (APL) developed disseminated tuberculosis. Mycobacterium tuberculosis (TB) was isolated from the blood and sputum. Initial drug susceptibility testing (DST) of the blood isolate revealed resistance to isoniazid and ethambutol but the sputum isolate showed no resistance. Due to drug resistance concerns, the patient was treated with multiple second and third-line drugs, and suffered from drug side effects. To further investigate the DST discrepancies, whole genome sequencing (WGS) was performed on both isolates. No known resistance mutations to first line or second line drugs were identified in either isolate, which was confirmed by additional susceptibility testing performed by a different reference laboratory and the California Department of Public Health (CDPH) laboratory. Treatment was reduced to a simpler and less toxic regimen due to these investigations. WGS is shown to be a valuable tool for resolving discordant phenotypic DST results of TB isolates and has the potential to provide accurate and timely results guiding appropriate therapy in the clinical setting.

Bacillus Cereus infection can be life-threatening in immunocompromised patients. We report here a case of Bacillus Cereus septicemia in a child with relapsed acute lymphoblastic leukemia (ALL) and present review of literature.

We collected clinical, laboratory and outcome data of our patient with relapsed ALL and Bacillus Cereus infection. We reviewed literature for Bacillus Cereus infection in pediatric oncology patients by searching MED-LINE/PubMed/Google/Google Scholar/Cochrane and summarized the data obtained. Various risk factors like presence of gastrointestinal or central nervous system (CNS) symptoms, neutropenia, central venous catheter in-situ, corticosteroids use, intrathecal chemotherapy and outcomes were analyzed using Fisher Exact Chi Square test.

A 15-years-old boy with relapsed ALL on induction chemotherapy presented with giddiness and difficulty in breathing. He had an episode of hematemesis followed by fainting at home. He had refractory shock which did not respond to fluid boluses, inotropes and hydrocortisone. He had severe metabolic acidosis with high lactate and ammonia and died within 36-hours of onset of symptoms. His blood culture was positive for Bacillus Cereus. We came across 36 published cases of Bacillus Cereus in children with cancer including present case. Of these, 28 had acute leukemia and rest 8 had other cancers. CNS symptoms were present in 13 patients. Overall mortality was 25%. Patients with multisystem involvement had significantly higher mortality compared to those having localized disease (p-value 0.033).

One of the most consequential effects of the COVID-19 lockdowns was a dramatic reduction in travel during peak hours. Transportation modes also shifted-in particular, travel by car became more rare while bicycling saw a resurgence. Given that a typical year sees the most severe bicycle crashes in peak commuter traffic, the shift toward bicycle travel that occurred in 2020 will likely have been accompanied by unique changes in rider behavior (e.g., where and when they choose to ride) as well as the frequency and severity of vehicle-bicycle crashes.

The current study compared weekday bicycle traffic and crashes in Arlington, VA from March-December 2020 with the same period from years prior, 2013-2019. Bicycle traffic data were obtained from 16 embedded counters placed throughout the study area, in both off-road trails and on-road bike lanes.

We found that 2020 midday traffic nearly doubled compared to the year before, increasing from an average of 68 riders per hour to 120 (+76%). By contrast, morning trave factor against bicycle-motor vehicle crash risk in the face of greater bicycle travel volume.

The unprecedented COVID-19 pandemic affects different domains of society, including the transport system. Due to the impacts of social distancing, research concerning electric bike (e-bike) applications and non(user) health and comfort concerns is needed. This research aims to understand the health and comfort concerns of e-bike (non)users in both pre (PR) and peri (PE) pandemic situations and explore how the PE situation triggers (non)users to use e-bikes.

An interpretive description qualitative method in the form of semi-structured interviews was used to gather the empirical material for this research.

Twenty-three (non)user participants were interviewed in the PR situation, and 12 of these participants were interviewed again in the PE situation. The comfort provided by the e-bike did not outweigh its cost as a factor for nonusers considering investing in the e-bikes in the PR situation. However, nonusers acknowledged that e-bikes can serve as substitutes for public transport by eliminating social intconsiderable potential for e-bikes as substitutes for public transport in the post-pandemic situation. The findings are applicable to e-bike level-of-service studies in order to provide an overview of the user's experience of comfort.The photoacoustic (PA) technique is uniquely positioned for biomedical applications primarily due to its ability to visualize optical absorption contrast in deep tissue at ultrasound resolution. In this work, via both three-dimensional (3D) numerical simulations and in vivo experiments on human subjects, we investigated the possibility of PA measurement of human calcaneus bones in vivo in a non-invasive manner, as well as its feasibility to differentiate osteoporosis patients from normal subjects. The results from the simulations and the experiments both demonstrated that, when one side of the heel is illuminated by laser with light fluence under the ANSI safety limit, the PA signal generated in the human calcaneus bone can be detected by an ultrasonic transducer at the other side of the heel (i.e. transmission mode). In this paper, a new strategy for constructing focus tunable photoacoustic transmitter is proposed. Different from existed prevailing device architecture, a sandwich like photoacoustic conversion layer is innovatively designed into a suspending elastic membrane with clamped boundary and it can be deformed using integrated pneumatic actuator. Owing to the membrane deflection property, concave spherical contours with variable radius of curvature can be obtained. Considering the shape determined sound emission characteristic, continuous tuning on the axial focusing length of the acoustic field has been successfully demonstrated in the photoacoustic transmitter for the first time. Besides, acoustic signal with significantly improved negative pressure has also been achieved especially at the focus, bringing additional advantage for applications.
