PURPOSE The purpose of this study was to describe faculty experiences with nursing students who sustain an NSI. METHODS A retrospective cross-sectional survey design was used to examine nursing faculty experience with student NSIs occurring during clinical practicum experiences. RESULTS Thirty-four percent (n = 904) of nursing faculty respondents reported having supervised at least 1 nursing student who experienced an NSI while providing care to a patient. Only 47% (n = 1112) of faculty indicated that a student could access effective exposure management. CONCLUSIONS Structured simulation activities in which faculty manage a student NSI would provide faculty with essential NSI exposure management experience. Integrating blood-borne pathogen exposure management simulation into faculty development activities would bring a level of realism to the training that a lecture or written policy cannot provide.Cigarettes and electronic cigarettes (e-cigarettes) are major sources of exposure to nicotine, an addictive chemical. Although these products are being regulated by the Food and Drug Administration (FDA) under the Tobacco Control Act, specifications about the nicotine content in these products have not been established yet. In e-cigarettes, nicotine concentration ranges from 0 to > 50mg/mL, and the recent e-cigarette devices provide control to change nicotine flux for higher nicotine delivery. Due to the lack of robust regulations in manufacturing, distribution and marketing, e-cigarettes have already infiltrated the market with youth appealing flavors and devices. As a result, the country is facing a youth epidemic of e-cigarette use. The unregulated nicotine levels in both cigarettes and e-cigarettes can lead to repeated and overexposure of nicotine to youth which can lead to the addiction and detrimental effects on their cognitive functions. Over the past decade, the corrective measures being taken by the FDA for cigarette and e-cigarette regulations also should focus on nicotine exposure levels. Before it is too late to prevent youth from lifetime addiction to nicotine, it is important to address the issues of nicotine concentration, nicotine flux and the e-cigarette device regulations while offering adults with smoking disorder less harmful alternatives to cigarettes.OBJECTIVES The present study aimed to identify multidimensional typologies of harmful alcohol use based on the Swedish Addiction Severity Index (ASI) assessment data on individuals aged 50 years and above. METHODS Latent class analysis examined 11 indicators from ASI data on 1747 individuals (men = 1255, women = 492) who reported they were troubled by alcohol problem at least one day in the past 30 days before their assessment. The discriminative validity of the classes was assessed by comparing other measures of individual characteristics and problem severity of other ASI dimensions. RESULTS Five subtypes of harmful alcohol use were identified. Two classes with alcohol problems varying in psychosocial functioning, age composition and ages of onset of both regular and heavy drinking. Two with psychiatric comorbidity but varying in violence, criminality, gender composition and ages of onset of regular and heavy drinking. One with high prevalence of concurrent use of other substances, psychiatric, legal, and employment problems. CONCLUSIONS The analysis identified, in a national sample, heterogeneous risk groups of older adults with harmful alcohol use. These findings suggest a need for healthcare providers to assess older adults not only for their substance use but also for associated problems and needs. Given these findings, the Addiction Severity Index is a valuable assessment tool for older adults with harmful alcohol use.OBJECTIVES We report on the largest series to date of minimally invasive medial plate osteosynthesis of high energy tibia fractures in skeletally immature patients. DESIGN Retrospective SETTING Level 1 trauma center PATIENTS Skeletally immature patients with high energy tibia fractures treated with minimally invasive medial plate osteosynthesis between 2006 and 2017. We identified 28 fractures in 26 patients. MAIN OUTCOME MEASURE We reviewed the record for patient, injury, and treatment factors as well as for complications and reoperation. We assessed the relationships between age, implant selection and implant removal using chi square and t-test analyses. RESULTS Mean age was 12 years (4 to 15) with OTA/AO classifications including 42A (18), 42B (6) and 42C (4). There were 10 open fractures (one type I, two type II, and seven type III). Mean follow-up was 37.6 months (minimum 7). Mean time to full weight-bearing and union was 2 and 3 months, respectively. read more Plate stock (2.7, 3.5 and 4.5mm) and length (6 to 20 holes) varied independent of age, p = 0.19. There were two superficial infections, one 15mm leg length discrepancy, and one case of fasciotomy at the time of plate fixation for trauma-related compartment syndrome. Implant removal was more likely in younger patients (10.9 vs 13.2 years, p = 0.04), and not associated with plate stock, p = 0.97. There were no neurovascular injuries, losses of reduction, or rotational deformities. Angulation was universally less then 5° in the coronal plane and less then 10° in the sagittal plane. CONCLUSIONS Minimally invasive medial plate osteosynthesis of high energy pediatric tibia fractures may be performed with a low rate of complications and should be considered in this select patient population with multiple injuries, high energy mechanisms, and significant soft tissue trauma. LEVEL OF EVIDENCE IV - case series.Transient receptor potential vanilloid 1 (TRPV1) is a polymodal cation channel gated by a large array of chemical and physical stimuli and distributed across different brain regions on neuronal and glial cells. Preclinical studies indicate that TRPV1 might be a target for the treatment of anxiety, depression and addictive disorders. The aim of this narrative review is to focus on studies examining the effects of TRPV1 antagonism on neuroinflammation, neuroprotection and epigenetic regulation. Results suggest that TRPV1 modulation leads to pro- or anti-inflammatory effects depending on the cytokine environment and that the TRPV1 antagonism can switch the microglia towards an anti-inflammatory phenotype. Moreover, TRPV1 inhibitors have neuroprotective properties through the regulation of calcium levels. Finally, TRPV1 antagonism exerts regulatory effects on genes involved in synaptic and cognitive functions through histone deacetylase 2 inhibition. These findings highlight different mechanisms that may underlie the efficacy of TRPV1 antagonists in animal models of severe psychiatric disorders.
