While the study contains a small group, it may provide guidance for the clinical management of COVID-19 patients to prevent the need for intensive care in the challenging course of therapy (Tab. 2, Fig. 2, Ref. 15).
This study highlights the promise of prone positioning performed in ward conditions for improving oxygenation in COVID-19. While the study contains a small group, it may provide guidance for the clinical management of COVID-19 patients to prevent the need for intensive care in the challenging course of therapy (Tab. 2, Fig. 2, Ref. 15).
Low molecular weight heparin (LMWH) may provide beneficial effects on outcomes of COVID-19. We aimed to examine the impact of LMWH treatment on clinical outcomes (duration of hospitalization, admission to intensive care unit, the requirement for mechanical ventilation, and death) of COVID-19 patients with normal D-dimer levels at admission.
Coronavirus disease-2019 (COVID-19) predisposes patients to arterial and venous thrombosis.
In this retrospective, multicentre and observational study we analysed the data of 308 confirmed COVID-19 patients with normal D-dimer levels at initial admission. After propensity score matching (PSM) patients were grouped; Group 1; patients who received LMWH with D-dimer ≤0.5 mg/L, Group 2; patients who received LMWH after D-dimer levels exceeded 0.5 mg/L, and Group 3; patients who did not receive LMWH.
After PSM, each group comprised 40 patients. The patients in Group1 had the best clinical outcomes compared to the other groups. Group 3 had the worst clinical outcomes (p<0.005). The benefit of LMWH increased with early prophylactic therapy especially when started while the D-dimer levels were ≤0.5 mg/L.
Our results strongly suggest that proactive LMWH therapy improves clinical outcomes in hospitalized COVID-19 patients even with normal D-dimer levels (≤ 0.5 mg/L) (Tab. 3, Fig. 2, Ref. 34).

Metallothionein's (MT) overexpression has been demonstrated immunohistochemically in neoplastic cells of many tumour types. Its elevation above the physiological level has been confirmed in circulation of their hosts. The results of studies dealing with the topic have been summarized to verify if this marker can be applied in the current oncologic practise.
The Pubmed and Google Scholar medical databases were reviewed for full-text articles focused on MT blood (plasma / serum) levels in patients with malignant tumours.
In our review, after a precise selection, we included 8 prospective randomized trials encompassing 561 blood samples taken from patients with a large histopathological spectrum of malignancies. In general, significant differences in blood MT levels between oncological patients and healthy subjects were confirmed. No particular value of the MT level has been demonstrated to be unequivocally predictive of oncologic diseaseCONCLUSION The results of our review suggest that although the elevatef. 49).
The article focuses on main topics related to disease, death, and dying in communication between parents and their adolescent children with this diagnosis.
We conducted qualitative research comprising 13 interviews with parents who lost their adolescent child to cancer. We used a semi-structured interview and interpretative phenomenological analysis.
Results introduced 6 basic topics mutual protection, openness in the communication about cancer and death, making treatment decisions together, talks at the time of passing, hope, and spiritual experience.
Adolescents appreciate age-appropriate, open communication about their disease. Talking about the disease and its prognosis appears to be the way from mutual protection to open truthfulness. Openness also includes the participation of adolescents in further treatment. For some parents, it makes sense to constantly protect the child from the fact of death. Caregivers should support discussions about death between parents and their terminally ill adolescent children and accept individual decisions to talk about death (Tab. 1, Ref. Selleckchem TGF beta inhibitor 25).

The rising incidence of insulin resistance associated with many comorbidities, especially due to the increase in obesity and unhealthy lifestyles, is a serious medical problem today. It is therefore necessary to be able to recognize and evaluate the presence of insulin resistance, prevent its occurrence, and ensure its elimination in high-risk individuals. In our study, we evaluated 106 patients with diabetes mellitus based on glycated hemoglobin parameters, ratio of triacylglycerols to high-density lipoproteins, and body mass index before and after adjustment of dietary and regime measures. Statistical analysis of our data showed a positive correlation between the assessed parameters of insulin resistance and metabolic compensation of diabetes with a Pearson correlation coefficient of 0.3156, and a decrease in glycated hemoglobin and insulin resistance after adjustment of dietary and regimen measures in 73.58 % of patients. Based on the above results, it is shown that non-drug measures are able to significantly improve the parameters of metabolic compensation of diabetes mellitus as well as those of insulin resistance (Tab. 4, Fig. 1, Ref. 18) Keywords insulin resistance, TAG/HDL ratio, diabetes mellitus.
Arthroscopic Latarjet surgery is a technique developed to minimize invasive surgical treatment of anterior shoulder instability. However, compared to an open Latarjet operation, it is considered more technically demanding. The aim of our study was to assess its mid-term success rate in the hands of a surgeon with sufficient experience.
We included 32 patients with recurrent anterior shoulder instability in the analysed cohort who had undergone arthroscopic intervention according to Latarjet as a revision after a previous failed Bankart operation or in the case of the significant bone loss of the glenoid. Patients were followed-up according to a defined protocol for an average of 30.2 months (range 12‒60 months). In this study, we present clinical and radiological results of a long-term follow-up evaluated using Rowe, UCLA, and SST scoring systems, comparing ranges of motion, and thorough analysis of CT examinations performed no earlier than 6 months after surgery. Rowe, UCLA and SST scores were determinedith a low incidence of complications and excellent mid-term clinical outcomes (Fig. 17, Ref. 25).
The aim of this study was to present our experience in the treatment of post-myocardial infarction ventricular septal defect and examine the various risk factors.
This is a retrospective study. From January 2010 to December 2018, 20 patients underwent an urgent /emergency surgical repair of post-myocardial infarction ventricular septal defect.
The mortality in our group of patients was 45 %. Non-survivors compared to the survivors were all in cardiogenic shock (p=0.0098), had an emergency/salvage operation (p=0.0055), preoperative mechanical ventilation (p=0.0081), shorter time between intraaortic balloon pressure insertion and surgery (p=0.0115), shorter median time between ventricular septal defect and surgery, postoperative renal replacement therapy (p=0.0498), and more patients had a residual effect (p=0.0022). In multivariate analysis, preoperative mechanical ventilation (p=0.0001), postoperative renal replacement therapy (p=0.0021) and residual defect (p=0.0000027) were shown to be strong predictors for hospital mortality.
This analysis showed that post-myocardial infarction ventricular septal defect repair is a devastating complication and preoperative mechanical ventilation, postoperative renal replacement therapy and residual defect were identified to be the predictors of mortality. Initial stabilization of the patients, when it is possible, and a delayed repair, may improve the outcome of these patients (Tab. 3, Ref. 17).

The aim of study was to evaluate periprosthetic bone mineral density (BMD) changes of proximal femur, osseointegration and clinical outcomes after implantation of short-stemmed and conventional straight-stemmed prostheses.
This prospective, randomized study included 50 patients with unilateral total hip replacement. The patients were randomized into 2 cohorts patients with a cementless short stem Metha (n=25) and patients with a cementless conventional straight stem Bicontact evaluated as the control (n=25). Periprosthetic BMD changes were measured using a DEXA performed at one-week, 3-monts, 6-months and 1-year follow up. Clinical evaluation with Harris hip score (HHS) and radiographic assessment were performed through a 1 year follow up.
Compared to 1-week postoperative assessment, there were differences in BMD changes between the groups at the final follow-up in all ROIs, with statistical significances in ROI 1, 2, 3, 6 and 7. The loss of periprosthetic BMD in all ROIs around straight stems at each ter, the results of this study suggest a more physiological strain distribution with the short stems versus the straight stems (Tab. 3, Fig. 3, Ref. 25).
The most promising strategy for managing COVID-19 pandemic is achieving sufficient vaccination rate worldwide. The question is how many people will be willing to get vaccinated.
We systematically reviewed peer-reviewed manuscripts monitoring people´s intention to receive a vaccine against COVID-19.
Up to December 28, 2020 we identified 62 relevant peer-reviewed articles in PubMed, Web of Science, Scopus and GoogleScholar.
Total sample size was 118 855 respondents with overall average COVID-19 vaccine acceptance rate of 72.5% which is "just" the level estimated to be sufficient for reaching herd immunity threshold. Surprisingly, healthcare workers showed smaller interest in receiving the vaccine when compared to general adult population and university students. On the other hand, their attitude to vaccination did not change over time. In case of general adult population, the longer the pandemic lasts, the smaller proportion of population wants to get vaccinated. Vaccination intentions were independent of gross domestic product and human development index.
