Pragmatic Free Trail Meta is an open data platform that facilitates research into pragmatic trials. It collects and distributes clean trial data, ratings and evaluations using PRECIS-2. This permits a variety of meta-epidemiological analyses that compare treatment effect estimates across trials of various levels of pragmatism.

Background

Pragmatic trials are increasingly recognized as providing real-world evidence to support clinical decision-making. The term "pragmatic" however, is not used in a consistent manner and its definition and evaluation require further clarification. Pragmatic trials should be designed to guide clinical practice and policy decisions, not to confirm an hypothesis that is based on a clinical or physiological basis. A pragmatic trial should try to be as similar to real-world clinical practice as possible, including in the participation of participants, setting and design, the delivery and execution of the intervention, and the determination and analysis of outcomes and primary analyses. This is a key distinction from explanatory trials (as described by Schwartz and Lellouch1) which are intended to provide a more complete confirmation of a hypothesis.

Trials that are truly pragmatic should be careful not to blind patients or the clinicians as this could lead to bias in estimates of treatment effects. Pragmatic trials will also recruit patients from various health care settings to ensure that their results can be applied to the real world.

Furthermore, pragmatic trials should focus on outcomes that are important to patients, such as quality of life or functional recovery. This is particularly relevant when trials involve surgical procedures that are invasive or may have serious adverse impacts. The CRASH trial29, for example, focused on functional outcomes to compare a two-page report with an electronic system to monitor the health of hospitalized patients with chronic heart failure, and the catheter trial28 focused on urinary tract infections caused by catheters as the primary outcome.

In addition to these characteristics pragmatic trials should reduce the procedures for conducting trials and requirements for data collection to reduce costs. Methods

The use of these terms in titles and abstracts could suggest a greater awareness of the importance of pragmatism, but it is unclear whether this is evident in the content of the articles.
