Cutaneous leishmaniasis is characterized by ulcers with raised edges and a granular bottom, mainly on the lower limbs. This is a case report of a male patient with an ulcer on the left plantar region. The diagnosis was confirmed by positive PCR for L. braziliensis and the presence of amastigotes of Leishmania sp. in the histopathological examination. After treatment with Glucantime, the patient showed full healing of the ulcer. We found no correlation between out-of-pocket costs and occupational status, psoriasis area severity index or other types of treatment. This article analyses the presence of gender bias in clinical trials of monoclonal antibodies used to treat multiple sclerosis.

We performed a systematic review of controlled clinical trials of 4 monoclonal antibodies used to treat multiple sclerosis (natalizumab, rituximab, alemtuzumab, and ocrelizumab). We searched the PubMed/MEDLINE database for articles published in English before March 2020. The study was conducted in accordance with the relevant international recommendations.

The search identified 89 articles, 55 of which met the inclusion criteria. Of all patients included in these trials, 64.6% were women. The lead authors of 10 of the studies were women. Fifteen of the 55 studies included a sex-based analysis of the primary endpoint. Only 8 articles discussed the results separately for men and for women.

Ataxia and hereditary spastic paraplegia are rare neurodegenerative syndromes. We aimed to determine the prevalence of these disorders in Spain in 2019.

We conducted a cross-sectional, multicentre, retrospective, descriptive study of patients with ataxia and hereditary spastic paraplegia in Spain between March 2018 and December 2019.

We gathered data from a total of 1.809 patients from 11 autonomous communities, provided by 47 neurologists or geneticists. Mean (SD) age in our sample was 53.64 (20.51) years; 920 patients were men (50.8%) and 889 were women (49.2%). The genetic defect was unidentified in 920 patients (47.6%). A total of 1371 patients (70.9%) had ataxia and 562 (29.1%) had hereditary spastic paraplegia. Prevalence rates for ataxia and hereditary spastic paraplegia were estimated at 5.48 and 2.24 cases per 100000 population, respectively. The most frequent type of dominant ataxia in our sample was SCA3, and the most frequent recessive ataxia was Friedreich ataxia. Diabetes is best managed by tight glycemic control, as achieving near-normal glucose levels is key to reduce the risk of microvascular complications. Currently, continuous glucose monitoring (CGM) systems have been recognized as the ideal monitoring systems for glycemic control of diabetic patients. Briefly, a CGM system measures blood glucose levels in subcutaneous tissue by attaching a CGM sensor to the skin, allowing the users to make appropriate modifications to their medical interventions according to experience or empirically derived algorithms. The principles of the glucose sensing employed in the current commercially available CGM systems are mainly electrochemical, and employ the gold standard enzyme, glucose oxidase, as the glucose sensing molecule with the combination of hydrogen peroxide monitoring or with the combination of redox mediator harboring hydrogel. Recently, by employing an abiotic synthetic receptor harboring a fluorescent probe combined with a fluorescent detection system, a chronic CGM was commercialized. In addition, the development of less or non-invasive monitoring sensors targeting glucose in tears, sweat, saliva and urine have become of great interest although their clinical relevancy is still controversial. This review article introduces current and future technological aspects of CGM systems, the flagship technology in biosensor research, which was initiated, matured and is still growing in North America.

Spontaneous adrenal hemorrhages (AH) are a rare condition with no consensus about their management.

Patients were identified using the Medicalization of the Information System Program database, imaging software and a call for observations to internists, intensivists and obsetricians working at our institution. Adult patients whose medical records were complete and whose diagnosis was confirmed by medical imaging were included.

From 2000 to 2007, 20 patients were identified, including 15 were women. The clinical onset of AH was non-specific. In five cases, AH occurred during pregnancy; four of them were unilateral and right sided. The etiology of the other fifteen (bilateral adrenal hemorrhage in 11) were as follows antiphospholipid syndrome (n=8), heparin-induced thrombocytopenia (n=4), essential thrombocythemia (n=3), spontaneous AH due to oral anticoagulants (n=1), complication of a surgical act (n=3), and sepsis (n=3). In seven cases, two causes were concomitant. The diagnosis of AH was often confirmed by abdominal CT. An anticoagulant treatment was initiated in 16 cases. Ten of the eleven patients presenting with bilateral adrenal hematomas were treated using a long-term substitute opotherapy. One patient died because of a catastrophic antiphospholipid syndrome.

To compare flexible distal-chip laryngoscopy (FDL) and rigid telescopic laryngoscopy (RTL) in image quality and diagnostic ability.

Prospective cohort study; blinded comparison.

Eighteen normal adult subjects were recruited to undergo both FDL and RTL and normalized videos were recorded. Three blinded laryngologists compared the videos for color fidelity, illumination, resolution, and vascularity, and indicated superiority with FDL, RTL, or no difference. Raters also reported if an abnormality was seen and in which video it was better visualized. Videos for two subjects were repeated to assess intra-rater reliability, making 20 video comparisons across 3 raters for a total of 60 ratings. Yet information regarding the degree to which masks affect acoustics and perception associated with voice performers is scarce. This study examines these effects with common face masks, namely a neck gaiter, disposable surgical mask, and N95 mask, as well as a novel material that could be used as a mask (acoustic foam). A recorded excerpt from the "Star-Spangled Banner" was played through a miniature speaker placed inside the mouth of a masked manikin. Experienced listeners were asked to rate perceptual qualities of these singing stimuli by blindly comparing them with the same recording captured without a mask. Acoustic analysis showed that face masks affected the sound by enhancing or suppressing different frequency bands compared to no mask. Acoustic energy around the singer's formant was reduced when using surgical and N95 masks, which matches observations that these masks are more detrimental to the perceptions of singing voice compared with neck gaiter or acoustic foam. It suggests that singers can benefit from masks designed for minimal impact on auditory perception of the singing voice while maintaining reasonable efficacy of filtering efficiency.

Diabetes is one of the most common comorbidities in patients undergoing total knee arthroplasty (TKA) for osteoarthritis. However, the evidence remains unclear on how it affects patient-reported outcome measures after TKA.

We reviewed prospectively collected data of 2840 patients who underwent primary unilateral TKA between 2008 and 2018, of which 716 (25.2%) had diabetes. All patients had their HbA1c measured within 1 month before surgery, and only well-controlled diabetics (HbA1c <8.0%) were allowed to proceed with surgery. Patient demographics and comorbidities were recorded, and multiple regression was performed to evaluate the impact of diabetes on improvements in patient-reported outcome measures (Short Form 36 (SF-36), Western Ontario and McMaster University Osteoarthritis Index (WOMAC), Knee Society Score (KSS)) and knee range of motion (ROM).
