Adrenocortical carcinoma (ACC) is an orphan but aggressive malignancy with limited treatment options. Cabozantinib (CAB), a tyrosine kinase inhibitor (TKI), has emerged as a new potential treatment. However, no data are available on whether and how CAB can be administered to patients undergoing hemodialysis.

An LC-MS/MS method was developed and validated according to the EMA and FDA guidelines for bioanalytical method validation. The samples were prepared using protein precipitation and online solid-phase extraction (SPE). The method was applied to clinical samples of an ACC patient receiving CAB treatment (80 mg daily). During the ten days of observation, the patient received periodic hemodialysis on seven days. Pharmacokinetic (PK) simulations were performed using Bayesian forecasting according to an existing population PK model for CAB.

Based on the PK simulation, a mean plasma trough concentration of 1375 ng/mL (90% prediction interval (PI) 601-2602 ng/mL) in the steady state at a daily dose of 80 mfor the evaluation of 'new' drugs in patients with renal impairment.Inflammatory myofibroblastic tumor (IMT) is a distinctive fibroblastic and myofibroblastic spindle cell neoplasm with an accompanying inflammatory cell infiltrate and frequent receptor tyrosine kinase activation at the molecular level. The tumor may recur and rarely metastasizes. IMT is rare in the head and neck region, and limited information is available about its clinicopathologic and molecular characteristics in these subsites. Therefore, we analyzed a cohort of head and neck IMTs through a multi-institutional approach. Fourteen cases were included in the provisional cohort, but 1 was excluded after molecular analysis prompted reclassification. Patients in the final cohort included 7 males and 6 females, with a mean age of 26.5 years. Tumors were located in the larynx (n=7), oral cavity (n=3), pharynx (n=2), and mastoid (n=1). Histologically, all tumors showed neoplastic spindle cells in storiform to fascicular patterns with associated chronic inflammation, but the morphologic spectrum was wide, as is chaiagnostic accuracy and broadened therapeutic options for patients.

The aim is to evaluate the efficacy of a novel device simultaneously delivering HIFEM and radiofrequency for subcutaneous fat reduction and muscle toning.

Forty-one subjects with an average age of 39.7 ± 11.5 years were recruited. The subjects received 3 abdominal treatments (one per week). Magnetic resonance imaging images of the treated area were evaluated at baseline and at 1-, 3-, and 6-month visits for changes in subcutaneous fat, muscle thickness, and abdominal separation (AS). Anthropometric data and digital photographs were collected. Subject satisfaction and therapy comfort were evaluated.

The muscle mass increase peaked at 3 months, showing 26.1% thickening. The fat thickness reduction was most prominent at 3 months, showing a 30.8% reduction. The AS decreased by 18.8% at 3 months after treatment. The waist circumference reduced by 5.87 ± 3.64 cm at a 3-month follow-up. Six-month data showed maintenance of these outcomes. The treatment was considered as comfortable with high patient satisfaction.

The last decade has witnessed tremendous advances in revealing an important role for the interleukin (IL)-17 cytokine family in the pathogenesis of spondyloarthritis (SpA). Although most attention has been focused on IL-17A, a potential role of other IL-17 family members in inflammation and tissue remodelling is emerging. Herein, I review recent studies covering the role of IL-17B-F cytokines in the pathogenesis of SpA.

Several recent studies provided new insights into the cellular source, regulation and function of IL-17F. IL-17F/IL-17A expression ratio is higher in psoriatic skin compared to SpA synovitis. IL-17F-expressing T cells produce different proinflammatory mediators than IL-17A-expressing cells, and IL-17F and IL-17A signal through different receptor complex. Dual IL-17A and IL-17F neutralization resulted in greater suppression of downstream inflammatory and tissue remodelling responses. Furthermore, there is additional evidence of IL-23-independent IL-17 production. In contrast to IL-17A, IL-17F and IL-17C, which play proinflammatory roles in skin and joint inflammation, an anti-inflammatory function is proposed for IL-17D. An increase in IL-17E is associated with subclinical gut microbiome alterations after anti-IL-17A therapy in SpA patients.

IL-17 family cytokines may act as agonists or antagonists to IL-17A contributing in concert to local inflammatory responses. Understanding their function and identifying their cellular sources, and molecular mechanisms driving their expression will be the key to designing rational therapies in SpA.

Although clinical presentation of coronavirus disease 2019 has been extensively described, immune response to severe acute respiratory syndrome coronavirus 2 remains yet not fully understood. Similarities with bacterial sepsis were observed; however, few studies specifically addressed differences of immune response between both conditions. Here, we report a longitudinal analysis of the immune response in coronavirus disease 2019 patients, its correlation with outcome, and comparison between severe coronavirus disease 2019 patients and septic patients.

Our aim in this article is to evaluate the safety and efficacy of triple antithrombotic therapy (TT, anticoagulant plus dual antiplatelet) versus dual antithrombotic therapy (DT, anticoagulant plus single antiplatelet) in patients with acute coronary syndrome. We included all randomized trials comparing the outcomes of single versus dual antiplatelet therapy in patients with acute coronary syndrome on anticoagulants. The primary outcome was major adverse cardiac events (MACE). Other outcomes studied were all-cause mortality, cardiovascular mortality, myocardial infarction (MI), stroke, stent thrombosis and major bleeding. The Mantel-Haenszel risk ratio (RR) random effects model was used to summarize data. Six studies, with a total of 11,437 patients, met our selection criteria. With a follow-up duration of 9-14 months, there was no significant difference between DT and TT in terms of MACE (RR 0.96, 95% confidence interval [CI] 0.79-1.17), all-cause mortality (RR 1.00, 95% CI 0.77-1.29), cardiovascular mortality (RR1.03, 95% CI 0.79-1.34), MI (RR 1.14, 95% CI 0.90-1.45), stroke (RR 0.83, 95% CI 0.56-1.23) and stent thrombosis (RR 1.32 95% CI 0.87-2.01). Compared with TT, DT was associated with significant reductions in major bleeding 4.1% vs 6.5% (RR 0.61, 95% CI 0.45-0.81, NNT=42), clinically significant bleeding 10.5% vs 16.4% (RR 0.62, 95% CI 0.48-0.80) and intracranial hemorrhage 0.4% vs 0.8% (RR 0.43, 95% CI 0.24-0.77). Hypomagnesaemia has been linked with increased cardiovascular mortality in heart failure, however previous studies have yielded conflicting results. Even fewer studies have addressed the association between hypermagnesemia and prognosis in heart failure. The aim of the present systematic review was to investigate the association of serum magnesium levels with cardiovascular and all-cause mortality in patients with heart failure and reduced ejection fraction (HFrEF). iCRT3 Cardiovascular morbidity, referring to heart failure rehospitalizations and ventricular arrhythmias, was also investigated. Eligible studies were identified by searching PubMed and Scopus. The QUIPS tool was used to assess the quality of included studies. Eight studies (total of 13539 patients with HFrEF) that assessed the effects of serum magnesium levels on cardiovascular mortality, all-cause mortality and cardiovascular morbidity met inclusion criteria. In half of the studies, hypomagnesaemia was found to be an independent risk factor for cardiovascular mortality, including sudden cardiac death. Only one study reported that hypermagnesemia (serum magnesium levels above 2.4mg/dl) is a prognostic factor for non-cardiac mortality suggesting that hypermagnesemia is more likely an indicator of co-morbidities rather than a true independent prognostic marker. Finally, low serum magnesium levels were not associated with readmissions for heart failure or ventricular arrythmias in patients with HFrEF.
