People often fail to keep their mind from wandering. Here, we examine how the tendency to mind wander is affected by people's beliefs, or lay theories. Building on research on lay theories and self-regulation, we test whether differences in people's beliefs about the extent to which mind wandering is controllable affect thought control strategies and mind-wandering rates in daily life and the laboratory. We develop a new scale to assess control-related beliefs about mind wandering. Scores on the scale predict mind wandering (Study 1) and intrusive thoughts (Study 2) in everyday life, thought control strategies and dysfunctional responses to unwanted thoughts (Study 2), and mind wandering during reading in the laboratory (Studies 3-6). Moreover, experimentally induced lay theories affect mind-wandering rates during reading (Studies 4 and 5). Ethical challenges are common in clinical nursing practice, and an infectious environment could put nurses under ethical challenges more easily, which may cause nurses to submit to negative emotions and psychological pressure, damaging their mental health.
To examine the ethical challenges encountered by nurses caring for patients with the novel coronavirus pneumonia (COVID-19) and to provide nurses with suggestions and support regarding promotion of their mental health.
A qualitative study was carried out using a qualitative content analysis. The participants were 18 nurses who agreed to attend an interview and describe their own experiences of providing care to COVID-19 patients in China. They were purposively sampled, and structured, in-depth interviews were performed. Data were iteratively collected and analyzed from February to March 2020.
The proposal was approved by the Research Ethics Committee of the Second Hospital of Shandong University, China.
The findings revealed three main themes and 10 categories. The themes were the following (1) ethical challenges (people with COVID-19, inequality, professional ethics, and job competency); (2) coping styles (active control and planning, seeking support as well as catharsis, and staying focused); and (3) impacts on career (specialized nursing skills, scientific research ability, and management skills).

Exit-site infection (ESI) is an important risk factor for peritonitis in patients under chronic peritoneal dialysis (PD). The International Society for Peritoneal Dialysis (ISPD) recommend its diagnosis as the presence of purulent drainage in the exit site (ES) but time-consuming scores using others skin signs are routinely used.
To investigate the correlation between the diagnosis of ESI with a score based on five clinical signs obtained from ES inspection, and also if there are interaction with patient's race.
Multicenter and prospective cohort. We included adult patients from 122 clinics, incident on PD and with a 12-month stay in therapy. The event of interest was ESI, defined as purulent drainage. The clinical score used was composed of hyperemia, edema, pain, scab, and granuloma. Statistical analysis was performed using multilevel logistic regression model, likelihood test, and Cohen concordance analysis.
A total of 35,354 ES assessments were performed during the first year of dialysis in 3297 pon in addition to the presence of purulent secretion as currently recommended by the ISPD.Objective The discovery of anti-citrullinated protein antibodies (ACPAs) and the introduction of new therapeutic options have had profound impacts on early rheumatoid arthritis (RA) care. Since ACPA status, most widely assessed as reactivity to cyclic citrullinated peptides (CCPs), influences treatment decisions in early RA, we aimed to determine whether anti-CCP remains a predictor of disease activity and radiographic joint damage in more recent 'real-world' early RA. Method Two observational early RA cohorts from Sweden enrolled patients in 1996-1999 (TIRA-1, n = 239) and 2006-2009 (TIRA-2, n = 444). Clinical and radiographic data and ongoing treatment were prospectively collected up to 3 years. Two other cohorts served as confirmation cohorts (TRAM-1, with enrolment 1996-2000, n = 249; and TRAM-2, 2006-2011, n = 528). Baseline anti-CCP status was related to disease activity, pharmacotherapy, and radiographic joint damage according to Larsen score. Results In the TIRA-1 cohort, anti-CCP-positive patients had significantly higher 28-joint Disease Activity Score, swollen joint count, C-reactive protein level, and erythrocyte sedimentation rate during follow-up compared with anti-CCP-negative patients. In TIRA-2, no such differences were found, but baseline anti-CCP positivity was associated with higher 3 year Larsen score (5.4 vs 3.5, p = 0.039). In TRAM-2, anti-CCP also predicted radiographic damage (8.9 vs 6.7, p = 0.027), with no significant differences in disease activity. Conclusion In the early RA cohorts recruiting patients in 2006-2011, baseline anti-CCP positivity was not associated with disease activity over time, but was associated with increased radiographic damage at follow-up. Hence, close radiographic monitoring is warranted in early anti-CCP-positive RA regardless of disease activity.
Dropped head syndrome (DHS) is characterized by the passively correctable chin-on-chest deformity. The characteristic feature is emphasized in the cervical flexion position. The purpose of this study was to analyze the influence of cervical flexion on sagittal spinal alignment in patients with DHS.
The study included 15 DHS subjects and 55 cervical spondylosis (CS) subjects as the control group. The following parameters were analyzed cervical sagittal vertical axis (C-SVA), occipitoaxial angle (O-C2A), C2 slope (C2S), C2-C7 angle (C2-C7A), T1 slope (T1S), sagittal vertical axis, T1-T4 angle (T1-T4A), T5-T8 angle (T5-T8A), T9-T12 angle, lumbar lordosis, sacral slope, and pelvic tilt, in cervical flexion and neutral positions.
The values of C-SVA, O-C2A, C2S, and T1S were significantly different between CS and DHS at cervical neutral and flexion positions. C2-C7A showed significant difference in cervical neutral position, but the difference disappeared in flexion position. T1-T4A did not present a significant difference, but T5-T8A showed a difference in neutral and flexion positions.
