BACKGROUND Financial pressures for reducing hospitalization costs have driven to a move toward same day discharge (SDD) following uncomplicated percutaneous coronary intervention. The UK healthcare system has transitioned to predominantly SDD for elective percutaneous coronary intervention. This study aimed to examine patient's clinical, procedural, and institutional characteristics that are associated with the increased adoption of SDD adoption over time in the United Kingdom and determine whether these vary by region. METHODS The data were derived from the British Cardiovascular Intervention Society including all the elective percutaneous coronary intervention from 2007 to 2014 in the United Kingdom. We structured 8 meaningful groups of variables, and their relative importance was obtained by decomposing the R2 in each study year. RESULTS The relative importance of Strategic Health Authorities was substantially higher than all other factors every year, with some reduction over time, from 49.2% (95% CI, 45.4%-52.4%) in 2007 to 43.4% (95% CI, 39.9%-46.6%) in 2014. Center volume followed with 8.95% (95% CI, 7.0%-10.9%) to 19.8% (95% CI, 16.7%-22.4%). Between patients' clinical and procedural characteristics, pharmacology and access site had the highest relative importance values, from 14.3% (95% CI, 12.1%-16.4%) to 7.1% (95% CI, 5.5%-8.8%) and from 3.6% (95% CI, 2.3%-5.1%) to 11.8% (95% CI, 9.4%-14.3%), respectively. Relative importance of different groups varied differently across Strategic Health Authorities. CONCLUSIONS Growth of SDD was mainly associated with regional characteristics, while subcontributors varied substantially between different regions. Standardized guidelines would provide more homogenous adoption of SDD nationally. This analysis might be of wider interest in healthcare systems slower in SDD adoption.BACKGROUND Women are more likely to suffer and die from cardiogenic shock (CS) as the most severe complication of acute myocardial infarction. Data concerning optimal management for women with CS are scarce. Aim of this study was to better define characteristics of women experiencing CS and to the influence of sex on different treatment strategies. METHODS In the CULPRIT-SHOCK trial (The Culprit Lesion Only PCI Versus Multivessel PCI in Cardiogenic Shock), patients with CS complicating acute myocardial infarction and multivessel coronary artery disease were randomly assigned to one of the following revascularization strategies either percutaneous coronary intervention of the culprit-lesion-only or immediate multivessel percutaneous coronary intervention. Primary end point was composite of death from any cause or severe renal failure leading to renal replacement therapy within 30 days. We investigated sex-specific differences in general and according to the revascularization strategies. RESULTS Among all 686 rCS complicating acute myocardial infarction and multivessel coronary artery disease should not be treated differently. However, further randomized trials powered to address potential sex-specific differences in CS are still necessary. The fast-growing literature on the use of computational fluid dynamics (CFD) in fixed-bed design reflects the rapid development of this subfield of reactor modeling. We identify recent trends and research directions in which successful methodology has been established, for example, in computer generation of packings of complex particles, and where more work is needed, for example, in the meshing of nonsphere packings and the simulation of industrial-size packed tubes. Development of fixed-bed reactor models, by either using CFD directly or obtaining insight, closures, and parameters for engineering models from simulations, will increase confidence in using these methods for design along with, or instead of, expensive pilot-scale experiments. I describe my family, my early education, my college and graduate school training in physics, my postdoc years in chemistry, and my subsequent transformation into a chemical engineering faculty member-one of the first women to be appointed to a chemical engineering faculty in the United States. I focus on the events that shaped me, the people who noticed and supported me, and the environment for women scientists and engineers in what some would call the "early days." My initial research activities centered on applications of statistical mechanics to predict phase equilibria in simple systems. Over time, my interests evolved to focus on applying molecule-level computer simulations to systems of interest to chemical engineers, e.g., hydrocarbons and polymers. Eventually, spurred on by my personal interest in amyloid diseases and my wish to make a contribution to human health, I turned to more biologically oriented problems having to do with protein aggregation and protein design. I give a candid assessment of my strengths and weaknesses, successes and failures. Finally, I share the most valuable lessons that I have learned over a lifetime of professional and personal experience. These microorganisms are able to accomplish this feat by enhancing the activity of the CO2-fixing enzyme Rubisco using biophysical CO2 concentrating mechanisms (CCMs). Biophysical CCMs operate by concentrating bicarbonate and converting it into CO2 in a compartment that houses Rubisco (in contrast with other CCMs that concentrate CO2 via an organic intermediate, such as malate in the case of C4 CCMs). This activity provides Rubisco with a high concentration of its substrate, thereby increasing its reaction rate. The genetic engineering of a biophysical CCM into land plants is being pursued as a strategy to increase crop yields. This review focuses on the progress toward understanding the molecular components of cyanobacterial and algal CCMs, as well as recent advances toward engineering these components into land plants. Additionally, the study investigates the association between physical activity and self-rated health while adjusting for various covariates at the individual, household, and district levels in Seoul, South Korea. A cross-sectional study of the 2009 Seoul Welfare Panel Study conducted by Seoul Welfare Foundation was utilized. The final sample included 7,761 individuals within 3,617 households in 25 administrative areas. Three-level random intercept logistic models were fitted. The results showed that a small proportion (3.52%) of variance in self-rated health was attributed to the administrative-area level, while a relatively large proportion (33.78%) was attributed to the household level. This study also found a positive association between physical activity and self-rated health, even after controlling for covariates at multiple levels. The results indicated that public health interventions promoting physical activity may have a beneficial effect on individual health. Additionally, public health interventions for improving individual health may not be efficient if directed only at the administrative-area level, as only a small portion of variance in self-rated health was attributable to this level.OBJECTIVE To determine the prevalence of hypnic headache. BACKGROUND The exact prevalence of hypnic headache is unknown since there are no published population-based prevalence studies. METHODS This study was a pilot for the SAGA cohort study, a population-based study on life stressors and various indices of health. Of 1398 invited adults, 921 (66%) participated; 402 men (average age 45.6 years, SD 13.2) and 519 women (52.6 years, SD 11.1). Subjects answered a headache questionnaire including a screening question for hypnic headache. "Do you have a headache that occurs only during sleep and causes wakening?". Diagnosis of hypnic headache was made by clinical interview using ICHD-3 criteria. RESULTS Among 921 participants, six screened positive for hypnic headache, of those two 0.22% (95% CI 0.06-0.79%) had probable hypnic headache and none had definite hypnic headache. CONCLUSION Confirming that hypnic headache is rare, these data suggest a 0.22% prevalence of probable hypnic headache.SIGNIFICANCE Selenenyl sulfides and thioseleninic acids are the mono-selenium analogues of disulfides and persulfides that contain Se-S bonds. These bonds are found in several antioxidant regenerating enzymes as derivatives of selenocysteine, making them an important player in redox biology as it pertains to sulfur redox regulation. Recent Advances Mechanistic studies of redox regulating selenoenzymes like thioredoxin reductase and glutathione peroxidase suggest crucial Se-S bonds in the active sites. Peptide models and small molecule mimics of these active sites have been prepared to study their fundamental chemistry. These advances help pave the road to better understand the functions of the Se-S bond in the body. CRITICAL ISSUES The Se-S bond is unstable in atmospheric temperatures and pressures. Therefore, studying their properties proposes a major challenge. Currently there are no trapping reagent specific to selenenyl sulfides or thioseleninic acids, making their presence, identity, and fates in biological environments difficult to track. Future Direction Further understanding the fundamental chemistry/biochemistry of selenenyl sulfides and thioseleninic acids are needed to understand what their intracellular targets are and to what extent they impact signaling. Besides antioxidant regeneration and peroxide radical reduction, the roles of selenenyl sulfide and thioseleninic acids in other systems need to be further explored.SIGNIFICANCE The p53 tumor suppressor has been dubbed the "guardian of genome" because of its various roles in the response to DNA damage such as DNA damage repair, cell cycle arrest, senescence and apoptosis, all of which are in place to prevent mutations from being passed on down the lineage. CRITICAL ISSUES More recently, ROS have been shown to also play important physiological roles as second messengers in so-called redox signaling. It is therefore not clear whether the observed activation of p53 by ROS is mediated through the DNA damage response, redox signaling or both. In this review we will discuss the similarities and differences between p53 activation in response to DNA damage and redox signaling in terms of upstream signaling and downstream transcriptional program activation. FUTURE DIRECTIONS Understanding whether and how DNA damage and redox signaling-dependent p53 activation can be dissected could be useful to develop anti-cancer therapeutic p53-reactivation strategies that do not depend on the induction of DNA damage and the resulting additional mutational load.
