The medical education community has implemented writing exercises that foster critical analysis and nurture reflective capacity. The REFLECT rubric (Wald etal. 2012) was developed to address the challenge of assessing these written reflections. The objective of this replication work is to explore the reproducibility of the reliability characteristics presented by the REFLECT developers.

Five raters evaluated narratives written by medical students and experienced clinicians using the REFLECT rubric. Reliability across rubric domains was determined via intraclass correlation coefficient and internal consistency was determined via Cronbach's alpha.

Intraclass coefficients demonstrated poor reliability for ratings across all tool criteria (0.350-0.452) including overall ratings of narratives (0.448). Moreover, the internal consistency between scale items was also poor across all criteria (0.529-0.621).

We did not replicate the reliability characteristics presented in the original REFLECT article. We consider these findings with respect to the contextual differences that existed between our study and the Wald and colleagues study, pointing particularly at the possible influence that repetitive testing and refinement of the tool may have had on their reviewers' shared understanding of its use. We conclude with adiscussion about the challenges inherent to reductionist approaches to assessing reflection.

The role of ecological factors in gut colonisation has been extensively investigated, as well as the role of micro- and macronutrients in supporting and selecting for bacterial species highly adapted for utilising those nutrients. However, the microbial community contained in poultry feed and its influence on colonisation and maturation of gut microbiota has not been directly addressed. In this study, we compared the microbiota found in poultry feed, with the microbiota of ileum, cecum and excreta, to identify substantial overlap in core microbiotas of the compared groups. We then investigated the microbiota present in raw feedstuffs meat and bone meal, wheat, corn, canola, barley, soybean, millrun, sorghum, poultry oil, oats, limestone and bloodmeal from four geographically distinct feedstuff suppliers. Each of the feedstuffs had diverse microbial communities. The meat and bone meal and bloodmeal samples had the most complex and distinct microbial populations. There was substantial overlap in the phylogenetic composition found in the grain and seed samples barley, canola, corn, millrun, oats, sorghum, soybean meal and wheat. Issues related to methodology, viability of microbial communities in the gut and feed, and the implications for biosecurity are discussed.Development of a delivery system to lower systemic toxicity and enhance doxorubicin (DOX) antitumor efficacy against multi-drug resistant (MDR) tumors is of great clinical significance. Dihydroartemisinin in vitro Here, lipid/hyaluronic acid (HA)-coated DOX-Fe3O4 was characterized to determine its optimal safety and efficacy on a tumor. DOX was first conjugated onto the Fe3O4 NPs surface, which was subsequently coated with phosphatidylcholine (PC) lipids, which consisted of a tumor cell-targeting HA ligand, to generate a dual-targeting nanoparticle (NP). DOX-Fe3O4 synthesis was validated by the Fourier-transform infrared (FT-IR) analysis results. Core-shell PC/HA@DOX-Fe3O4 formation, which had an average particle size of 48.2 nm, was observed based on the transmission electron microscopy (TEM) and dynamic laser light scattering (DLS) results. The saturation magnetization value of PC/HA@DOX-Fe3O4 was discovered to be 28 emu/g using vibrating-sample magnetometry. Furthermore, the designed PC/HA@DOX-Fe3O4 achieved greater MCF-7/ADR cellular uptake and cytotoxicity as compared with DOX. Graphical abstract.Sea cucumbers are fished worldwide for export to Asia, but few studies have evaluated metal pollution and risk assessment. This study assessed concentration of trace metals and the potential ecological risk in sea cucumber (Holothuria floridana) and sediments at the Cispatá Bay, in the Caribbean Coast of Colombia. The trace metal concentrations in biota and sediments showed a similar decreasing trend as follows Cu > Zn > Hg > Pb > Cd. The highest bioconcentration factor was found for Hg, and according to the sediment quality guidelines, Cu levels indicate adverse biological effects in the ecosystem. In this regard, Cu levels were higher than effects range low and the threshold effect levels (TEL) and lowest effect level in all the stations, whereas Hg levels were higher than TEL in most of them. Results for health risk assessment based on the maximum allowable daily consumption rate (CRlim) suggest that there is no risk to adults; however, children should limit or avoid its consumption. Results from multivariate statistical analysis suggest that agricultural activities (i.e., application of fertilizers and agrochemicals) were identified as the main anthropogenic sources of metal pollution. This research suggest that sea cucumber could be used as a bioindicator species in studies of monitoring metal contamination, with special attention to the highly significant correlation between Zn in tissue and sediments. This study also reveals that anthropic activities may have negative effects in the quality of the sediments of the bay and contributing to the knowledge of metal accumulation in sea cucumber.Graphical abstract.It was aimed to evaluate serum zinc and copper levels and oxidative stress parameters in ectopic pregnancy cases, healthy pregnant women, and healthy non-pregnant women. In this cross-sectional case-control study, 30 patients diagnosed with tubal ectopic pregnancy in the gynecology clinic of a tertiary hospital constituted the study group. A healthy pregnant control group (n = 30) was formed of age, body mass index (BMI), and gestational week-matched subjects, and a healthy non-pregnant control group (n = 30) was formed of age and BMI-matched women. The groups were compared in terms of demographic characteristics and laboratory parameters including serum zinc (Zn) level, serum copper (Cu) level, serum malondialdehyde (MDA) level, serum catalase (CAT) activity, serum glutathione peroxidase (GPX) activity, and serum superoxide dismutase (SOD) activity. The groups were similar in respect of demographic characteristics. link2 In the ectopic pregnancy group, serum GPX activity and Cu level were significantly lower, and current study can be considered of value as the first study in literature to show a significantly lower serum Zn level and higher serum Cu level in ectopic pregnancy cases compared with healthy pregnant control cases. This is also the first study to have revealed an association between the serum Cu/Zn ratio, oxidative status, and ectopic pregnancy. Furthermore, the serum Cu/Zn ratio was found to be useful in the diagnosis of ectopic pregnancy cases.Cadmium (Cd) is a highly toxic metal and kidney is its main target. However, the molecular effects and associated potential impacts of Cd-accumulated kidney have not been well investigated. In this study, mouse was used as a model to investigate the Cd-induced proteomic profile change in kidney, and a total of 34 differentially expressed proteins were detected by two-dimensional gel electrophoresis (2-DE) and further identified by matrix-assisted laser desorption/ionization time of flight mass spectrometry (MALDI-TOF-MS). Through Gene Ontology analysis and KEGG pathway annotation, it showed that Cd-regulated kidney metabolism and promoted renal damage and cell migration. By validation of Western blotting and RT-qPCR, metastasis-related proteins LIM and SH3 domain protein 1 (LASP1) and phosphoenolpyruvate carboxykinase/cytosolic [GTP] (PEPCK1) were confirmed to be upregulated; Acyl-CoA synthetase medium-chain family member 3 (ACSM3) was downregulated. Furthermore, carcinoma development-related proteins initiation factor 4A (eIF4A) and pyridoxine-5'-phosphate oxidase (PNPO) were upregulated, and pyridoxal kinase (PK) was downregulated. The downregulation of Na(+)/H(+) exchange regulatory cofactor (NHERF3) might promote renal damage which associated with decrease of transferrin (TRF) in kidney. Taken together, our results revealed proteomic profile of Cd-induced nephrotoxicity and provided data for further insights into the mechanisms of Cd toxicity.Herbal medicines are widely consumed by the people of Pakistan for the treatment of various diseases. In the current study, quantitative assessment of four heavy metals namely zinc, lead, chromium, and cobalt was carried out in forty commonly consumed herbal medicines purchased from the markets of Khairpur and Sukkur, Pakistan. The conventional wet acid digestion method was used for the dissolution of samples and the concentration of heavy metals was determined by using flame atomic absorption spectrometry. link3 Varied concentrations of metals were observed in different herbal medicines. The levels of Zn, Pb, Cr, and Co were in the range of 7.05-87.5 ppm, 5.62-44.9 ppm, 3.16-14.1 ppm, and 5.69-9.88 ppm, respectively. The findings of our study were compared with the WHO maximum permissible limits of these metals and it was observed that most of the analyzed samples exceeded the permissible levels. Keeping in view the levels of these metals in the selected herbal medicines, the prolonged intake of these drugs might be hazardous for human beings.Presence of Sudan I in food stuff can be problematic and need to be checked in order to protect our health from possible carcinogen. Therefore, it is essential to detect Sudan I by efficient, rapid and reliable method. In this work, we have designed a Ru(II) polypyridyl complex, [Ru(bpy)2(CIP)]2+ probe for the selective and sensitive detection of Sudan I. Upon addition of Sudan I to the solution of [Ru(bpy)2(CIP)]2+ in ethanol, the luminescence quenched rapidly, and linear concentration range with analyte has been obtained from 0.8 to 100 μM with the limit of detection as low as 0.26 μM (S/N = 3). The effective luminescence quenching was resulted due to the inner filter effect (IFE) between luminophore, [Ru(bpy)2(CIP)]2+ and quencher, Sudan I. Our spectroscopic study was essentially provided sufficient analytical evidences in order to prove occurrence of IFE mechanism. As there were no interferences observed in luminescence measurement from the other substances the present probe has been successfully applied for the detection of Sudan I in commercial chili powder sample, making the probe suitable for practical usage.
