Despite the growing evidence that metastatic lymph node ratio (MLNR) is a valuable predictor for the prognosis of papillary thyroid carcinoma, it has not yet been fully determined which factors give the ratio predictive value independent of the number of metastatic lymph nodes (MLNs).

Retrospective cohort study.

A comprehensive cancer center.

Recurrence and clinicopathologic factors were analyzed in 2409 patients with papillary thyroid carcinoma who underwent total thyroidectomy and central node dissection.

Cutoff values of MLNs ≥2 and MLNR ≥28.2% increased the recurrence risk (hazard ratio [95% CI], 9.97 [4.73-21.0] and 11.4 [5.53-23.3], respectively). Younger age, male sex, multifocality, tumor size, lymphatic and vascular invasion, and gross extrathyroidal extension positively correlated with MLN and MLNR (all

< .05). Meanwhile, lymphocytic thyroiditis negatively correlated with MLNR in female patients (

< .001), by increasing total lymph node yields as compared with papillary thyroid carcinoma without lymphocytic thyroiditis. In multivariate analysis, younger age, tumor size, and lymphatic invasion remained significant in male and female patients for MLN and MLNR; lymphocytic thyroiditis was also significantly correlated with MLNR in female patients.

Our study demonstrates that MLN and MLNR are independently observed prognostic markers for tumor recurrence. However, lymphocytic thyroiditis in female patients seems to have lower MLNR by increasing total lymph node yields. In light of their association, a different cutoff for MLNR needs to be applied according to the presence or absence of underlying lymphocytic thyroiditis in the use of MLNR for predicting the recurrence.

4.Deficit-based accounts of social and communication abilities continue to dominate autism research. However, emerging findings suggest that this view may be overly simplistic and discount the two-way nature of interaction. Here we discuss the reconceptualization of social cognition to consider such difficulties as examples of bidirectional, multifaceted misattunement between autistic and nonautistic individuals. Aligned with progressive theoretical frameworks, emerging empirical research indicates that mismatches in communication styles can contribute to autistic social difficulties and the important role that nonautistic difficulties play. We highlight two areas of future research with the aim of providing empirical support for the views that the autistic community has proposed over the past 2 decades. We discuss the impact of such a paradigm shift on a number of levels, including how bridging the gap between different interaction styles can reduce stigma and increase understanding. Adopting such a framework will provide radical opportunities for transformative societal changes and education around inclusion.Because of the strong overreliance on p values in the scientific literature, some researchers have argued that we need to move beyond p values and embrace practical alternatives. When proposing alternatives to p values statisticians often commit the "statistician's fallacy," whereby they declare which statistic researchers really "want to know." Instead of telling researchers what they want to know, statisticians should teach researchers which questions they can ask. In some situations, the answer to the question they are most interested in will be the p value. As long as null-hypothesis tests have been criticized, researchers have suggested including minimum-effect tests and equivalence tests in our statistical toolbox, and these tests have the potential to greatly improve the questions researchers ask. If anyone believes p values affect the quality of scientific research, preventing the misinterpretation of p values by developing better evidence-based education and user-centered statistical software should be a top priority. Polarized discussions about which statistic scientists should use has distracted us from examining more important questions, such as asking researchers what they want to know when they conduct scientific research. Before we can improve our statistical inferences, we need to improve our statistical questions.

To compare audiometric outcomes and complication rates between primary and revision stapes surgical cases.

Retrospective cohort study.

Large single-institution database.

Data on 809 patients (including 170 revisions) undergoing primary and revision stapes surgery were reviewed, with Pearson chi-square and multivariable logistic regression analysis performed.

Rates of postoperative air-bone gap (ABG) closure to ≤10 dB were significantly worse in the revision group (40.2% vs 61.8%,

< .001), as were those to ≤20 dB (78.1% vs 90.9%,

< .001). The bone conduction pure tone average worsened by >10 dB in 7.1% of primary cases and 13.1% of revisions (

= .016). The mean postoperative ABG for revision cases was significantly higher at 15.5 dB as compared with 11.0 dB for primaries (

< .001), despite a slightly higher preoperative ABG for primary cases (30.6 vs 28.24 dB,

= .010). In multivariate analysis, revision surgery had an odds ratio 0.41 (

< .001) in closing the ABG to ≤10 dB. Postoperative reparative granuloma (2.4% vs 0.2%,

= .001) and hydrops (1.8% vs 0.2%,

= .008) were higher in revision cases.

Revision stapes surgery was found to have less predictable and inferior results as compared with primary cases.

OBJECTIVE To test whether the ExactCare program improves medication adherence and reduces health care utilization and costs. METHODS Using a national database from a large U.S. insurer, we identified Medicare Advantage plan members in 8 states from 2007 to 2018 who had both medical and prescription drug coverage. The index year for an ExactCare patient was identified using the date of the first prescription filled by ExactCare, with the previous year being the baseline. All patients without a prescription from an ExactCare pharmacy were considered potential comparison patients. To propensity match ExactCare$226 PMPM; P = 0.008), largely attributable to decreases in hospital inpatient costs (-$119 PMPM; P = 0.001) and SNF costs (-$30 PMPM; P = 0.007). Rose has nothing to disclose.Purpose To describe eight patients with toxoplasma retinochoroiditis following exposure to wild game.Methods Retrospective, multicenter case seriesResults Eight men, aged 29 to 71 (mean, 56 years), developed toxoplasmic retinochoroiditis after hunting and/or consuming wild game in the United States, including seven deer and one bear. Five patients developed the disease after eating undercooked game meat, while three developed ocular findings after cleaning hunted animals. Seven patients were healthy prior to exposure. Disease complications developed in five (62.5%) patients, of which recurrence of retinochoroiditis was the most common.Conclusions Contact with wild game is a potential source of primary ocular toxoplasmosis in immunocompetent adults. Hunters and consumers of rare game are at risk of serious ocular disease and appropriate contact precautions and cooking may reduce this complication.
