Several predictors of RLN injury during thyroidectomy are identified, while use of intraoperative nerve monitoring was associated with a decreased risk of RLN injury. RLN injury is associated increased postoperative complications.Null.Null.Null.Null.Null.Null.The objective of study was to investigate the effect of transcranial pulsed ultrasound combined with alteplase on treatment of vascular recanalisation in acute ischemic stroke patients. Eighty-two patients were randomly divided equally into two groups. Group A was treated with transcranial pulsed ultrasound combined with alteplase intravenous thrombolysis. Group B received alteplase intravenous thrombolytic treatment. At 2 and 24 hours after treatment, National Institutes of Health Stroke Scale (NIHSS) score of group A was lower than that of group B (both p less then 0.001), and vascular recanalisation rate of patients in group A was higher than that of group B (p=0.004, and 0.002 respectively). At 90 days after treatment, rate of favourable prognosis [(modified rankin scale (mRS) ≤2 points)] of group A was higher than that of group B (p=0.001). Transcranial pulsed ultrasound combined with alteplase intravenous thrombolytic treatment can improve vascular recanalization rate and prognosis. The etiology of Behcet's disease is still unknown but genetic predisposition has been described to play a role. The disease is reported more in males than females. Neurological involvement could cause brain parenchymal, vascular or meningeal changes with other distinct systemic symptoms, which may include orogenital ulcers, uveitis and skin lesions. Parenchymal neuro-Behcet's disease has a poor prognosis. MRI aids in diagnosing and differentiating it from other entities. Treatment includes anti-inflammatory drugs and immunotherapeutics. Our case is the youngest adult reported so far and is an unusual presentation of neuro-Behcet's disease, as the patient was already in remission and on infliximab infusion. Cases of surgical field fire have been reported in literature. Usually, surgical field fire involves airway, but it can also occur in surgical field. We, hereby report a case of surgical field fire while operating on a 4-week baby for jejunal atresia. In this case report, we review causes and preventive measures for surgical field fire. Key Words Surgical field, Fire, Neonate, Prevention, Operation theatre.Parathyroid carcinoma is a rare cause of hyperparathyroidism and leads to severe hypercalcemia. The etiology is not fully known. Parathyroid cancer should be considered in the differential diagnosis, if serum calcium and parathyroid hormone levels increase, and parathyroid gland is palpable. Severe hypercalcemia is the most common cause of death in patients diagnosed with parathyroid carcinoma. Fluid replacement, diuretic therapy, bisphosphonates, and calcimimetic agents are the main treatment steps in the control of life-threatening hypercalcemia. Surgery is the primary treatment option, while denosumab is a treatment option for refractory hypercalcemia caused by parathyroid carcinoma, or for patients who are not eligible for surgery. There are few case reports in literature about denosumab treatment for parathyroid carcinoma. Herein, we report a case of a patient who presented with the complaint of leg pain and was diagnosed with parathyroid carcinoma. The elevated calcium level of the patient was controlled with denosumab. Key Words Parathyroid carcinoma, Denosumab, Hypercalcemia, Hyperparathyroidism.Although Langerhans cell histiocytosis (LCH) may arise from any bone, flat bones (skull, ribs, scapula, clavicle, and mandible) are more commonly involved.The overall reported incidence of scapular involvement by LCH is nearly 3% in the literature. Intra-lesional corticosteroid injection can be given for lesions that cause pain or postural deformity. We, herein, report a 26-year male with scapular LCH who was suffering from right shoulder pain and right arm weakness. The patient received intra-lesional methylprednisolone under CT-guidance for treatment. On follow-up imaging, almost total regression was observed. We suggest that percutaneous CT-guided intra-lesional steroid injection is an efficient method for the treatment of LCH located in bone. To describe clinical features and treatment options in pediatric patients with ALCL (Anaplastic large cell lymphoma) and their outcome over a span of 10 years.

A retrospective-observational study.

Shaukat Khanum Memorial Cancer Hospital and Research Centre, Lahore,from January 2005 to December 2015.

Medical records of pediatric patients with anaplastic large cell lymphoma was retrospectively collected after IRB approval. Data was reviewed for patients confirmed on histopathology and age less than 20 years at the time of diagnosis to see clinical features and treatment outcomes. Descriptive statistics were applied.

A total of 40 children, 27 males (67.5%) and 13 females (32.5%) with ALCL (CD30 +), were reviewed. B symptoms were present in 32 (80%) patients, nodal involvement in 39 (97.5%), and mediastinum involvement was present in 8 (20%) patients. Visceral (lung, liver, spleen) and cutaneous involvement was seen in 16 (40%) and 6 (15%) patients, respectively. ALK was positive in 19 patients (48%) and Bone marrow was involved in 3 patients (7.5%). Stage III was seen in 29 (72.5%). All patients were treated on ALCL 99 protocol. Five-year EFS (event-free survival) and OS (overall survival) was 30 and 60%, respectively. There were 7 relapses, 2 progressive disease, 16 death and 3 refusal for treatment.

This analysis shows poor outcomes in pediatric ALCL. The most common cause of mortality was hematological toxicity and febrile neutropenia associated with it. Supportive care needs to be improved. Key Words Clinical features, Outcomes, Anaplastic large cell lymphoma (ALCL), Children.

To determine the neonatal outcome with maternal idiopathic thrombocytopenia.

Descriptive study.

Obstetrics and Gynecology Department, Liaqat National Hospital, Karachi, from January to June 2017.

All 73 pregnant women diagnosed with idiopathic thrombocytopenia (ITP) during the study period were included. All the patients were kept under observation till birth of newborns. Neonates were assessed for thrombocytopenia, classified as mild, moderate and severe thrombocytopenia as outcome on the first post-natal day.

Frequency of neonatal thrombocytopenia in 73 pregnant women complicated by idiopathic thrombocytopenia was 16.44% (12 neonates). Out of 12 cases, three (25%) neonates had mild thrombocytopenia, four (33.33%) neonates had moderate thrombocytopenia and five (41.66%) neonates had severe thrombocytopenia.

To analyse the efficiency of thyroid function tests (thyroid stimulating hormone, thyroxine and tri-iodothyronine) on prediction of postoperative atrial fibrillation.

Literature review was carried out in PubMed, Science Direct and Ovid Database. No date limitations was applied. Trials, which evaluated the preoperative levels of thyroid stimulating hormone, thyroxine and tri-iodothyronine in cardiac surgery patients, were included. Only the articles in English language were reviewed. We evaluated the results with fix or random effect models according to the presence of heterogeneity (I2 >25%).

Five hundred and forty-seven articles were obtained after screening of databases. After checking over the titles and abstracts, five trials were included which covered 380 patients and complied with inclusion criteria. To find out the sero-prevalence of COVID-19 IgG antibodies among policemen performing duties at high risk areas of Lahore, Pakistan.

Cross-sectional, observational study.

Chughtai Lab Lahore from 20th to 30th May 2020.

One hundred and fifty-four young policemen (mean age 27.1 ±3.8 years) were included in the study after written consent. Squad officers who had any sign of acute respiratory infection were excluded from the study. The participants were asked to fill a questionnaire regarding nature of exposure with the infected cases, kind of personal protective equipment they are provided with and living arrangements. Three ml of blood samples were collected from the participants and immediately sent to the lab for analysis. COVID-19 IgG antibodies were analysed using chemiluminescence immunoassay method on Architect Ci8200 (Abbott). SPSS 23.0 was used for data analysis.

All participants were males and mean age was 27.1 ±3.8 years. Out of 154 squad members, 24 were reactive for COVID-19 IgG antibodies and performing duties at the areas of high virus exposure. None of COVID-19 IgG reactive cases exhibited symptoms and most of them performed duties at high risk areas with only masks provided as personal protective equipment. Only 10% of the participants who were exposed to confirmed cases of COVID-19 were found to have anti-COVID IgG.

COVID-19 IgG antibody reactive cases may not show any symptoms. Police force is at high risk of exposure. Serosurveys can help to find the spread of infection in the community and aid in planning healthcare strategies. Key Words COVID-19, SARS CoV-2, Seroprevalence, Pandemic, Asymptomatic cases.
